Resumen.-OBJETIVOS: Valorar las características clí-nicas y urodinámicas de una serie de mujeres con residuo postmiccional (micción descompensada) y diferentes grados de cistocele asociado. MÉTODOS: Se estudiaron 119 pacientes de sexo femenino mediante evaluación clínica, urodinámica, y radiológica (cistouretrografías). Todas las pacientes fueron sometidas a historia clínica, exploración física uroginecológica (valorando el cistocele, al vasalva, de grado 0 a grado 3), exploración física neurourológica y estudio urodinámico completo. A 50 pacientes (42%), se les realizó estudio radiológico del tracto urinario inferior (TUI). Se consideró micción descompensada (MD), la existencia de un residuo postmiccional (RP) mayor del 20% del volumen miccional. La resistencia uretral se midió mediante el URA. La tipificación de la obstrucción estructural se realizó mediante el PURR (clasificación CHESS). El estudio de la obstrucción funcional se realizó mediante el DURR y la EMG perineal (asociada a la flujometría libre Respecto a los datos clínicos, solo existieron diferencias significativas (DS) entre ambos grupos en la dificultad miccional. En los datos flujométricos, existieron tan solo DS en el percentil (nomogramas de Haylen), del Qmax (35 MC vs 22, MD; p=0,02). En la cistomanometría, se demostró hiperactividad vesical en la MD en 16 casos (64%) frente a 31 casos (34,8%) en la MC (p=0,008), que presentó relación con incremento de la resistencia uretral (URA) p=0,01. En el estudio P/F, se demostró DS en el valor del URA (14.7 MC vs 25.3 MD; p=0,001). Existieron DS en los grados de obstrucción constrictiva (0.5 MC vs 1.1 MD; p=0,009) y compresiva (0.02 MC vs 0.4 MD; p=0,04)(clasificación CHESS). Estos datos no presentaron relación significativa con el grado de cistocele. No existieron DS en los análisis de contractilidad isométrica (Wmax), pero sí en la contractilidad isotónica (W80-20) y duración de la contracción del detrusor. Objectives: To determine the prevalence of radiologic images suggestive of urethral diverticula (UD) in men with spinal cord injury (SCI) and to study the interobserver diagnostic reproducibility. Methods: Radiological studies (i.e. voiding cystourethrography and retrograde urethrography) performed over 1 year on men with SCI were independently reviewed by 3 researchers (1 urologist and 2 radiologists). Results: The prevalence of UD was found to be between 4.2 and 9.8% of the patients, the higher figure obtained when including also the doubtful images. The kappa index of agreement between the researchers was low (between 0.15 and 0.40). The factors that significantly influenced agreement were localization in the prostatic urethra (p = 0.021), localization in the penile urethra (p = 0.000) and fusiform morphology (p = 0.004). Logistic regression analysis showed that the variables that independently influenced diagnostic agreement were the following: localization in the penile urethra (in favor of agreement) and fusiform morphology (against agreement). Conclusions: Radiologic images suggestive of UD constitute a frequent finding in men with SCI and raise important diagnostic problems.

CONCLUSIONES: Se sigue demostrando la utilidad de dicha clasificación con la introducción de algunas modificaciones, necesarias para su continua actualización.
