Objetivo: Describir las características clínicas y el manejo de pacientes graves con COVID-19. Método: Estudio observacional, descriptivo, longitudinal y restrospectivo. Resultados: Ingresaron 56 pacientes, el 80.3% (n = 45) de sexo masculino, con un promedio de edad de 58 años. Las principales condiciones de comorbilidad fueron obesidad, hipertensión y diabetes. El tiempo de inicio de los síntomas al ingreso fue de 9 [7][8][9][10][11][12][13][14] días, siendo los más frecuentes disnea, fiebre y tos seca. Los datos de laboratorio fueron linfopenia y elevación de deshidrogenasa láctica, fibrinógeno, dímero D, ferritina y proteína C reactiva. El 100% de los pacientes requirieron ventilación mecánica, con una mediana de tiempo de ventilación de 12 [6][7][8][9][10][11][12][13][14][15][16][17] días, y el 66% (n = 37) requirieron posición en prono. El tratamiento farmacólogico fue a base de azitromicina, hidroxicloroquina, tocilizumab y esteroides, principalmente. Las complicaciones más frecuentes fueron lesión renal aguda, enfermedad tromboembólica venosa e infarto agudo al miocardio. La tasa de mortalidad fue del 17.8% (n = 10). Conclusión: Los pacientes graves en nuestro hospital fueron en su mayoría personas de la tercera edad y con obesidad, siendo las variables de mayor puntaje SOFA y lesión renal aguda las asociadas con mayor mortalidad.
Objetivo: Describir las variables ecocardiográficas de los pacientes con COVID-19 y su correlación con biomarcadores y mortalidad. Método: Estudio observacional, descriptivo, longitudinal y retrospectivo. Resultados: El 70% (n = 21) eran de sexo masculino, el promedio de edad fue de 59 (46-67) años y el índice de masa corporal fue de 30.1 (27.3-32.4) kg/m 2 . En la evaluación de la función sistólica del ventrículo izquierdo se observó una adecuada correlación entre la excursión sistólica del plano del anillo mitral (MAPSE) y la fracción de eyección del ventrículo izquierdo (r 2 = 0.508, p = 0.004). La evaluación de la función sistólica del ventrículo derecho indicó una óptima correlación entre la excursión sistólica del plano del anillo tricuspídeo (TAPSE) y la fracción de acortamiento del ventrículo derecho (r 2 = 0.649, p = ≤ 0.0001). No hubo correlación de los biomarcadores y las variables ecocardiográficas entre los pacientes sobrevivientes y no sobrevivientes, a excepción del MAPSE y las concentraciones séricas de deshidrogenasa láctica (r 2 = −0.427, p = 0.019). La tasa de mortalidad fue del 16% (n = 5). Conclusión: Existe una adecuada correlación entre las variables ecocardiográficas de la función sistólica del ventrículo izquierdo y del ventrículo derecho, así como también entre la MAPSE y los valores séricos de DHL.
RESUMEN.La diabetes mellitus se ha convertido en un problema de salud pública en el mundo, ya que su incidencia y prevalencia se ha incrementado de manera exponencial en las últimas décadas, lo que ha traído consigo un aumento en las complicaciones agudas y crónicas asociadas a su diagnóstico tardío y poco apego a su tratamiento. Los pacientes con diabetes mellitus tienen una mayor probabilidad de requerir una cirugía en comparación con la población general, quienes podrían llegar a presentar complicaciones graves asociadas al evento quirúrgico como hipertensión, cardiopatía isquémica, nefropatía y neuropatía autonómica, las cuales se deben a un incremento de la resistencia a la insulina y a la hiperglucemia. Por lo anterior, el control metabólico es pieza clave en la conducta perioperatoria del paciente con diabetes mellitus donde la participación de un equipo multidisciplinario (médico internista, cirujano y anestesiólogo) definan de manera conjunta las mejores estrategias quirúrgicas y anestésicas para un mejor desenlace previo, durante y posterior a la cirugía.ABSTRACT. Diabetes mellitus has become a public health problem worldwide, its incidence and prevalence have exponentially increased in recent decades, bringing with it an increase in acute and chronic complications associated with late diagnosis and failure of adherence to treatment. Patients with diabetes mellitus are more likely to require surgery compared to the general population, they may have serious complications associated with the surgical event such as hypertension, ischemic heart disease, nephropathy and autonomic neuropathy, due to an increase in insulin resistance and hyperglycemia. Therefore, metabolic control is a key element in the perioperative behavior of patients with diabetes mellitus where the joint collaboration of a multidisciplinary team (internist, surgeon and anesthesiologist) to define the best surgical and anesthetic strategies for a better outcome before, during and after surgery.
review of clinical guideline implementability. Two analysts independently evaluated each PEM to determine how design principles were applied. Results Though the sample consisted of PEMs designed and developed to influence care, no single PEM scored well across all categories. Some PEMs failed to differentiate major recommendations and did not present them in a stepwise fashion. Most used clear and easy to read text, but highlighting was often inappropriate. Some algorithms lacked logic and consistency. Images were poorly designed and used, which may distract and confuse the reader. Discussion Design principles are not consistently applied in the development of PEMs and improvements are needed to images, presentation of recommendations, and usability of algorithms. Improvements to the design of PEMs may influence their uptake by combating information overload and increasing their perceived ease of use and perceived usefulness. Implications for Guideline Developers/Users Those who create guidelines and other PEMs consider some design principles, but do not implement them consistently. Our checklist can assist guideline developers in employing a range of design principles. Background The use of the recommendation of clinical practice guidelines (CPGs) by health professionals, depends on the diffusion process and local strategies of implementation of a particular guide in a specific service of the institution. Objectives Assess the adherence to the recommendations of CPGs by health professionals internationally. Methods A systematic review of the literature in PubMed was conducted (MeSH term 'Guideline Adherence', filters: published in the last 5 years, meta-analysis). Results Out of 33 documents that were obtained, seven were selected, one systematic review and one document in google academic (Mexico). The percentage of adherence differs markedly depending on the directory in question and on the professionals involved from 61.1 to 72.2%. The median adherence was 45%. The professionals with the greatest adherence were dentists, whereas cardiologists and surgeons did not change their behaviour due to the recommendations of a CPG. Discussion The degree of adherence to the recommendations of the CPG is influenced by different factors, related to the efforts of professional associations, the management of health care organisations, the professionals themselves involved in the care of the patient and the patient himself. Implications for Guideline Developers/Users The key elements for adherence to the recommendations of the CPG are: involvement of the professionals with the strategy, occupational type, and suggested recommendations.
