Introducción: Cuando el dolor asociado a deformidad en hallux valgus es el principal síntoma y el tratamiento conservador no es eficaz, está indicada la cirugía. El objetivo de este artículo fue evaluar nuestra experiencia con la técnica mínimamente invasiva y la osteotomía tipo scarf, mediante la escala AOFAS, radiografías con la corrección del ángulo intermetatarsiano, el ángulo metatarsofalángico, la escala Regnauld y los criterios de Reynolds.Materiales y Métodos: Estudio observacional descriptivo de tipo cohorte retrospectiva. Se incluyeron los pies operados mediante las técnicas mínimamente invasiva y tipo scarf. Se revisaron los datos clínicos preoperatorios, y a los 3 y 6 meses de la cirugía, sobre funcionalidad, hallazgos imagenológicos de tres ángulos, y las clasificaciones de Reynolds y Regnauld.Resultados: Se incluyeron 123 pacientes en quienes se realizaron 150 procedimientos: cirugía mínimamente invasiva (44,7%) y tipo scarf (55,3%). La mediana de edad era de 56 años (RIC 42-61), el 88,6% eran mujeres. No hubo diferencias entre el preoperatorio inmediato, y a los 3 y 6 meses para el ángulo intermetatarsiano entre las dos técnicas; sin embargo, el ángulo metatarsofalángico se corrigió (p <0,001) y el puntaje AOFAS fue mejor a los 3 meses con la cirugía mínimamente invasiva (p <0,001).Conclusiones: Ambas técnicas corrigen la deformidad a valores normales. Con la técnica mínimamente invasiva, la corrección fue mayor y el puntaje AOFAS resultó mejor. La incidencia de complicaciones agudas fue similar. Se necesitan ensayos clínicos sobre este tema para confirmar los hallazgos de este estudio.
ResumenLa luxación tibioperonea proximal es poco común, se relaciona con lesiones deportivas, como consecuencia de un trauma en rotación con el pie en inversión y flexión plantar, la rodilla en flexión y la pierna en rotación externa simultáneamente. Provoca dolor en la región lateral de la rodilla y la pierna, con arcos de movilidad completos de la rodilla, pero dolorosos. El diagnóstico es clínico y radiológico. Es una entidad que pasa inadvertida en el Servicio de Urgencias (60%), con consecuencias funcionales y biomecánicas si no se diagnostica oportunamente. El objetivo es describir la experiencia diagnóstica y el tratamiento ortopédico de una entidad inusual en trauma de rodilla. Se presenta a un paciente de 23 años que sufrió una caída patinando y refiere protuberancia proximal en la cara lateral de la pierna izquierda, edema, dolor y limitación para extender la rodilla.Palabras clave: Luxación; articulación tibioperonea proximal; diagnóstico. Nivel de Evidencia: IVAcute traumatic dislocation of the proximal tibiofibular joint: case report and review of the literature Abstract Proximal tibiofibular dislocation is uncommon, and it is related to sport injuries as a result of an external rotation trauma with the knee in fully flexed position, and foot pointing inwards and downwards. It causes pain in the anterolateral aspect of the knee, motion is complete but painful. Diagnosis is clinical and radiological. This entity goes unnoticed in the emergency department (60%), with functional and biomechanic consequences if diagnosis is no immediate. The objective is to describe diagnostic experience and orthopedic treatment of an unusual knee condition. We present a 23-year-old patient who fell while skating. He refers a proximal protuberance in the lateral face of the left leg, edema, pain, and limitation for knee extension.
La osteomielitis crónica recurrente multifocal es un trastorno inflamatorio estéril en las extremidades. Afecta principalmente a niños simulando una osteomielitis infecciosa. Puede estar asociada a trastornos inflamatorios cutáneos y del tracto intestinal. Los niveles de reactantes de fase aguda están normales o levemente altos. Las radiografías muestran lesiones osteolíticas rodeadas por esclerosis, pero pueden ser normales al inicio de la enfermedad. Identificar las características propias de la enfermedad y descartar una etiología infecciosa permiten establecer un diagnóstico oportuno y ofrecer el manejo adecuado.Se presentan tres casos de esta patología. Un hombre de 24 años con síntomas inflamatorios persistentes y recidivantes en el tobillo derecho, de 8 años de evolución. Una niña de 11 años que acude reiterativamente por dolor en hemitórax, clavícula derecha y tobillo izquierdo, y una niña de 12 años con múltiples episodios breves de síntomas inflamatorios en ambos tobillos. Los tres casos, sin leucocitosis, reactantes de fase aguda levemente aumentados o negativos, con criterios mayores y menores de Jasson para diagnóstico de esta patología. Los cuadros mejoraron con antinflamatorios no esteroides. Es importante conocer esta rara entidad como posibilidad diagnóstica, pues un diagnóstico oportuno evita el uso innecesario de antibióticos y la resolución del cuadro se logra con antinflamatorios no esteroides, sin necesidad de procedimientos quirúrgicos que aumentan las comorbilidades.
<p><strong>Introducción: </strong>La displasia acromesomelica tipo Grebe es un desorden raro autosómico recesivo que pertenece al grupo de las osteocondro displasias. Clínicamente se caracteriza por un severo dismorfismo con una marcada micromelia y deformidad de las extremidades inferiores y superiores. Conocer este tipo de síndrome orienta a dar mejores diagnósticos y permite hacer el diagnóstico diferencial con patologías más comunes como la acondroplastia. <strong></strong></p><p><strong>Reporte de caso: </strong>Paciente de 35 años de sexo femenino, peso de 40 kg y talla de 100cm, con diagnóstico de síndrome de Grebe desde los 10 años, con hermana y dos tías en segundo grado en la línea materna con el mismo síndrome. Entre las manifestaciones clínicas del síndrome de Grebe encontramos enanismo severo (100cm), micromelia y deformidad en miembros superiores e inferiores con diferentes grados de severidad. La apariencia facial e inteligencia son normales, con un esqueleto axial normal, miembros superiores e inferiores con segmentos proximales cortos y el medio aún más cortos.<strong></strong></p><p><strong>Discusión: </strong>Conocer este tipo de síndrome orienta a dar mejores diagnósticos y permite hacer el diagnóstico diferencial con patologías más comunes como la acondroplastia. Los pacientes con acromesomelia tipo Grebe tienen complejas anomalías en el esqueleto periférico.</p><p><strong>Conclusión</strong>: El síndrome de Grebe presenta una muy baja incidencia por lo cual es poco conocido por el cuerpo médico en general y aún menos para los ortopedistas, quienes por tener contacto con patologías musculo esqueléticas tendrán que ser los encargados de manejar estos pacientes. </p>
<p><strong>Introducción</strong></p><p><strong></strong>Los encondromas solitarios son los tumores más comunes de la mano. Aproximadamente el 40% comprometen esta extremidad,con predilección por los rayos cubitales y las falanges proximales. El riesgo de malignización a condrosarcoma es de 1%y de recurrencia entre el 2 al 15%.Teniendo esto en cuenta se describe la experiencia y resultadosde un importante número de pacientes con un seguimiento a largo plazo.</p><p><strong>Material y métodos</strong></p><p>Estudio descriptivo, retrospectivo, tipo serie de casos. Se seleccionaron pacientes con diagnóstico confirmado de encondroma solitario de la mano que fueron manejados mediante resección con curetaje del tumor más aplicación de injertos autólogos de cresta iliaca o radio. Después del tratamiento quirúrgico, se evaluó la función mediante la clasificación Takigawa, los resultados radiográficos y la frecuencia de complicaciones o recidivas.</p><p><strong>Resultados</strong></p><p>Se incluyeron 19 pacientes con un seguimiento postoperatorio promedio de 11 años.<strong> </strong>De acuerdo a la clasificación de Takigawa la función fue excelente en 16 pacientes (84,2%) y buena en 3 (15,7%). Como complicación un paciente presentó infección del sitio operatorio incisional superficial, con resolución completa. No se presentó ningún caso con recidivas.</p><p><strong>Conclusión</strong></p><p><strong></strong>En los pacientes evaluados, se presentaron buenos resultados funcionales y radiológicos después del tratamiento quirúrgico. Los encondromas pueden presentar un riesgo de fracturas patológicas, recidiva y en menor proporción malignización, sin embargo esto no ocurrió en ninguno de los casos. Se describen los resultados de una opción de manejo quirúrgico, sencilla con buenos y excelentes resultados a largo plazo.</p><p> </p>
