BackgroundKnowledge translation (KT) is an imperative in order to implement research-based and contextualized practices that can answer the numerous challenges of complex health problems. The Chronic Care Model (CCM) provides a conceptual framework to guide the implementation process in chronic care. Yet, organizations aiming to improve chronic care require an adequate level of organizational readiness (OR) for KT. Available instruments on organizational readiness for change (ORC) have shown limited validity, and are not tailored or adapted to specific phases of the knowledge-to-action (KTA) process. We aim to develop an evidence-based, comprehensive, and valid instrument to measure OR for KT in healthcare. The OR for KT instrument will be based on core concepts retrieved from existing literature and validated by a Delphi study. We will specifically test the instrument in chronic care that is of an increasing importance for the health system.MethodsPhase one: We will conduct a systematic review of the theories and instruments assessing ORC in healthcare. The retained theoretical information will be synthesized in a conceptual map. A bibliography and database of ORC instruments will be prepared after appraisal of their psychometric properties according to the standards for educational and psychological testing. An online Delphi study will be carried out among decision makers and knowledge users across Canada to assess the importance of these concepts and measures at different steps in the KTA process in chronic care.Phase two: A final OR for KT instrument will be developed and validated both in French and in English and tested in chronic disease management to measure OR for KT regarding the adoption of comprehensive, patient-centered, and system-based CCMs.DiscussionThis study provides a comprehensive synthesis of current knowledge on explanatory models and instruments assessing OR for KT. Moreover, this project aims to create more consensus on the theoretical underpinnings and the instrumentation of OR for KT in chronic care. The final product--a comprehensive and valid OR for KT instrument--will provide the chronic care settings with an instrument to assess their readiness to implement evidence-based chronic care.
C'est au cœur du projet de recherche-action participative « Vers une autonomie alimentaire pour tou-te-s : agir et vivre ensemble le changement » (VAATAVEC) que s'est élaborée une définition nouvelle et évolutive de l'autonomie alimentaire. De ce projet regroupant des personnes en situation de pauvreté, chercheurs et intervenants, a émergé une conceptualisation résultant à la fois d'une méthode de réflexion collective et d'un processus d'analyse collective, inspiré de la théorisation ancrée et de l'analyse conceptualisante de Paillé et Mucchielli (2003). Le recours à l'expertise des personnes en situation de pauvreté, expertes du vécu de l'insécurité alimentaire, a contribué à ancrer cette définition dans les causes structurelles de l'insécurité alimentaire dans une perspective de développement d'un pouvoir d'agir collectif, de toutes les personnes concernées. Il en résulte une définition de l'autonomie alimentaire d'où ressort une contribution surtout méthodologique et théorique.
La haute prévalence du diabète de type 2 (DT2) chez les Innus est indissociable des transformations de l’acte alimentaire émergées dans leurs communautés depuis le milieu du XXe siècle. Dans cet article, nous proposons que ces transformations s’inscrivent dans une trame identitaire collective, façonnée dans le contexte de la globalisation et de l’exclusion au travers de rapports sociaux prévalant au sein des communautés innues. Ces transformations ont permis l’inclusion dans la trame identitaire de nouveaux aliments originalement associés à la culture dominante mais qui, rapidement, sont devenus des éléments de la culture locale. Dans cet article nous situons d’abord l’émergence des transformations de l’alimentation des Innus dans des rapports politiques et socioéconomiques du milieu du XXe siècle pour mieux saisir certaines règles d’inclusion et d’exclusion qui caractérisent leur acte alimentaire contemporain. L’évolution de leurs codes alimentaires comme marqueurs de l’« identité innue » est ensuite discutée. Pour conclure, nous suggérons que les transformations de l’acte alimentaire des Innus reflètent, au niveau microsociétal, l’évolution plus globale des dynamiques sociopolitiques entre Autochtones et non-Autochtones et le durcissement des discours identitaires en réaction à l’exclusion sociale.
