Il s'agit d'une étude rétrospective portant sur les 30 premiers cas d'ostéonécrose associée au traitement par BPs diagnostiqués dans la Division de Stomatologie, Chirurgie orale et Radiologie dento-maxillo-faciale de l'Ecole de Médecine dentaire de l'Université de Genève. Les caractéristiques cliniques, radiologiques et histologiques des ostéonécroses sont précisées ainsi que leur évolution sous traitement. L'ostéonécrose des maxillaires représente le principal effet indésirable d'un traitement par aminoBPs mais on doit distinguer les formes rencontrées en oncologie de celles survenant dans le traitement de l'ostéoporose. Dès le début, nous avons adopté une approche thérapeutique raisonnée et moins passive que celles conseillée par l'ensemble des auteurs: dans les formes sévères, cette prise en charge a toujours permis au minimum de ralentir l'évolution et de contrôler la symptomatologie assurant ainsi une qualité de vie correcte aux patients.
