Objective To analyze the strengths and limitations of the Family Health Strategy (ESF) from the perceptions of healthcare professionals and community. Methods Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was applied to evaluate the ESF with 77 healthcare professionals and 293 caretakers of children under five. Health professional ESF training, community access to care, patient communication, and delivery of health education and pediatric care were of main interest in the evaluation. Logistic regression analysis was used to obtain odds ratios (OR) and 95% confidence intervals (CI). Results The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caretakers and professionals identified similar weaknesses (services not accessible to the caretakers, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caretaker satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caretakers reported being generally satisfied with the ESF services. Conclusions Identifying the limitations and strengths of the ESF from the healthcare professional and caretaker perspective may serve to advance primary community healthcare in Brazil.
Objective To assess factors influencing perspectives on Brazil’s national Family Health Program (PSF) by exploring satisfaction with PSF units and home-visit community health agents and perceptions about PSF unit accessibility among frequent users (primary caretakers of children under age 5) in Vespasiano, Minas Gerais. Methods Data were collected though cross-sectional household surveys to determine programmatic and demographic factors affecting user satisfaction with the PSF. Multivariate logistic modeling was used to estimate users’ satisfaction with PSF units and agents and perceived access to PSF unit services. Chi-square and analysis of variance (ANOVA) tests were used to estimate statistical differences. Results The majority of caretakers were satisfied with both their PSF unit and their PSF community health agent and had received at least one monthly home visit from the health agent. Satisfaction with both the health agent and the unit was positively associated with perceived access to the unit and frequency of agent home visits. Caretakers who reported that their PSF agent made one or more home visits per month were more likely to perceive the PSF unit as being “accessible” (or “sometimes accessible”). Conclusions The current data are important indicators of population health in Minas Gerais, Brazil, and suggest that users’ satisfaction with the PSF and perceptions about its accessibility can be improved by ensuring that all households receive at least one health agent home visit per month. These results could be applied to other parts of Brazil or Latin America to improve understanding of user perceptions of health systems.
PCR was used to amplify a targeted region of the ribosomal DNA of 76 Candida spp. isolates from immunocompromised and seriously diseased patients. Thirty-seven strains isolated from different anatomical sites of 11 patients infected with HIV (Vitória, ES, Brazil), 26 isolates from patients under treatment at Odilon Behrens Hospital and 13 isolates from skin and urine samples from São Marcos Clinical Analysis Laboratory (Belo Horizonte, Brazil) were scored. Fragments of rDNA were amplified using primer pairs ITS1-ITS4, for the amplification of ITS1 and ITS2 regions, including the gene for the 5.8 s subunit. Amplification resulted in fragments ranging in size from 350 to 950 bp. Amplicons were digested with eight restriction enzymes. A pattern of species-specificity among the different medically important Candida species could be identified following restriction digestion of the PCR products. Candida albicans was the species most frequently observed, except for the group of newborns under treatment at the Odilon Behrens Hospital and for the isolates from the clinical analysis laboratory. C. parapsilosis was the species most frequently observed in these two groups.
ARAÚJO, P. C.; SALIBA, M. T. A.; CONSOLARO, R. B. & GARBÍN, C. A. S. Pemphigus vulgaris in adolescence: a case report. Int. J. Odontostomat., 7(2):215-220, 2013.ABSTRACT: Pemphigus vulgaris is an autoimmune disease, normally associated with oral lesions that generally affects patients aged 40 -60 years. The aim of this study was to report a case of pemphigus vulgaris presenting at an uncommon age with advanced periodontal disease, and to describe the treatment performed from the time of diagnosis through oral environment restoration and follow-up monitoring. One gingival vesicular lesion was biopsied. After the histological confirmation of pemphigus vulgaris, the patient was referred to a specialist for treatment of this systemic disease. With the improvement of the patient's clinical condition, teeth were extracted and oral rehabilitation was performed. The patient is being followed and is taking the appropriate medication. A direct immunofluorescence test was performed, but the patient had already started treatment with immunosuppressive medication at the time of the perilesional biopsy. The result of this test was negative. Pemphigus vulgaris manifests as extremely painful and persistent oral lesions. Thus, patient motivation to maintain oral health, preventive measures and the promotion of oral health, should be prioritized to avoid the worsening of the patient's systemic condition. We present here some discussions about the appointed bodies transvestites, and its historical and social crossings, emphasizing the discursive and essentialist influences that invented them. Briefly Pontuaremos the subjective process, in which both can produce crops in masses, normalizing and standardizing uniform identities, how can distinguishes them, enabling new experiences, trials, desires and pleasures. Then we will discuss about sex system/gender/sexuality and the creation of heteronormative society, pointing out that this system is not at all natural, being strategically deployed to keep the power and hegemony. In this sense, we propose in this article, discuss the psychological violence experienced by transvestites in public health institutions, institutions which in itself should eradicate all forms of violence, but end up contributing to its intensification. The aim of this study was to identify the teaching discipline of Implantology in all the dental schools in Brazil and report the 9 years of experience after the induction of this material in a teaching institution. The universal sample consisted of dental schools accredited by the Federal Council of Dentistry (n= 198). The results were analyzed with the SPSS Statistics IBM program. It was observed that the discipline of Implantology is offered by 26.9% of the institutions in the fourth year of the undergraduate course. The format of the lessons is 83.3% theory and laboratory. It is also important to note that 2.5% of the institutions do not offer Implantology as a discipline, and in 33.3% of the cases, it appears as a mandatory subject. In order to report the experience, it was observed that the patients' quality of life increased after implantation, and 69.8% classified mastication as excellent after placement of the implant element. Thus, the format offered in the discipline of Implantology proved quite variable according to each institution. The high levels of satisfaction with dental implants and the increased demand for this procedure allowed a suggestion for a possible standardization of the discipline.
