Resumen Introducción: La trombosis venosa mesentérica superior es una enfermedad rara que debuta con sintomatología inespecífica. Su diagnóstico en estadios iniciales es fundamental para instaurar tratamiento anticoagulante precoz y si precisa, tratamiento endovascular para evitar cirugía. Métodos: Presentamos el caso de un paciente de 42 años acude por dolor abdominal. En la exploración destacaba dolor en hemiabdomen derecho, sin irritación peritoneal y estable hemodinámicamente. La analítica mostraba 22.540leucocitos/mm3. El TAC presentaba trombosis en la vena porta y mesentérica superior y engrosamiento del íleon con débil realce de pared. Ante la estabilidad se inició tratamiento anticoagulante y posteriormente trombectomía mecánica, manteniendo un catéter para fibrinolisis. Resultados: A las 72horas del alta hospitalaria acude a urgencias por dolor abdominal. Se realizó TAC abdominal objetivando perforación intestinal y persistencia de la trombosis. Se decidió laparotomía urgente. Presentaba perforación y placas de necrosis intestinal. Se realizó resección intestinal. Discusión: La trombosis venosa mesentérica aguda es una forma rara pero grave de isquemia mesentérica. La etiología puede ser identificada en el 75% de pacientes. Su presentación clínica varia ampliamente, lo que dificulta el diagnóstico y retraso del tratamiento. Las mejoras en las técnicas de imagen permiten un diagnóstico precoz y un tratamiento no quirúrgico. Aunque no hay un consenso, el objetivo primordial es evitar el infarto transmural, la perforación y peritonitis. Aunque no hay consenso en el tratamiento de la isquemia venosa mesentérica hay series que describen el manejo endovascular con trombectomía mecánica y farmacológica como medidas iniciales para evitar la resección intestinal con resultados alentadores.
Resumen Introducción: la fístula anal se define como un trayecto que comunica el interior del canal anal con la piel perianal. Su origen puede ser multifactorial. Su complejidad viene determinado por la relación con la musculatura esfinteriana y factores interpersonales (enfermedad inflamatoria intestinal, síndrome de inmunodeficiencia adquirida, etc.). El tratamiento de elección es quirúrgico, existiendo gran variedad de técnicas (fistulotomía, fistulectomía, setón, LIFT, etc.), con posibilidad de complicaciones importantes (incontinencia). Material y métodos: estudio descriptivo de los pacientes diagnosticados de fístula perianal desde 2015 a 2017 registrándose aspectos demográficos, tipo de fístula, tratamiento, complicaciones postquirúrgicas y régimen de intervención (ingreso vs. cirugía mayor ambulatoria). Resultados: se incluyeron 151 pacientes, sometidos a 176 procedimientos, mayoritariamente fueron varones (74,2%) con mediana de edad de 48 años (DS±13,605). La mayoría de las fístulas se encontraron en el cuadrante inferior izquierdo con paciente en posición de litotomía, predominando el trayecto radial y un mayor porcentaje de diagnóstico postoperatorio de fístulas transesfinterianas de canal anal bajo. La técnica quirúrgica más empleada fue la fistulotomía y/o fistulectomía, seguido de colocación de setón (especialmente en el caso de fístulas complejas). El manejo del procedimiento en el 89,8% de los casos fue en régimen de cirugía mayor ambulatoria. Conclusiones: la fístula perianal es una patología prevalente compleja pero que en manos de cirujanos proctológicos es factible en régimen de CMA. La fistulotomía y fistulectomía asociada o no a la colocación de setón, siguen siendo las técnicas de elección, con baja tasa de complicaciones.
Resumen La hernia inguinal es una patología muy frecuente a nivel mundial, por lo que su reparación debe ser lo menos dolorosa posible y con la menor tasa de complicaciones. La cirugía endoscópica de la hernia inguinal aporta todas las ventajas de la cirugía sin tensión, por lo que acelera la reincorporación de los paciente a su vida normal. Las guías clínicas actualizadas recomiendan la reparación endoscópica de la hernia inguinal siempre que haya suficiente experiencia. Las técnicas endoscópicas requieren una curva de aprendizaje larga siendo recomendable estar tutorizados y realizar una cirugía sistemática.
Resumen Mujer de 73 años con antecedentes de hipertensión arterial e hipertrigliceridemia. Acude a urgencias por dolor abdominal, náuseas y vómitos de 5 días de evolución. Presenta regular estado general, hipotensión, taquicardia y en la exploración física dolor abdominal generalizado con irritación peritoneal en mesogastrio. En la analítica destaca 20600 leucocitos/µL, proteína C reactiva 524 mg/L. Se realiza TAC abdominal urgente que describe masa en segmento yeyunal localizado en hipogástrico con engrosamiento y realce parietal, burbujas de neumoperitoneo y líquido libre, sugestivo de perforación. Se decide intervención quirúrgica urgente, se realiza laparotomía media infraumbilical con presencia de masa de 10 cm dependiente de trompa izquierda que infiltra un segmento de intestino delgado inferior al centímetro, procediéndose a salpingooforectomía izquierda y resección intestinal atípica. La evolución clínica en el postoperatorio fue favorable. El resultado anatomopatológico fue: tumor del estroma gastrointestinal (GIST) bien delimitado, con tamaño de 10x8x7cm, CD117+ y recuento mitótico <5 copias. Los criterios de consenso clasifican en tres grupos en función del riesgo: muy bajo, bajo, intermedio y riesgo alto. Está basado en tamaño, índice mitótico y localización. Hay peor pronóstico a mayor tamaño, mayor recuento mitótico y con localización intestinal. Se ha descrito que la ruptura tumoral se considera como micro metástasis peritoneales. Se presentó en comité oncológico y dado el tamaño tumoral, índice mitótico, localización y forma de debut se decidió quimioterapia adyuvante con imatinib.
