Renal cell carcinoma is the third most common urogenital neoplasia (1). The lung and bone tissues represent the most frequent metastatic sites of renal cell carcinoma (2). Metastases in the stomach are rare and have been essentially described during autopsies (3). In this paper, we present a case of large metastatic gastric tumor whose origin was a renal cell carcinoma treated years ago. Case reportA 56 year old woman was diagnosed with stage one renal adenocarcinoma 6 years ago. She was treated with right radical nephrectomy and adjuvant chemotherapy. Three years later was diagnosed and treated of brain metastasis located in the frontal lobe, as well as in lung. A year ago, she came to our hospital for upper gastrointestinal bleeding. She presented acceptable general condition and nutritional, blood pressure 120/70 mmHg and normal cardio-respiratory auscultation. The abdomen was soft, non-tender, and no masses or organ enlargements. The analytical study revealed 6.3 g/dl hemoglobin. A thoracic abdominal TAC showed a large mass in the body and antrum stomach of 11 cm diameter without invasion adjacent structures. An upper gastrointestinal endoscopy revealed a large neoplasm from body to gastric antrum with mamelonated aspect. The patient underwent subtotal gastrectomy. The postoperative course was uneventful. The pathology and immunohistochemical study showed metastasis of kidney carcinoma ( Fig. 1). DiscussionMetastatic gastric cancer is uncommon. The most frequent location sites of cancer cells are in the body and gastric fundus, and single tumors predominate against multiple. Although generally gastric metastases account for 0.2-0.7% of stomach tumors, the metastatic tumor from carcinoma renal cells is extremely rare (3) and is an event late. The gastric metastases from renal cell carcinoma are diagnosed years after the primary tumor (4). The histological diagnoses require immunohistochemical analysis for differential diagnostic. We studied AE1/AE3 and vimentin markers, and the most recent CD10 and renal cell carcinoma markers (RCC-Ma). CD10 is a cell surface enzyme expressed in several types of normal cells including the brush border of renal tubular epithelial cells. Positivity for this marker is seen in more than 90% of renal clear carcinomas (5). RCC-Ma is a monoclonal antibody against a normal renal proximal tubule antigen. RCCMa expression is relatively specific for primary clear cell in renal carcinoma (6). In our case, the markers confirmed the diagnosis of renal carcinoma. The classic vimentin and AE1/AE3 markers were strongly positive, while CD10 and RCC-Ma showed moderately and weakly positive staining, respectively. The treatment of gastric metastases is controversial. The patients have poor prognosis with frequent extragastric metastases, and the treatment is endoscopic therapy and arterial embolization (7,8). It is imperative to identify and give voice to the Nursing personnel that have remained in the first line of care to the population during epidemics and pandemics. Presenting the particular needs of these professionals through international perspectives will allow proposals to be made in the regional scope to improve the working conditions.Cross-border research studies should also focus on one of the main actors suffering the effects of the pandemic (health personnel, and nurses in particular), as one of the groups most affected by all the changes required to face a problem of this magnitude, as safety of their practice must be guaranteed as part of their work, which is provision of care to the population 1 .However, in almost all of Latin America, the preparation to face the COVID-19 pandemic was insufficient and late, reason why, unfortunately, there were many human losses, both among the population and among the health professionals who did not manage to survive 2 . It should be noted that at least three Latin American countries (Brazil, Mexico and Peru) were among the five nations in the world with the highest mortality rates 3 . HISTORICAL
La enfermería reconocida como disciplina profesional cuenta con conocimientos dirigidos al cuidado de la vida humana, su desarrollo la ha diferenciado de otras disciplinas dándole visibilidad particular desde cuatro roles: gestión, enseñanza, investigación y atención clínica. Para lo cual se generó una reflexión sobre los impactos positivos y negativos del evolucionar de enfermería en América Latina ante estos roles, desde la gestión se evidencia avances importantes desde el liderazgo gremial y organizacional, pero retrocesos en el escalamiento político, e institucional. Existe brecha de la formación profesional en comparación a otras regiones, aun así, el crecimiento de programas educativos ha dado la posibilidad de la profesionalización, pero debate el reto de la calidad, la disparidad formativa y la oferta centrada en el paradigma clínico. En el campo investigativo se resalta la creación de programas doctorales y post graduales que amplían la generación de conocimiento, pero también se evidencian barreras en la transferencia y visibilidad colectiva del conocimiento. En el área clínica espacio con mayor abordaje de las acciones formativas y laborales del profesional, cuenta con grandes retos en su crecimiento, organización, escalamiento, seguridad y precariedad laboral. En esa perspectiva la comunidad U-Nursing LatAm se ha integrado para aportar a la región en estos retos. Finalmente se concluye que la mayoría de los desafíos son relacionados con las habilidades sociales y blandas que deben potencializarse en el plano personal y colectivo. Todas las situaciones requieren tener una valoración directa de las posibilidades de fortalecer y mejorar la enfermería desde cada país de la región, teniendo en cuenta los cuatro roles.
<p class="INNOVAESTIC-Palabrasclave"><strong>Introducción. </strong>Los procesos de liderazgo en la enfermería en el mundo y Latinoamérica han estado siempre presentes, representados por espacios organizacionales como el Consejo Internacional de Enfermería – CIE, Federaciones y Asociaciones. <strong>Desarrollo. </strong>Los avances en cada uno de los países de la región se han dado en diferentes épocas; Brasil, Colombia y México en la primera mitad del siglo XX y el resto de las naciones luego de los años 50´s. En todas las organizaciones un aspecto de importancia es el liderazgo, dinamizado por las Asociaciones Nacionales de Enfermería, Federaciones y Colegios, de igual forma estos espacios han reconocido el surgimiento más reciente de sociedades, organizaciones y colectivos que de manera conjunta y/o independiente trabajan por la enfermería en cada país. La nueva era de las tecnologías de la información y comunicación que mueven el mundo, llevan a dinámicas organizaciones virtuales y en red, en ese ejercicio Growing Up Foundation entre 2015 y 2019 ha generado nuevas estrategias para aportar a las dinámicas de cambio permanente desde la gestión, el cambio, la investigación y el desarrollo social en el caso particular de la profesión y brinda el espacio denominado campaña U-Nursing Latam. <strong>Conclusión.</strong> Las dinámicas de redes abiertas, flexibles, democráticas y participativas, son tendencias actuales que permiten dar los espacios para poder crecer y trascender, desde el reconocer las necesidades para fortalecer las capacidades que tienen cada uno de los enfermeros, partiendo desde sus propias experiencias de vida.</p>
