Lactating adenoma is a rare benign breast lesion that most often presents as a small (up to 3 cm), solid, well-circumscribed, solitary, painless, mobile, lobulated mass. The highest incidence occurs in primiparous women (20 to 40 years old) during the third trimester of pregnancy. However, in the rare case presented herein, in addition to its giant size (more than 10 centimeters on palpation), this lactating adenoma is distinctive due to the presence of multiple nodules, poorly defined ultrasonographic margins, worrisome radiologic features, growth since early pregnancy, presence of infarction and association with chronic mastitis. From the clinical-radiologic perspective, the differential diagnoses included abscess associated with puerperal mastitis, phyllodes tumor, and galactocele. Biopsy was performed, and pathologic examination revealed the classic characteristics of lactating adenoma with multiple infarcted areas, leading to an unexpected confirmed case of giant lactating adenoma.
Introdução: O câncer de mama associado à gravidez é uma situação desafiadora, de manejo delicado, em função do dilema entre a terapia ideal para a mãe e o bem-estar do feto. O diagnóstico frequentemente é tardio e dificultado pelas alterações fisiológicas próprias da gestação, de modo que tende a apresentar estágio mais avançado. O objetivo do tratamento, em geral, é o controle locorregional e sistêmico da doença a fim de reduzir a morbimortalidade fetal e materna. Relato de caso: M.C.A., 33 anos, G3P2CA1, sem comorbidade prévias, iniciou acompanhamento com mastologista e pré-natal de alto risco no Hospital Universitário Cassiano Antônio Moraes, com 19 semanas e três dias. A gestação e o carcinoma foram constatados simultaneamente, após consulta em razão de dor e aparecimento de nódulo em mama esquerda, confirmados na 11ª semana gestacional. A ultrassonografia mamária evidenciava nódulo sólido hipoecoico (BI-RADS 3) e o exame histopatológico mostrou carcinoma mamário invasivo do tipo ductal infiltrante, associado a carcinoma papilífero intracístico grau 3 do tumor de Nottingham. No serviço, apresentava queixa de dor intensa em mama esquerda e, ao exame físico, nódulo palpável de 10 cm em quadrante superior lateral da mama esquerda e nódulo de 1 cm fixo e endurecido em axila esquerda. A paciente foi submetida a quimioterapia (QT) neoadjuvante com antraciclina e taxanos e, após oito sessões, compareceu à maternidade com perda de líquido vaginal, sendo indicada a interrupção por cesárea com 33,5 semanas. Retornou ao consultório com um mês de puerpério apresentando crescimento tumoral e intensa dor. Propôs-se cirurgia antes do fim da QT neoadjuvante pela resposta insuficiente e pela piora clínica local. A paciente foi submetida a mastectomia radical esquerda com esvaziamento axilar, sem intercorrências. Constatou-se carcinoma ductal in situ sem comedonecrose, medindo 6,5x4 cm, com múltiplos focos de invasão capsular de até 1 cm. Estadiamento anatomopatológico pT3N2a. Presença de metástases em quatro de 10 linfonodos, medindo até 1 cm, sem extensão extracapsular. Paciente mantém tratamento com radioterapia adjuvante e seguimento com mastologista e oncologista assistente. Conclusão: A grande maioria das gestantes com câncer de mama é candidata à quimioterapia sistêmica, que pode ser administrada com segurança durante o segundo e terceiro trimestres e risco mínimo ao feto. Estudos recentes revelam pouca diferença na sobrevida de gestantes e não gestantes com câncer. A terapia moderna, associada a equipes multidisciplinares ágeis, é chave para um desfecho favorável como o da paciente relatada, mesmo diante das intercorrências do trabalho departo prematuro e da interrupção da QT antecipadamente.
Introdução: O câncer de mama é atualmente a neoplasia com maior mortalidade no mundo em mulheres, seja em países desenvolvidos, seja naqueles em desenvolvimento. Estudos apontam que podehaver recorrência da neoplasia mamária de até 30% nos casos com linfonodos negativos e de aproximadamente 70% nos casos com linfonodos positivos. Um estudo americano mostrou taxa de recidiva de câncer de mama em 10 anos de 36,8%, dos quais a maior parte ocorreu nos primeiros cinco anos após o diagnóstico (81,9%). Outros diferentes trabalhos afirmam que o período de maior risco para a recorrência do câncer de mama são os primeiros três anos após o tratamento primário. Relato de caso: Paciente de 51 anos, com histórico de quadrantectomia e esvaziamento de mama esquerda em razão de carcinoma ductal invasivo em 2009 (imuno-histoquímica: KI67 40%/HER2 negativo/receptor de estrogênio >75%/receptor de progesterona 0; TNM: T2 N0 M0; linfonodos positivos), quimioterapia neoadjuvante e radioterapia adjuvante, com posterior implante de prótese de silicone na mama esquerda em 2012. A paciente prosseguiu em acompanhamento ambulatorial e, em agosto de 2020, foi detectado nódulo suspeito em região supraclavicular esquerda, optando-se por não fazer punção em razão da presença de importante vascularização local. Prosseguiu-se à investigação com ressonância magnética da mama, a qual revelou uma massa sólida mal definida na região infraclavicular infiltrando os músculos peitorais maior e menor e estendendo-se posteriormente até os vasos axilares, medindo 7,7x5,3 cm, com presença de linfonodos aumentados no mediastino anterior, nódulo BI-RADS 5 em proximidade da prótese. Decidiu-se então realizar nova abordagem cirúrgica em novembro de 2020 por causa da recidiva local, com retirada da prótese e biópsia da lesão, cujo laudo evidenciou carcinoma mamário tipo não especial G3. A paciente evoluiu sem intercorrências, recebendo alta no 1º dia de pós-operatório, com recomendação de retorno ambulatorial para acompanhamento. Conclusão: A neoplasia mamária é uma doença que abrange fatores psicológicos, sexuais e da autoimagem feminina e, infelizmente, a recidiva é uma realidade no carcinoma mamário. Assim, conforme a incidência do câncer de mama está aumentando com o passar dos anos, os métodos de diagnóstico precoce, a terapêutica correta e o acompanhamento contínuo para a identificação de casos de recidiva devem ser constantemente valorizados e aplicados pelo governo e serviços de saúde, para que possa haver melhorias nos desfechos das pacientes acometidas pelo câncer de mama, com o aumento da sua sobrevida e da sua qualidade de vida.
Introdução: As neoplasias da mama afetam principalmente as mulheres na perimenopausa. No entanto, aqueles que estão em plena atividade reprodutiva também podem ser afetados e, embora o carcinoma de mama seja incomum em mulheres jovens, eles geralmente têm um prognóstico pior. Este estudo descreve as características clínicas agressivas e o prognóstico sombrio de um carcinoma de mama invasivo em uma paciente de 21 anos. Relato de caso: T.S., 21 anos, G2P2, primeiro filho aos 16 anos, menarca aos 12 anos, lactante há um ano e oito meses, sem comorbidades e sem história familiar de câncer de mama. Admitida em serviço de urgência ginecológica com nódulo doloroso em mama direita associado a hiperemia local. Foi feita a hipótese diagnóstica de abscesso mamário e a paciente foi conduzida para drenagem cirúrgica de urgência. No intraoperatório, diagnosticou-se nódulo sólido sugestivo de neoplasia, que foi submetido a biópsia incisional com resultado histopatológico de carcinoma ductal tipo não especial grau 3. Estadiamento clínico IV: (cT4d cN1 cM1 - metástases óssea, pulmonar e para linfonodos cervicais). Imuno-histoquímica: RH negativos, HER2 3+, KI67 80%). A paciente foi submetida a quimioterapia neoadjuvante, sem resposta satisfatória, e encaminhada para cirurgia higiênica após a 9ª sessão. Realizou-se tratamento cirúrgico com mastectomia radical direita e, após três meses, mastectomia simples à esquerda em razão de nódulo neoformado cuja biópsia evidenciou carcinoma invasor com diferenciação apócrina. Posteriormente, a paciente evoluiu com metástases cerebrais sintomáticas e óbito que se deu três anos após o diagnóstico da lesão primária. Conclusão: Trata-se de caso de carcinoma mamário em idade extremamente jovem, visto que qualquer protocolo de rastreamento de paciente de alto risco recomenda o início do rastreamento mamário com 25 anos. Por isso, o câncer de mama deve ser sempre aventado como diagnóstico diferencial das mastites, mesmo as puerperais.
Relato de casoResumo O fibroadenoma é a neoplasia benigna mais freqüente da mama feminina e é considerado tumor misto, constituído por quantidades variáveis de tecido conjuntivo e epitelial. A ciclosporina parece ter implicações no desenvolvimento de fibroadenomas mamários em pacientes transplantadas renais em idade reprodutiva. Descrevemos o caso no qual a paciente, em uso terapêutico de ciclosporina A, após transplante renal, apresentou vários nódulos mamários bilaterais na evolução. O exame físico e os achados de imagem sugeriram fibroadenoma, diagnóstico que foi confirmado após biópsias. AbstractFibroadenoma is the most frequent benign neoplasia in the female breast and it is considered a mixed tumor, constituted by variable amounts of connective and epithelial tissue. Cyclosporine A seems to be related with the development of mamary fibroadenomas in patients who underwent kidney transplantation in reproductive age. We reported the case in which the patient, in therapeutic use of cyclosporine A, after kidney transplantation, presented several bilateral lumps. The imaging and palpable findings suggested fibroadenoma, confirmed after biopsy.
