Background The spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the possible development of serious illness, and the possibility of severe obstetric outcomes highlight the importance of addressing SARS-CoV-2 infection in obstetric management. Methods and findings A cross-sectional study of pregnant women assisted in a high-risk maternity hospital in Brazil in 2020. All patients admitted for delivery or miscarriage care were tested for SARS-CoV-2 using polymerase chain reaction (PCR) and for immunoglobulin (I)gM, and/or IgG by immunochromatography. Clinical aspects and obstetric outcomes were analyzed. A total of 265 pregnant women were included in the study. There were 38 (14.4%) PCR positive cases during pregnancy, 12 (31.6%) on admission screening, and 71(27.2%) patients were IgM- and/or IgG-positive. Among the participants, 86 (32.4%) had at least one positive test during pregnancy. SARS-CoV-2 positive patients had greater contact with known positive patients (p = 0.005). The most frequently reported symptoms were runny nose, cough, loss of smell and taste, headache, and fever. There was also a 35% rate of asymptomatic infections and a 4.6% rate of severe or critical infections. Patients exposed or infected with SARS-CoV-2 had a higher incidence of preterm delivery, cesarean section, need for resuscitation in the delivery room, Apgar score <7 at 5 min, admission to the neonatal intensive care unit, and jaundice. Newborns with at least one positive test had a significantly greater need for phototherapy after delivery (p = 0.05). The results showed a high rate of positive tests among newborns (37.5%), which seems to be compatible with both neonatal and perinatal infection. Conclusions It is important to further investigate SARS-CoV-2 infection during pregnancy, including the clinical course and the possibility of adverse outcomes with impact on maternal and fetal health, regardless of the development of symptoms.
Introduction: Endometrial stromal sarcoma is a rare tumor that corresponds to approximately 1% of uterine tumors. Its diagnosis is challenging to make due to the similarity with benign tumors both in the characteristics of the lesion in imaging exams and about the symptoms, which is only obtained from the histopathological analysis. Case report: In the case reported, to obtain the diagnosis was necessary several attempts and methods, and the patient was in an advanced stage, with distant metastasis, impairing her prognosis. This article discusses ways to diagnose, staging, prognosis, and management of this rare neoplasm.
Introdução: Ao se tratar da população transgênero, é necessário entender significados e terminologias que representam a enorme diversidade. Identidade de gênero refere-se à própria experiência de quem são, diferente de orientação sexual ou expressão de gênero. A desinformação leva a sociedade a adotar estigmas e preconceitos, causando consequências negativas à saúde de pessoas transgêneras, que não se sentem confortáveis em buscar serviços de saúde. As infecções sexualmente transmissíveis são transmitidas, principalmente, por contato sexual e podem estar associadas a fatores biológicos, psicossociais e culturais. Assim, essa população se inclui no grupo que requer uma atenção estrategicamente mais focada no diagnóstico e tratamento precoces de infecções sexualmente transmissíveis. Objetivo: Demonstrar o impacto do ambulatório multidisciplinar de diversidade de gênero nos diagnósticos de infecções sexualmente transmissíveis da população transgênero. Métodos: Estudo quantitativo, retrospectivo, descritivo sobre infecções sexualmente transmissíveis diagnosticadas em pacientes atendidos no ambulatório multidisciplinar de diversidade de gênero no período de 1 ano. As informações foram coletadas por questionário previamente estruturado e resultado de testes rápidos, exames laboratoriais e análises citológicas de amostras cervicais e anais. Os dados foram registrados e tratados estatisticamente no programa SPSS. Resultados: Foram analisados os dados de 76 usuários. Houve predominância de homens transgêneros (65,8%). Desses, 20,5% apresentaram positividade para papilomavírus humano anal e 27% para papilomavírus humano cervical. Lesões pré-neoplasicas foram descritas em 16,7% dos exames de citologia cervical. A média de idade foi de 27 anos. Os homens transgêneros apresentaram maior nível de escolaridade em relação a mulheres transgêneros, sendo 100% daqueles que têm ensino superior completo e 62% dos que possuem ensino médio completo. Apenas as mulheres transgêneros apresentaram positividade para vírus da imunodeficiência humana (7,7%), sífilis (15,4%), hepatite B e C (3,8%). Tricomoníase, gonorreia e clamídia não foram diagnosticadas. Conclusão: A existência de ações como o ambulatório multidisciplinar de diversidade de gênero é essencial para fornecer orientações, medidas de prevenção e diagnóstico de infecções sexualmente transmissíveis, além de ser porta de entrada dessa população a serviços de saúde especializados.
Introdução: A placenta acreta (AP) é uma condição patológica de placentação grave associada a parto prematuro e alto risco de hemorragia obstétrica maciça intraparto que afeta a saúde materna em todo o mundo. Na última década, estudos mostraram que o crescimento dos casos da AP é atribuído ao aumento da prevalência de parto cesáreo, e o diagnóstico pré-natal demonstrou diminuir a morbidade materna, tornando-se essencial para melhorar o manejo. Contudo, o envolvimento vesical e/ou de vasos pélvicos pode ocorrer, embora seja raro, resultando em mortalidade materna e fetal expressiva. Relato de caso: Paciente com 31 anos, G5 P5C A0, sem comorbidades, iniciou pré-natal de risco em função de quatro cesáreas prévias e, durante acompanhamento, foi diagnosticada com placenta prévia. Com 30,5 semanas, apresentou quadro de sangramento vaginal com duração inferior a 24 horas, sem outros sintomas associados, e compareceu ao pront-socorro do Hospital Universitário, onde foi internada. Evidenciaram-se por meio de ressonância magnética e ultrassonografia placenta prévia total, acrestimo placentário e suspeita de vasa prévia. Durante a internação realizaram-se dois ciclos de betametasona, e com 34,1 semanas foi indicada a interrupção por cesariana após o planejamento do manejo com uma equipe multidisciplinar. Previamente à cirurgia, um cateter duplo J foi inserido pelo urologista e logo em seguida a equipe de hemodinâmica inseriu um cateter em artérias uterinas para embolização, sem intercorrências. Procedeu-se à histerectomia total, na qual se visualizou acretismo placentário sem evidência de invasão a órgãos adjacentes, e a aderência em útero e bexigas foram desfeitas sem necessidade de embolização das artérias uterinas. Conclusão: A forte associação com cesárea e curetagem prévias ocorre pela formação de uma cicatriz que propicia a inserção inadequada da placenta. Feito o diagnóstico de acretismo placentário e possível invasão da bexiga, a conduta aplicada será a histerectomia total abdominal. É de extrema importância uma equipe multidisciplinar composta de obstetra, cirurgião vascular intervencionista, urologista e transfusionista nesses casos. As complicações incluem sangramento excessivo, coagulação intravascular disseminada, secção ou transfixação de um dos ureteres, sendo necessário segura hemostasia e drenagem da cavidade abdominal e, em alguns casos, passagem profilática de um stent ureteral duplo J. Em resumo, a fisiopatologia do espectro da placenta acreta é complexa. O diagnóstico pode ser feito no transoperatório, por ressonância magnética ou ecografia, e o tratamento vai depender do grau de acretismo. Neste caso de desfecho de sucesso, a abordagem pré-operatória multidisciplinar foi fundamental para prevenir lesões de órgãos adjacentes e sangramento descontrolado intraoperatório, impactando significativamente a morbimortalidade materna.
