Os autores apresentam uma nova técnica para dosagem de metaemoglobina em relação à oxiemoglobina, sem interferentes químicos ou enzimáticos. A base metodológica da técnica proposta se fundamenta nos valores reais de IntroduçãoDenomina-se metaemoglobinemia a situação clínica caracterizada pela elevação da concentração de metaemoglobina no sangue, acima do nível padrão estabelecido por dosagens químicas ou enzimáticas. As três principais causas que determinam o aumento de metaemoglobina têm origens diferentes e se devem à: 1) deficiência de enzimas eritrocitárias específicas para as atividades redutoras da oxidação do ferro do grupo heme; 2) à indução tóxico-oxidativa da hemoglobina causada por compostos químicos oxidantes; 3) ao defeito molecular da hemoglobina que causa contínua auto-oxidação. Esse fato faz com que as enzimas eritrocitárias com capacidade redutora atuem com eficiência para manter o equilíbrio assim representado OxiHb ↔ MetaHb. O equilíbrio que mantém a relação OxiHb/MetaHb no sangue é alcançado quando a indução oxidativa que transforma a oxiemoglobina em metaemoglobina se iguala à sua redução para oxiemoglobina mediante a ação de três enzimas específicas: NADH-diaforase, glicose-6-fosfato desidrogenase (G-6-PD) e metaemoglobina redutase.1 A redução da atividade de uma dessas três enzimas motivada por herança genética autossômica recessiva pode resultar na elevação da concentração de metaemoglobina, que, ao atingir valores entre 8% e 40%, promove a mudança na cor 1
A Hb D é uma hemoglobina variante com mutações em diferentes pontos da globina beta, fatos que determinam pelo menos quatro tipos distintos: Hb D Iran (β22 Glu → Lis), Hb D Ibadan (β97 Tre → Lis), Hb D Beograd (β121 Glu → Val) e Hb D Punjab ou Los Angeles (β121 Glu → Gln). O tipo mais comum entre os genótipos de Hb D é a Hb D Punjab ou Los Angeles que foi descrita em 1950 por Itano e Neel (1). No Brasil, em amplo estudo realizado em diversas regiões e avaliando 100 mil pessoas, observou-se que os casos de Hb D identificados eram do tipo D Punjab ou Los Angeles, com prevalência média do genótipo da Hb AD na proporção de um caso para cinco mil pessoas analisadas (2, 3). A Hb D migra na mesma posição da Hb S em eletroforeses alcalinas, e a diferenciação entre as hemoglobinas S e D se faz por meio de eletroforese em agar-ácido (3, 4). A determinação específica dos tipos de Hb D pode ser efetuada por eletroforeses de isofocalização e de cadeias polipeptídicas de globinas, PCR e HPLC (5).A homozigose da Hb D, ou Hb DD, é uma situação rara, mesmo na região de Punjab (India) onde a prevalência do heterozigoto (Hb AD) é variável entre 2 e 3% (4,6). A Hb DD -também conhecida por doença de Hb D -se caracteriza por discreta anemia microcítica e hipocrômica, e a concentração da Hb D é próxima de 97 e 98% (4).A interação entre Hb D e talassemia beta também é uma situação muito rara, com poucos casos relatados na literatura internacional e também apresenta discreto grau de anemia microcítica e hipocrômica (4, 5).A presente comunicação se refere aos resultados do estudo familiar de um paciente do sexo masculino, com 37 anos de idade, que relata anemia desde a infância.
