Nowadays, nutrition is said to be an integral aspect of acute pancreatitis (AP) treatment. Early enteral nutrition (EEN) is safe and beneficial for patients. This was confirmed by clinical experience and can be found in guidelines on managing adults with AP. Furthermore, paediatric recommendations encourage EEN use in AP. However, paediatric guidelines are based exclusively on studies in adults. Therefore, we present a review of published studies on the time of nutritional interventions in children with AP. A search was independently conducted in April 2022 by two of the authors. Only full-text papers published in English involving children between 0–21 were considered. Only four papers met our inclusion criteria: one randomised-control trial (RCT), one prospective study with retrospective chart review, and two retrospective chart reviews. All studies supported EEN and there was no recommendation of any delay in its initiation. The results of all four papers suggested EEN with a regular, normal-fat diet. EEN is safe in children with mild or moderately severe AP and may decrease the length of hospitalisation. Unfortunately, all the conclusions are based on a small amount of heterogeneous data that are mostly retrospective. Future prospective RCTs are needed.
Wstęp: Choroba trzewna (coeliac disease-CD) to występująca u osób z predyspozycją genetyczną choroba o podłożu immunologicznym indukowana przez gluten. Jednym z pozajelitowych objawów CD jest hipertrans aminazemia-dobrze scharakteryzowana u dorosłych, ale z ograniczoną liczbą badań w populacji dziecięcej. Cel pracy: Celem pracy była ocena częstości występowania podwyższonej aktywności transaminazy alanino wej (AlAT) u dzieci z nowo rozpoznaną CD. Materiał i metody: Retrospektywnie przeanalizowaliśmy dane od 168 dzieci z nowo rozpoznaną CD. Cho roba trzewną została rozpoznana na podstawie dodatniego wyniku przeciwciał charakterystycznych dla CD (przeciwko transglutaminazie tkankowej i/lub przeciwko endomysium mięśni gładkich) oraz oceny histo patologicznej wycinków błony śluzowej dwunastnicy. Aktywność AlAT została oznaczona przy rozpozna niu CD u 137 (81,6%) pacjentów, których zakwalifikowano do badania. Grupa składała się z 73 dziewcząt i 64 chłopców w wieku od 11 do 202 miesięcy. Wyniki: Występowanie podwyższonej aktywności AlAT, zdefiniowanej jako > 30 U/l, stwierdziliśmy u 30,7% badanych. Nie znaleźliśmy istotnej statystycznie korelacji pomiędzy podwyższoną aktywnością transamina zy a wiekiem, płcią i objawami klinicznymi CD. Przyjmując jako definicję podwyższonej aktywności AlAT wartość > 40 U/l, stwierdziliśmy, że częstość występowania wyniosła 10,9%. Dzieci z hipertransaminazemią były młodsze (p = 0,01) i częściej występowały u nich objawy ze strony przewodu pokarmowego (p = 0,03). Wnioski: Zwiększona aktywność AlAT stosunkowo często występuje u dzieci z nowo rozpoznaną CD. Częściej obserwowana jest u dzieci młodszych.
