To present the first hysteroscopic findings of 2 cases of complete hydatidiform mole (CHM) and partial hydatidiform mole (PHM) within the context of the patients' clinical histories. Design: Presentation of 2 hysteroscopic videos with narration of the intrauterine findings of molar pregnancy (MP) from Rio de Janeiro Gestational Trophoblastic Disease Reference Center. Setting: MP is characterized by abnormal fertilization that generates 2 clinical syndromes: CHM and PHM [1]. Interventions: In the first case, the patient was aged 50 years, and hysteroscopy was indicated to assess abnormal uterine bleeding in the presence of normal serum human chorionic gonadotropin (hCG) and transvaginal ultrassonography showing an endometrial cavity with heterogeneous content. Hysteroscopy found translucent hydropic structures diagnosed as CHM. The negative hCG value was due to the hook effect (hCG after dilution: 2 240 000 IU/L). In the second case, an 18-year-old patient underwent hysteroscopy to assess the endometrial cavity with retained abortion at 7 weeks in which, during conservative management, the hCG level increased over 4 weeks from 25 000 IU/L to 58 000 IU/L. Hysteroscopy visualized the embryo with its umbilical cord and hydatidiform vesicles diagnosed as PHM. Conclusion: MP can be an incidental finding during hysteroscopy for abnormal uterine bleeding or retained abortion [2−4]. Knowing its morphology during hysteroscopy is helpful for the correct management of this uncommon clinical situation. Hysteroscopy as an adjunct diagnostic tool (not as first-line treatment for MP) can be of significant benefit in challenging clinical scenarios. Further studies should assess the possible risk of spreading molar cells into the peritoneal cavity owing to hysteroscopic fluid.
Realizou-se um estudo clínico prospectivo, fase III, multicêntrico, aberto, aleatório e comparativo. Foram avaliadas 60 pacientes portadoras de câncer de mama localmente avançado, estádio IIIA, divididas em dois grupos. As pacientes foram submetidas à quimioterapia primária, com ou sem intensificação de dose, por quatro ciclos, e à cirurgia. Utilizou-se o protocolo FEC 50 no Grupo A (5-FU 500 mg/m2, epirubicina 50 mg/m2 e ciclofosfamida 500 mg/m2) e o FEC 100 no Grupo B (5-FU 500 mg/m2, epirubicina 100 mg/m2 e ciclofosfamida 500 mg/m2). Foram analisados, durante a quimioterapia, o estado geral, variação ponderai, alopécia, alterações digestivas, hematológicas e cardiotoxicidade. Após a quimioterapia avaliou-se a resposta tumoral clínica e, na peça cirúrgica, a resposta anatomopatológica. A resposta clínica objetiva (resposta completa e resposta parcial) nos grupos A e B foi 93% e 96%, respectivamente. Os resultados foram similares, porém no Grupo B houve um percentual maior de respostas clínicas completas. Avaliou-se a doença residual no sítio primário e nos linfonodos axilares. No Grupo A houve 4 (13%) casos de resposta anatomopatológica completa, 12 (40%) de tumor residual microscópico e 14 (47%) de tumor macroscópico. No Grupo B ocorreram 10 (33%) casos de resposta anatomopatológica completa, sete (23%) de tumor residual microscópico e 13 (44%) de tumor macroscópico. Concluiu-se que a quimioterapia primária proporcionou resposta clínica e anatomopatológica maior no grupo com intensificação de dose. A toxicidade, tolerável e reversível, foi mais acentuada no grupo de altas doses.
Objetivo: avaliar a eficácia da biópsia percutânea estereotáxica (BPE) no diagnóstico das lesões mamárias subclínicas, comparando seus resultados com a biópsia cirúrgica precedida de localização estereotáxica com fio guia. Métodos: no período de janeiro de 1995 a fevereiro de 1997, realizamos um estudo transversal com 41 casos de lesões não-palpáveis em pacientes com idade superior a 35 anos, no Instituto de Ginecologia da Universidade Federal do Rio de Janeiro. Os casos foram classificados radiologicamente como benignos, provavelmente benignos, suspeitos e malignos. Para fins de análise estatística, as lesões benignas e provavelmente benignas foram estudadas em conjunto. O laudo histopatológico da BPE foi classificado em: inadequado para diagnóstico, ausência de malignidade, suspeito e maligno. O laudo histopatológico da biópsia cirúrgica foi classificado em: ausência de malignidade, pré-maligno e maligno. Calculamos a sensibilidade, a especificidade e os valores preditivos da BPE, assim como as razões de verossimilhança da mamografia e da BPE com a finalidade de predizer a probabilidade de câncer de mama. Resultados: a BPE coincidiu com a biópsia cirúrgica em 86,2% dos 29 casos de ausência de malignidade. Todos os casos suspeitos à BPE foram malignos à biópsia cirúrgica. Todos os casos de malignidade à BPE estiveram acordes com a biópsia cirúrgica. A sensibilidade e a especificidade da BPE foram 36,4% e 100%, respectivamente. O valor preditivo positivo do método foi 100% e o negativo 78,1%. No grupo classificado como maligno à mamografia, a razão de verossimilhança foi 9,7, sendo 1,3 para os casos suspeitos e 0,1 para o laudo provavelmente benigno. A razão de verossimilhança da BPE foi infinita (∞) para as lesões suspeitas e malignas, 0,4 para os casos classificados como ausência de malignidade e 1,4
