OBJETIVO: comparar a mortalidade materna declarada pelo Sistema Nacional de Informação sobre Mortalidade (SIM) com a investigação pela pesquisa de óbitos de mulheres em idade reprodutiva (RAMOS), de 10 a 49 anos; identificar a subnotificação e investigar as causas de morte materna (MM) no período de 1999 a 2006. MÉTODOS: série temporal e de base populacional a partir das informações das declarações de óbito (DO), fornecidas em banco de dados pela Fundação Sistema Estadual de Análise de Dados (SEADE) e com as causas de morte codificadas pela Classificação Internacional de Doenças (CID), décima revisão e o número de nascidos vivos (NV). Os óbitos foram categorizados em MM declarada, presumível e não-materna. A identificação dos casos foi feita a partir de listagem com a data de nascimento e de óbito no velório municipal, e as informações complementares ao estudo foram obtidas no Setor de Vigilância Epidemiológica do Comitê Municipal de Investigação da MM (CMIMM). Foram levantadas informações sobre MM contidas no SIM. Nos casos de MM declaradas e não-declaradas, foi identificado o percentual de subnotificação; foram calculadas as razões de mortalidade materna (RMM) oficial e corrigida e o fator de ajuste para o período, e as causas de MM foram revisadas e classificadas. RESULTADOS: foram identificadas 12 MM, sendo seis declaradas e seis não-declaradas. A subnotificação foi de 50%, o que correspondeu a um fator de ajuste igual a dois. A RMM oficial foi 14,7 e a corrigida de 29,4 mortes por 100.000 NV. As causas básicas foram mal atribuídas na maioria dos casos. As causas obstétricas diretas foram mais prevalentes, dentre elas a eclâmpsia e a síndrome HELLP, seguida por infecções. CONCLUSÕES: são necessárias medidas políticas e administrativas para a efetiva atividade dos Comitês de Investigação das MM. A prevalência de causas obstétricas diretas é indicativa de falhas na assistência materna e perinatal.Abstract PURPOSE: to compare maternal death data from the National Death Information System (DIS), with a death survey of 10 to 49 year-old women at reproductive age (RAMOS), in order to identify sub-notification and to search for causes of maternal death (MD) from 1999 to 2006. RESUMO OBJETIVO. Identificar fatores associados à cesárea em mulheres com um único parto anterior por cesárea e submetidas à prova de trabalho de parto. MÉTODOS. Estudo de corte transversal retrospectivo, incluindo 1746 mulheres com uma cesárea anterior submetidas à prova de trabalho de parto no segundo parto ocorrendo entre 1986 e 1998. Foram excluídos os casos com atual gestação múltipla e/ou com malformações fetais incompatíveis com a vida. Elas foram divididas pelo tipo de parto atual em dois grupos: cesárea (n=731) e parto vaginal após cesárea (PVAC, n=1015). A análise estatística para a identificação de fatores associados ao tipo de parto foi feita por meio do cálculo da razão de prevalência (RP) e IC 95%, sendo feito também o ajuste por idade, excluindose os casos com informações ignoradas em cada análise. RESULTADOS. A taxa total de parto vaginal após cesárea foi de 58,1%. Os fatores significativamente associados com parto por cesárea foram: maior idade materna, maior altura uterina (RP 1,5; IC 95% 1,19-1,88), rotura prematura de membranas (1,3; 1,08-1,54), líquido amniótico não claro (1,22; 1,04-1,43) ou com quantidade alterada (1,32; 1,01-1,73), alteração dos batimentos cardíacos fetais (1,96; 1,68-2,28), apresentação não cefálica (2,03; 1,54-2,66), indução do parto (1,74; 1,42-2,11) e ausência de analgesia (2,57; 2,11-3,11). CONCLUSÃO. Os fatores associados ao parto por cesárea foram a maior idade, apresentação não-cefálica, rotura prematura de membranas, fetos grandes, necessidade de indução do trabalho de parto, e sinais de comprometimento da vitalidade fetal.UNITERMOS: Cesárea. INTRODUÇÃONo decorrer do século XX, observou-se o crescimento linear das taxas de cesárea em todo o mundo devido principalmente à diminuição da morbidade materna associada à sua realização, sobretudo após o advento da antibioticoterapia e das modernas técnicas de hemoterapia. Na década de 60, com o surgimento de novos métodos para avaliação da vitalidade fetal, foram acrescentadas às indicações maternas de cesárea aquelas por sofrimento fetal, crescendo, assim, sua popularidade e freqüência. Paralelamente, alcançou-se, no decorrer deste período, a simplificação da técnica de sua execução e a redução significativa da morbi-mortalidade perinatal 1 . No entanto, esta redução parece relacionar-se mais diretamente ao desenvolvimento da assistência à gravidez, ao parto e ao recém-nascido do que à via de parto propriamente dita 1 . Algumas evidên-cias neste sentido podem ser encontradas em países desenvolvidos, onde a mortalidade materna e perinatal declinaram, mesmo em vigência de taxas de cesárea baixas e estáveis 2 . A prevalência de CE foi significativamente maior em mulheres com idade >35 anos, história pregressa ou atual de síndrome hipertensiva, diabetes ou primeiro filho morto, bem como com alterações no volume do líquido amniótico. CONCLUSÕES: A realização da PTP aumentou progressivamente ao longo dos treze anos, sem aumento na morbidade materna e/ou neonatal.
