Introdução: No contexto das doenças oncológicas, o câncer infanto-juvenil representa uma condição com consequências físicas, psicológicas e sociais, com impactos nos diversos atores envolvidos, incluindo a equipe de saúde, o paciente e a família. Em se tratando dos cuidados paliativos oncopediátricos, o objetivo das intervenções é a prevenção e alívio do sofrimento, e a identificação e manejo da dor e demais sintomas, mediante uma abordagem integral, na qual a comunicação médica ocupa lugar relevante. Objetivo: Identificar os desafios enfrentados na comunicação médica, tendo como cenário de discussão os cuidados paliativos oncológicos pediátricos. Metodologia: O estudo se desenvolveu por meio de uma revisão integrativa, seguindo as diretrizes preconizadas pelas Preferred reporting items for systematic reviews and meta analiyses. A seleção dos artigos ocorreu na base de dados Biblioteca Virtual de Saúde (BVS) e pelo serviço de indexação de artigos da National Library of Medicine (PubMed). Resultados: No total, foram encontrados 318 artigos, sendo 270 no PubMed, e 48 na BVS. Entretanto, apenas 12 artigos atenderam aos critérios de inclusão e foram selecionados para compor a revisão integrativa. Conclusão: A comunicação médica é um aspecto relevante nos cuidados paliativos oncopediátricos, porém o manejo das informações pode ser impactado por diferentes variáveis, incluindo o desconhecimento sobre a abordagem e a dificuldade para lidar com a finitude da vida. Ademais, são necessários estudos que abordem a perspectiva da criança e do adolescente como co-autores do processo de cuidado, haja vista que a maioria das pesquisas não adota esse enfoque.
