Two mechanisms are most widely considered to be crucial in overcoming traumatic events: (a) habituation to the frightening stimuli that occurs after exposure through self-confrontation to the traumatic memories and avoided stimuli (Foa & Riggs, 1995; chapter 6, this volume;Vaughan & Tarrier, 1992) and (b) cognitive reappraisal of the traumatic experiences (chapters 6 and 10, this volume; Resick & Schnicke, 1992). These mechanisms lead to two main ingredients in the cognitive-behavioral treatment of posttraumatic stress disorder (PTSD) and pathological grief: Imaginary exposure is used to help patients to confront the sensory perceptions, emotions, and thoughts they usually avoid. Cognitive reappraisal implies challenging dysfunctional automatic thoughts and stimulating reinterpretation of misattributions about the traumatic event, in order to accommodate a new symbolic meaning about the experience. ‘Interapy’ Burnout: Prevention and Therapy of Burnout via the Internet Background: In recent years a lot of computer-driven self-help programs have been developed in which no client-therapist interaction whatsoever takes place. These programs have yielded varying successes. The Internet offers new possibilities, including treatment programs in which clients do interact with their therapist, without the two ever meeting face to face. The therapist responds to client input and vice versa. This is the essence of the low-threshold Interapy program for treatment of burnout (work-related stress): www.interapy.nl. In this article we describe the development of this Internet treatment, its procedures and interventions and an outcome study. Method: Randomized controlled trial (n = 133) comparing the treated group to a psycho-education only control group. Results: Significant improvements and high effect sizes on burnout symptoms, anxiety and depression were yielded in the experimental group, in comparison to the control group that received psycho-education only. Illustration: The treatment is illustrated by a case description written by the client (the fourth author) herself. Discussion: The specific elements that explain the high effect sizes of this treatment are discussed.

Er is de afgelopen jaren flink wat onderzoek verricht naar de bruikbaarheid van computerprogramma's voor behandelingen waaraan geen behandelaar te pas komt. De resultaten hiervan zijn niet denderend. Het Internet biedt nieuwe mogelijkheden, zoals geprotocolleerde interventieprogramma's waarbij een behandelaar wel degelijk contact heeft met de clie¨nt zonder dat ze elkaar zien. De behandelaar reageert op de input van de patie¨nt en vice versa. Dit is de achtergrond van het laagdrempelige Interapy-programma, waarin psychologische hulp via Internet wordt aangeboden. Het gaat hierbij niet om e-mailcontact of een chatprogramma. Dergelijke therapiecontacten zijn er al jaren, maar zijn nooit op hun effectiviteit onderzocht; hun 'format' wijkt bovendien niet wezenlijk af van face-to-face contacten. De Interapy-site bevat daarentegen een uiterst gestructureerde behandeling, waarbij clie¨nt en behandelaar samen een vast geprotocolleerd traject doorlopen. WerkwijzeDe Interapy-site op het world wide web (interapy.psy. uva.nl ) is momenteel nog uitsluitend gericht op onverwerkte rouw en posttraumatische stress. Wie inlogt, krijgt eerst voorlichting over trauma's en gangbare therapiee¨n, over de Interapy-behandeling en over het onderzoeksteam. Geı¨nteresseerden die zich vervolgens aanmelden ondergaan dan een on line screeningsprocedure. Voor deelname aan Interapy worden de volgende uitsluitingscriteria gehanteerd: ernstig alcohol-of drugsmisbruik, psychose, dissociatie en ernstige depressie. Personen die jonger zijn dan achttien jaar of elders voor dezelfde problematiek in behandeling zijn, komen evenmin voor deelname in aanmerking. De clie¨nten geven een korte beschrijving van de traumatische gebeurtenis(sen), waarna de screening vervolgt met een Biografische Vragenlijst. Deze bevat vragen over alcohol-en drugsgebruik en consumptie van medicijnen en zorg. Vervolgens komt de clie¨nt de volgende gevalideerde multiple choice vragenlijsten tegen: Alle vragenlijsten worden on line afgenomen en automatisch gescoord. Als een clie¨nt een item overslaat, kan hij niet verder. Het scherm geeft dan aan dat een vraag alsnog beantwoord dient te worden. Een clie¨nt die op een bepaalde vragenlijst een te hoge score heeft, bijvoorbeeld een depressiescore boven de normscore 'hoog depressief' voor een psychiatrische bevolking of een te hoge dissociatiescore, krijgt automatisch on line uitleg waarom hij niet aan het programma kan meedoen en krijgt alternatieven geboden. Gedetailleerde beschrijving van de site is elders gepubliceerd. 4 Als de clie¨nt de vragenlijsten heeft ingevuld en aan de criteria voldoet, volgt het tweede deel van de voormeting, noodzakelijk om later de resultaten van de behandeling te kunnen evalueren. De clie¨nt krijgt nu de Schokverwerkingslijst (SVL) 5 on line in te vullen.
