Middle ear implantation is an efficient procedure to restore moderate to severe sensorineural hearing loss (HL) in selected patients. Implantation of such devices requires ossicular chain integrity. Patients suffering from otosclerosis with mixed HL should be eligible for this treatment after stapes surgery with air-bone gap closure. To address this issue, we report four cases of middle ear implantation after or during stapes surgery. Results and complications obtained with Vibrant SoundBridge, MedEl and Middle Ear Transducer, Otologics are reported. Audiologic results were similar to those obtained in cases of sensorineural HL. One case of postoperative labyrinthitis was observed.

The Vibrant Soundbridge is a semi-implantable middle ear hearing device used in the rehabilitation of adults with sensorineural hearing loss. In order to evaluate the long-term effects of the implanted part of the device, audiological data from 39 patients implanted over several implant sites across France were collected and analyzed retrospectively. The mean follow-up time was 16 months; 25 patients had a follow-up period of over 1 year. Surgery was uneventful in all cases. The present study of the 39 implanted patients with a mid- to long-term follow-up found a statistically significant modification of hearing thresholds (pre- versus postoperative) for frequencies of 0.5 and 4 kHz. However, the shift of threshold was rather limited (2.79 and 3.34 dB, respectively), and this variation was not statistically different from the evolution of the opposite non-operated ear.

We should consider observation a therapeutic modality. We valid the intrameatal staging as prognostic factor and propose a stratification of patients into low, moderate, or high risk for potential tumor growth to guide the initial management of ICVS.

L’obiettivo di questo lavoro è stato di valutare la storia naturale di crescita degli schwannomi vestibolari (VS), la qualità di vita di quelli trattati in maniera conservativa e di validare una scala specifica per tale malattia in lingua francese, Penn Acoustic Neuroma Quality-of- Life (PANQOL). Sono stati studiati retrospettivamente 26 pazienti con VS trattato in maniera conservativa. Sono state raccolte le caratteristiche dei pazienti e i reperti radiologici, e sono state utilizzate due scale per validare valutare la qualità di vita: la Short Form-36 Health Survey (SF-36) e la PANQOL scale, tradotta in francese. I punteggi ottenuti sono stati comparati con gli studi precedenti. Il tempo medio di follow up è stato di 25 mesi (range 6-72). È stata osservato un accrescimento del tumore in 14 pazienti (53,8%), nessun accrescimento in 12 pazienti (46,2%), e non si è verificata nessuna riduzione. La crescita media del tumore è stata di 2,22 mm/anno, e non sono stati individuati fattori predittivi di crescita. I pazienti con vertigini e instabilità hanno riferito una più bassa qualità di vita, sia secondo la scala SF-36, sia secondo la scala PANQOL. Utilizzando la scala SF-36, i nostri risultati si sono rivelati paragonabili a quelli della letteratura. Utilizzando la scala PANQOL, i nostri punteggi non si sono rivelati statisticamente diversi da quelli derivanti da studi tedeschi e nordamericani, ad eccezione di quelli riguardanti l’udito (p=0,019). La qualità di vita diventa sempre più importante nella gestione dei VS. In linea con questi risultati, noi sosteniamo la strategia non conservativa associata ad una riabilitazione vestibolare per quei pazienti con vertigini ed instabilità. La scala PANQOL, disponibile in lingua francese, si è rivelata specifica per i VS.

BACKGROUND AND PURPOSE: Recurrent middle ear cholesteatomas are commonly preoperatively assessed using MR imaging (non-EPI-DWI) and CT. Both modalities are used with the aim of distinguishing scar tissue from cholesteatoma and determining the extent of bone erosions. Inflammation and scar tissue associated with the lesions might hamper a proper delineation of the corresponding extensions on CT images. Using surgical findings as the criterion standard, we assessed the recurrent middle ear cholesteatoma extent using either uncoregistered or fused CT-MR imaging datasets and determined the corresponding accuracy and repeatability. MATERIALS AND METHODS:Twenty consecutive patients with suspected recurrent middle ear cholesteatoma and preoperative CT-MR imaging datasets were prospectively included. A double-blind assessment and coregistration of the recurrent middle ear cholesteatoma extent and manual delineation of 18 presumed recurrent middle ear cholesteatomas were performed by 2 radiologists and compared with the criterion standard. "Reliability score" was defined to qualify radiologists' confidence. For each volume, segmentation repeatability was assessed on the basis of intraclass correlation coefficient and overlap indices. RESULTS:For the whole set of patients, recurrent middle ear cholesteatoma was further supported by surgical results. Two lesions were excluded from the analysis, given that MR imaging did not show a restricted diffusion. Lesions were accurately localized using the fused datasets, whereas significantly fewer lesions (85%) were correctly localized using uncoregistered images. Reliability scores were larger for fused datasets. Segmentation repeatability showed an almost perfect intraclass correlation coefficient regarding volumes, while overlaps were significantly lower in uncoregistered (52%) compared with fused (60%, P Ͻ .001) datasets. CONCLUSIONS:The use of coregistered CT-MR images significantly improved the assessment of recurrent middle ear cholesteatoma with a greater accuracy and better reliability and repeatability. These two approaches allow non-invasive and wide exposure of the infratemporal fossa as compared to classical approaches. Surgical approach should be selected according to the tumour's anatomical location with respect to the maxillary sinus posterior wall. The preauricular subtemporal approach is recommended for tumours localised posterolaterally with respect to the maxillary sinus posterior wall. Medial and anterior tumours near the maxillary sinus posterior wall can be best removed using a transnasal endoscopic approach.

VSB surgery has been performed using a facial recess approach with risk for facial nerve and taste disturbance. Transcanal approaches are good alternative for this surgery. Three major limitations are to be assessed in future patient studies: the pathologic findings of the EAC, the design of the FMT regarding the axis of the ossicular chain, the long-term evaluation of the skin of the external ear canal.
