ZusammenfassungEs werden zwei weitere Patienten mit kongenitalem Faktor-XIII-Mangel vorgestellt, bei denen sich ein Plasmagerinnsel in Harnstoffoxalat in Zeiten unter 1 Min. auflöst. Immunologisch war bei beiden Patienten das Faktor-XIII-Protein nachweisbar. Die gerinnungsphysiologische Routineuntersuchung ergab mit Ausnahme einer sich bei langer Laufdauer vermindernden Amplitude im Thrombelastogramm beider Patienten keinen pathologischen Befund. Auch die Plattchenfunktionsprüfungen sowie die Retraktion waren bei beiden Kranken normal.Eine Methode zur Bestimmung partieller Faktor-XIII-Defekte – wie sie bei sog. Heterozygoten der kongenitalen F aktor-XIII-Mange]-Kranken vorkommt – wurde in Form eines modifizierten Monojodessigsäuretoleranztests entwickelt. Die Verwendung von Harnstoffoxalat als Lösungsmittel hat hierbei den Vorteil, daß zur Beurteilung der Faktor-XIII-Aktivität die totale Lösung des Gerinnsels herangezogen wird. Für die Bestimmung geringer Faktor-XIII-Aktivitäten haben wir zusätzlich das Reaktionsgemisch mit Cystein aktiviert. Dabei kommt es zwar zur Interferenz aller 3 Reaktionsteilnehmer, aber die Lösungszeiten hängen dennoch von dem Faktor-XIII-Gehalt der Gerinnsel ab, wenn die Monojodessigsäure- und Cysteinkonzentration entsprechend eingestellt sind.
