SPA cholecystectomy is safe, although the operation is significantly longer. No differences in terms of major complications or the incidence of incisional hernia were seen after 1 year. QoL was significantly better in patients operated on with the SPA technique.
Sehr geehrter Herr Hausdörfer, in Ihrem letzten Satz des oben genannten Artikels plädieren Sie gegen die gelegentliche, wenig geübte und nicht kunstgerechte Ausfuhrung der Regional-und Leitungsanästhesie im Kindesalter. Dieser Aussage möchte ich mich ohne jede Einschränkung anschließen. Eine Reihe von Argumenten und Widersprüchen in Ihrem Beitrag verhindert jedoch nachgerade, daß sich diese Methoden im Kindesalter hierzulande im größeren Maße durchsetzen und somit entsprechende Erfahrungen gemacht werden.Hier muß eine Erwiderung erlaubt sein.Es ist richtig: Die Ailgemeinanästhesie im Kindesalter ist heutzutage sicher, aber von einer suffizienten postoperativen Schmerztherapie in dieser Altersgruppe kann meines Erachtens keine Rede sein. Dies belegen unsere Umfrageergebnisse an 30 Kliniken aller Größenordnungen anläßlich unseres Symposiums "Perioperative Schmerztherapie im Kindesalter" im Juni vergangenen Jahres in Bonn und bestätigen eine Reihe von Veröffentlichungen zu diesem Thema in den vergangenen Jahren (1,2). Das von Ihnen propagierte "ausgeprägt-wirksame, altbewahrte Paracetamol-Kodein-Zäpfchenu wird unserer Erfahrung nach häufig nicht zeigerecht appliziert und zudem unterdosiert. Um ausreichende Blutspiegel zu erreichen, müssen 30mg/kg/KG Paracetarnol rektal verabreicht werden (3). Darüber hinaus ist es auch in der Kombination mit Kodein als schwaches Analgetikum einzustufen (4).Die von Ihnen als Alternative empfohlene Substanz Ketorolac ist erst seit einigen Jahren auf dem Markt und nur wenig Erfahrungsberichte über die Anwendung im Kindesalter liegen vor (5,6). Es bleibt mithin abzuwarten, ob in der zukünftigen Anwendung neben den schon bekannten Nebenwirkungen dieses NSAID-Medikamentes -wie Verlän-gerung der Blurungszeit sowie Blutungen und Ulzera am Gastrointestinaltrakt -weitere hinzukommen (7). Die Arzneimittelkommission der Bundesärztekammer nahm jüngst dazu Stellung (8).Als supplementierende Verfahren zur Allgemeinanästhesie fuhren die beiden einfachen Regionalanästhe-sietechniken Peniswurzelblock (PWB) und Ilioinguinalis/Iliohypogastricusblock (IIB/IHB) zu einer äußerst wirksamen postoperativen Analgesie und senken darüber hinaus die intraoperativ erforderliche Narkosekonzentration. New York thoden sind bei einem Groi3teil des kinderchirurgischen Tagesprogramins anwendbar und werden von uns seit mehreren Jahren routinemdig durchgeführt (9). Diese T e c h d e n kommen in den von Ihnen als ,,kontaminiert und unhygienisch" bezeichneten Regionen zur Anwendung. Dies suggeriert eine nicht zu realisierende sterile Arbeitsweise und damit ein unzulässig erhöhtes Infektionsrisiko. Weder Berichte aus der Literatur noch unsere eigenen Erfahrungen lassen diesen Schlug zu.
