Zusammenfassung Hintergrund Unterscheidet sich der Rehaverlauf von akut Covid-19-Erkrankten von genesenen Covid-19-Patienten und anderen Rehapatienten? Vergleich des Verlaufs zwischen akut Covid-19-Erkrankten, genesenen Patienten und Rehapatienten ohne Covid-19. Design: Fall-Kontroll-Studie. Material und Methoden Einschluss von im Zeitraum 01.04. bis 30.06.2020 zugewiesenen Patienten mit aktiver Covid-19-Erkrankung (IG), genesenen Covid-19-Betroffenen (PCG) und einer gleichzeitig rehabilitierten, nach Alter, Geschlecht und Komorbidität gematchten Kontrollgruppe (Ko). Primärer Endpunkt war die Selbständigkeit im Alltag (FIM), weitere Endpunkte waren Lebensqualität (EQ5D-VAS), physische und mentale Gesundheit (PROMIS10) sowie Mobilität (TUG und 6MWT). Ergebnisse 101 Patienten wurden eingeschlossen: IG=15; PCG=22; Ko=64. Die Selbständigkeit und Lebensqualität bei Eintritt waren in der IG signifikant am höchsten (FIM IG 95,3 vs. PCG 78,3, vs. Ko 79,6; ANOVA p=0,029; EQ5D-VAS IG 56.1 vs. PCG 36,6 vs. Ko 51,0; ANOVA p=0,010). Im PROMIS10 sowie in der Mobilität gab es keine Unterschiede. Alle drei Gruppen verbesserten sich von Ein- zu Austritt signifikant. Die IG schnitt bei Austritt im FIM am besten ab (IG 109,7 vs. PCG 97,2 vs. Ko 93,6; ANOVA p=0,039). Die Kontrollgruppe erreichte hinsichtlich Mobilität bei Austritt die schlechtesten Resultate (TUG Ko 19,3 Sek. vs. 9,6 Sek IG und 8,3 Sek PCG). Im 6MWT zeigte die PCG das beste Resultat (471 m vs. IG 416 m und Ko 389 m; Differenz n.s.). Schlussfolgerung Alle drei Patientengruppen konnten sehr gut vom Rehaprogramm profitieren. Rehamassnahmen bringen auch für isolierte akut Covid-19-Erkrankte einen Benefit. Insbesondere genesene Covid-19-Patienten profitieren sehr von den Rehamassnahmen.
Neck pain is a common problem in the general population with high risk of ongoing complaints or relapses. Range of motion (ROM) assessment is scientifically established in the clinical process of diagnosis, prognosis and outcome evaluation in neck pain. Anatomically, the cervical spine (CS) has been considered in two regions, the upper and lower CS. Disorders like cervicogenic headache have been clinically associated with dysfunctions of the upper CS (UCS), yet ROM tests and measurements are typically conducted on the whole CS. A cross-sectional study assessing 19 subjects with non-specific neck pain was undertaken to examine UCS extension-flexion ROM in relation to self-reported disability and pain (via the Neck Disability Index (NDI)). Two measurement devices (goniometer and electromagnetic tracking) were employed and compared. Correlations between ROM and the NDI were stronger for the UCS compared to the CS, with the strongest correlation between UCS flexion and the NDI-headache (r =-0.62). Correlations between UCS and CS ROM were fair to moderate, with the strongest correlation between UCS flexion and CS extension ROM (r =-0.49). UCS flexion restriction is related to headache frequency and intensity. Consistency and agreement between both measurement systems and for all tests was high. The results demonstrate that separate UCS ROM assessments for extension and flexion are useful in patients with neck pain.
Zusammenfassung Hintergrund: 80?% der Schweizer leiden mindestens einmal im Jahr an Low Back Pain (LBP). Es ist unklar, ob Spondylodese oder konservative Therapie (Physiotherapie) bei chronischem LBP (CLBP) effektiver ist. Ziel: Vergleich des Behandlungseffekts von konservativer Therapie (Physiotherapie) und Spondylodese bei LBP (spezifisch oder unspezifisch) hinsichtlich Schmerz und Behinderung. Methode: Der systematische Review mit Literatursuche erfolgte in Pubmed, PEDro, Cochrane und CINAHL, die Risk-of-Bias-Analyse mit den PEDro-Kriterien (??6/10?=?gut). Ergebnisse: Von 82 Publikationen erf?llten 10 alle Einschlusskriterien und betrafen 4 Studien. Von den 4 Studien hatten 3 ein geringes Risk of Bias. 2 Studien davon untersuchten Patienten mit Diskusdegeneration oder CLBP ?ber 1 Jahr nach einer Diskushernienoperation. Das Outcome nach 1, 4 und 9 Jahren war nach konservativer Therapie (Physiotherapie mit kognitiven ?bungen) und Spondylodese gleich gut. Eine 3. Studie mit einem h?heren Risk of Bias zeigte nach 2 Jahren einen statistisch signifikanten, aber nicht klinisch relevanten Vorteil der Spondylodese. Laut der Ergebnisse der 4. Studie war bei isthmischer Spondylolisthesis die Spondylodese nach 2 Jahren effektiver als konservative Therapie (Physiotherapie mit kognitiven ?bungen). Nach 5???13 Jahren ergab sich kein Unterschied. Schlussfolgerungen: Bei CLBP aufgrund diskogener Ursache ist konservative Therapie (Physiotherapie mit kognitiven ?bungen) bez?glich Schmerz und Behinderung gleich effektiv wie Spondylodese. Im Falle von CLBP bei isthmischer Spondylolisthesis erweist sich die Spondylodese bis 2 Jahre nach der Intervention als effektiver, wohingegen es nach durchschnittlich 9 Jahren keinen Unterschied bez?glich Schmerz und Behinderung zwischen den beiden Interventionen gibt.
