Already 94 years ago in 1910, Dr. Hans Christian Jacobaeus performed the first clinical laparoscopic surgery in Stockholm. His pioneering procedure was based on the animal experiments of Georg Kelling (1866-1945), a German physician from Dresden, who performed the first laparoscopic intervention in 1901 using a Nitze cystoscope in a dog. In 1910 Jacobaeus published his first experiences with laparoscopic surgery in the Münchner Medizinische Wochenschrift under the title "The possibility to perform cystoscopy in examinations of serous cavities." He used this technique for diagnostic purposes in unclear abdominal complaints and functional impairment. Jacobaeus was the first who pointed out the possibility of causing injury to organs, especially the gut, by inserting the trocar. In 1910 Jacobaeus recognized the immense diagnostic and therapeutic possibilities of laparoscopic surgery, but also the difficulties and limits. He also was the first who recognized the need to complete training sessions on animals and corpses. He demanded the development of special laparoscopic instruments to optimize and simplify the operation.
Stress incontinence is the most frequent form of incontinence found in females. The usual method of surgery for this is to lift the bladder neck towards cranial and ventral. Two competitive techniques--bladder neck suspension in accordance with Stamey and colposuspension in accordance with Burch--were retrospectively investigated. A total of 95 women underwent surgery and it was possible to evaluate the postoperative course in 46 and 30 (total 76) patients respectively. Initially, both surgical techniques demonstrated a very good success rate (91.3% and 96.7% resp.). However, the continence rate of the Stamey patients deteriorated after 6 weeks down to 78.3% and after more than 2 years only 28.6% of these patients were still completely continent (mean follow-up 40.4 mths), whereas deterioration in the Burch patients was only 87% (mean follow-up 44.25 mths). The disappointing results with the Stamey technique did not correlate with the degree of initial continence. It was noted here that obese patients showed a greater tendency towards regression. The three cases of recurring incontinence after Burch colposuspension were already relapses at the time of surgery. With respect to long-term follow-up, our results with the Stamey method were distinctly poorer than with the Burch technique: Therefore, in our opinion, the Burch colposuspension procedure and fascioplasty should be the method of choice for the management of stress incontinence.
Die Zirkumzision, eine der ältesten Operationsmethoden, ist die häufigste Operation am männlichen Geschlecht. Sie wird in verschiedenen Kulturkreisen, wie Juden, Moslems, Aborigines und einigen afrikanischen Stämmen auch heute noch regelmäûig durchgeführt. Beweggründe für die routinemäûige Durchführung waren bzw. sind religiöser oder hygienischer Art und eine Prophylaxe gegen Harnwegsinfektionen, Zervixund Peniskarzinome. Bei heiûem, trockenen Klima und gleichzeitig mäûiger Hygiene konnte seinerzeit ein gesundheitlicher Nutzen aus der Zirkumzision resultieren. Aufgrund der guten hygienischen Bedingungen haben diese Motive heute nicht nur im westlichen Europa an Bedeutung verloren. Auch der Wunsch nach einer prophylaktischen Zirkumzision, die in Studien aus den Vereinigten Staaten mit Komplikationsraten bis zu 35 % beschrieben wird, ist in unseren Breiten selten. Bei bestehender Phimose wünschen Eltern und Kind häufig eine Vorhautplastik, d. h. eine Korrektur unter Erhaltung der Vorhaut, um das ,,natürlicheª Aussehen des Penis beizubehalten. So kann heute aus medizinischer Sicht meist das persönliche ¾sthetikempfinden die Frage nach der Vorhauterhaltung entscheiden. ZusammenfassungEinleitung: Trotz millionenfacher jährlicher Hernienreparationen weltweit ist ein verbindlicher Standard bezüglich der anzuwendenden Techniken auch mehr als 100 Jahre nach Bassini nicht erkennbar. Zielsetzung dieser Arbeit ist die Analyse der verschiedenen Techniken, um hierdurch die Wahl des jeweiligen optimalen Verfahrens zu erleichtern. Techniken: Neben herniorrhaphischen Verfahren wie den Reparationstechniken nach Shouldice, Bassini oder Lotheisen und Mc Vay wurden hernioplastische Verfahren insbesondere in der Technik nach Lichtenstein in den vergangenen Jahren zunehmend populär. Dieser Technik folgt auch die laparoskopische Hernienchirurgie, welche transperitoneal oder extraperitoneal durchgeführt werden kann. Ergebnisse: Bei den herniorrhaphischen Techniken ist die Technik nach Shouldice, insbesondere bei Rezidivhernien, der Technik nach Bassini überlegen. Die Hernienplastik nach Lichtenstein zeigt nahezu keine Rezidive, die laparoskopischen Daten nähern sich diesen Ergebnissen an, wobei hier auch die Einflüsse der Lernkurve berücksichtigt werden müssen. Offene Verfahren ermöglichen intraoperativ den Entscheid über das adäquate Therapieverfahren (Raphie oder Plastik), wodurch Übertherapien bei niedriggradigen Nyhus-Hernien vermieden werden. Zumindest bei Rezidiv-und beidseitigen Hernien sollte das laparoskopische Vorgehen die Therapie der Wahl darstellen. Auch unter sozio-ökonomischen Gesichtspunkten ist die laparoskopische Technik das Verfahren der Wahl. Schlussfolgerung: Beim offenen Vorgehen empfiehlt sich die ¹tension-freeª-Technik in Abhängigkeit von der Nyhus-Klassifikation (höhergradige Nyhus-Hernien), beim laparoskopischen Vorgehen die extraperitoneale Technik. Abschlieûend muss die Frage nach der adäquaten Technik mittels prospektiv-randomisierter und einheitlich klassifizierter (Nyhus) Studien erfolgen. AbstractIntroduction: More than 100 years after Bassini, the technical approach to hernia is still a subject of debate, despite the mag-nitude of hernia repairs performed worldwide each year. We evaluated the outcome of the most common technical approaches in order to specify adequate treatment options. Techniques: Besides the herniorrhaphy procedure (Shouldice, Bassini, Lotheisen and McVay), hernioplasty (Lichtenstein) has gained popularity over the last few years. This technique is also performed laparoscopically ± either trans-or extraperitoneally. Results: The Shouldice procedure is superior to the Bassini method, especially for recurrent hernias. The success of hernioplasty (Lichtenstein) shows almost no recurrencies. Similar results are demonstrated by the laparoscopic procedure. However, these latter results are still influenced by the learning curve. Only open surgical management allows intraoperative decision on the most suitable procedure (-raphy or -plasty), thus avoiding overtreatment of low-degree Nyhus hernias.
