OBJECTIVE:Treponema pallidum is the etiological agent of congenital syphilis, which results from fetal contamination by the infected mothers, who were not treated or were inadequately treated during pregnancy. METHODS: An observational, prospective and longitudinal study, was performed (2010)(2011)(2012)(2013)(2014), through the evaluation of 428 newborns during 18 months in a syphilis clinic from a Philanthropic Maternity Hospital in Aracaju, capital city of the Northeastern state of Sergipe, Brazil. The findings were statistically expressed as descriptive data and the statistical program used was SPSS (Statistical Package for Social Sciences). RESULTS AND CONCLUSIONS:The prevalence of congenital syphilis was 10.02/1,000 live births. A total of 120 (28%) of newborns did not attend the first appointment. During the observational period, at 18 months, the rate of abandonment was 75%. The average interval of healing of the newborns was 4.25 months. A high prevalence of congenital syphilis was found with low adhesion to the first consultation and monitoring period; 67.1% of newborns were treated with Crystalline Penicillin (Penicillin G) and only 3% of them required a repeat treatment.
Esse artigo buscou analisar a ocorrência de sífilis congênita nas regiões do Brasil no período de 2010-2018. A maioria dos casos da doença ocorre porque a mãe durante o pré-natal não foi testada para sífilis ou porque o tratamento preconizado pelo Ministério da Saúde não foi realizado adequadamente antes ou durante a gestação. Trata-se de um estudo que teve como instrumento de coleta de dados o Sistema de Informação de Agravos de Notificação (SINAN) do Ministério da Saúde. Foram utilizados como variáveis: zona de moradia, escolaridade, pré-natal, raça, tratamento do parceiro, classificação final da sífilis. O presente estudo veio corroborar com a persistência da alta prevalência dos casos de sífilis congênita em várias regiões brasileiras. Sua ocorrência vem aumentando nos últimos anos: 2017 (24.666 casos), 2018 (26.219 casos) e estar associada ao manejo inadequado dos casos, baixo nível socioeconômico e sociodemográfico, além da baixa qualidade do pré-natal recebido pelas gestantes. Ressaltou-se também que a baixa adesão do parceiro ao tratamento esteve relacionada às causas sociais e ao aumento dos casos de sífilis congênita.
Introduction: Syphilis is an infectious disease caused by Treponema pallidum, its two main routes of transmission are sexual and transplacental (vertical). The latter is of particular worrisome, since it can generate congenital syphilis and can be avoided by early maternal serological screening. Objective: To analyze the conditions of prenatal care for syphilitic pregnant women in Sergipe State between 2007 and 2019. Methods: A cross-sectional, retrospective, and descriptive study was carried out, with the collection of notified cases of gestational and congenital syphilis in the Brazilian Notifiable Diseases Information System (Sistema de Informação de Agravos de Notificação - SINAN). Results: There was a considerable increase in the number of reported cases of gestational syphilis in the last 13 years. About 36% of pregnant women were identified in the 3rd trimester of pregnancy, 68.1% were brown, 56.8% had studied for up to 8 years, and 50.1% were between 20 and 29 years old. The clinical phase latent to the diagnosis was responsible for 70.3% of the cases, followed by the primary (11%) and tertiary (7.3%) phases. Of the total number of pregnant women, 20.2% did not perform the non-treponemal test, and 97.2% were treated with penicillin. Regarding the numbers of congenital syphilis, although 75% of the mothers performed prenatal care, 37.8% received the diagnosis at the time of delivery/curettage, resulting in 72.9% of infant deaths from the disease. Moreover, there was a predominance of untreated partners (77.7%) in relation to those treated (10.8%). Conclusion: Although most of them performed prenatal care, there was a predominance of diagnoses performed only in the 3rd trimester of pregnancy, mainly at the time of delivery or curettage, not respecting the minimum therapeutic interval of 30 days before delivery. Thus, in Sergipe State, the most important factor in the high prevalence of vertical transmission of syphilis is the ineffectiveness of prenatal care provided to infected pregnant women, which remains.
Objetivo: Avaliar o impacto do perfil socioeconômico de gestantes e dos parceiros no número de consultas do pré-natal de recém-nascidos de uma maternidade de ensino no município de Aracaju-Sergipe. Métodos: Trata-se de um estudo transversal, retrospectivo e analítico. Foram analisados prontuários de crianças que necessitaram de internação em Unidade de Terapia Intensiva Neonatal (UTIN) no período compreendido entre janeiro de 2018 e junho de 2020. Foram incluídas variáveis dos genitores. Resultados: Entre genitoras dos 261 pacientes, houve uma média da idade de 25,4 anos e 56,1% possuíam atividade trabalhista. Em relação à escolaridade 56% possuíam ensino médio, 35,6% eram analfabetas e 8,4% possuíam ensino superior. Em relação ao número de consultas pré-natal, 74,9% tiveram 6 ou mais consultas, enquanto 2,7% não tiveram nenhuma. Ademais, 30,4% dos parceiros possuíam vínculo empregatício. Conclusão: A população avaliada obteve baixa escolaridade e porcentagem reduzida de vínculo empregatício, o que contribui para a dificuldade de adesão do pré-natal assim como acompanhamento necessário, podendo resultar em riscos materno-fetais. Evidencia-se necessidade do pré-natal completo, além da tentativa de reduzir os fatores socioeconômicos que impactam negativamente.
Esse artigo buscou avaliar a prevalência de Sífilis Adquirida em território brasileiro no período de 2010 a junho de 2019. Foi realizado um estudo transversal, observacional, retrospectivo e de caráter analítico e quantitativo, a partir de uma coleta de dados do Sistema de Informação de Agravos de Notificação (SINAN) do Ministério da Saúde no período de 2010 a junho de 2019. Foi observado um aumento dos casos de Sífilis Adquirida no Brasil, com maior taxa de incidência na região Sudeste e a menor na região Norte. Percebeu-se que o perfil das pessoas acometidas por Sífilis Adquirida, no Brasil, no mesmo intervalo de tempo, foi do gênero masculino, com 59,6%; com idade entre 20 a 29 anos, 33,6%; analfabetos (1,2%), com ensino fundamental incompleto (1ª a 4ª incompleta (5,8%), 4ª completa (3,6%) e 5ª a 8ª incompleta (12,1%)) e completo (7,9%); e de cor branca (37,8%), parda (34,2%) e preta (9,7%). O estudo das taxas de Sífilis Adquirida é uma ferramenta de grande valia para a saúde pública, possibilitando o planejamento de prevenção e controle de acordo com a epidemiologia, estimulando-se à adesão ao tratamento e seu seguimento.
