RESUMOObjetivo: analisar os fatores relacionados à não investigação dos óbitos fetal e infantil ocorridos no Vale do Jequitinhonha, Minas Gerais. Métodos: trata-se de estudo ecológico que teve, como unidades de análise, todos os 33 municípios da Região Ampliada de Saúde Jequitinhonha, Minas Gerais. Nesses municípios foi aplicado um questionário a um membro do Comitê de Prevenção do Óbito Infantil e Fetal com questões sobre a composição dos comitês, mecanismos de investigação do óbito infantil, fatores dificultadores para a investigação dos óbitos e critérios de evitabilidade entre os anos 2007 e 2012. Foi feita análise descritiva com frequências simples, com o intuito de descrever o perfil dos municípios estudados. Resultados: dos 598 óbitos infantis e 477 fetais registrados, apenas 22,2 e 18,4% foram investigados, respectivamente. Constatou-se a existência de problemas de infraestrutura, técnico-operacionais e políticos que interferiram na prática de investigação. Conclusão: a operacionalização da investigação do óbito infantil e fetal, apesar de exigido por lei, apresenta deficiências, o que acarreta elevado número de óbitos não investigados no período do estudo, o que poderá comprometer as ações para a redução da mortalidade infantil e fetal e a qualidade da assistência infantil. ABSTR ACTThis study aims to analyse aspects related to the lack of investigation of foetal and infant deaths in the Jequitinhonha Valley, State of Minas Gerais. This is an ecological study whose unit of analysis were the 33 municipalities in the Extended Jequitinhonha Health Region. A questionnaire was applied to a member of the Infant and Foetal Death Prevention Committee of each municipality. The questions were related to the committees' structure, mechanisms of infant death investigation, difficulties related to the investigation of deaths, and avoidable criteria between 2007 and 2012. A descriptive analysis with simple frequencies aiming at profiling the studied municipalities was performed. Of the 598 infant and 477 foetal deaths registered, only 22.2% and 18.4%, respectively, were investigated. The researchers detected that infrastructure, technical, operational and policy issues interfered with the investigation. Although required by law, the investigation over infant and foetal deaths is deficient. This led to a large number of deaths being left uninvestigated. Such situation may hinder programmes for the reduction of infant and foetal mortality and the quality of childcare.
The objective of this study was to analyse the effect of using two health education approaches on knowledge of transmission and prevention of schistosomiasis of school children living in a rural endemic area in the state of Minas Gerais, Brazil. The 87 children participating in the study were divided into three groups based on gender, age and presence or absence of Schistosoma mansoni infection. In the first group the social representation model and illness experience was used. In the second group, we used the cognitive model based on the transmission of information. The third group, the control group, did not receive any information related to schistosomiasis. Ten meetings were held with all three groups that received a pre-test prior to the beginning of the educational intervention and a post-test after the completion of the program. The results showed that knowledge levels in Group 1 increased significantly during the program in regard to transmission (p = 0.038) and prevention (p = 0.001) of schistosomiasis. Groups 2 and 3 did not show significant increase in knowledge between the two tests. These results indicate that health education models need to consider social representation and illness experience besides scientific knowledge in order to increase knowledge of schistosomiasis transmission and prevention.Key words: schistosomiasis -health education -evaluation models -Brazil Schistosomiasis is a serious public health problem with more than 200 million individuals infected in the world and approximately 2.5 milion in Brazil and is also one of the most difficult parasitic diseases to control (WHO 2001). Reasons for the persistence of schistosomiasis in spite of prolonged and intensive control and prevention efforts include migration patterns and residence, the wide distribution of the intermediate hosts, the dependence of many poor populations in both rural and urban areas on schistosome-infested water sources for their domestic, occupational, and recreational needs, lack of sanitation and potable water, and scarcity of and deficiencies in preventive and curative health services (Coura-Filho 1998, Ximenes et al. 2000, WHO 2001, Funasa 2002. Chemotherapy is still the main means of managing morbidity from schistosomiasis. However, the prevention of reinfection, the infection of others, and the reduction in the incidence of infection require that attention be given to socioeconomic, cognitive, behavioral, and other factors (Kloos 1995, Barbosa 1998, Uchôa et al. 2000, Asaolu & Ofoezie 2003. Introdução: a redução da mortalidade materna e infantil é prioridade internacional e nacional devido ao alto potencial de evitabilidade desses óbitos. Os Comitês de Prevenção de Óbitos Maternos, Fetais e Infantis se configuram como um importante mecanismo de vigilância da mortalidade materna e infantil. Objetivo: avaliar a estrutura, processos e resultados dos comitês de prevenção de óbitos nos municípios da Unidade Regional de Saúde de Belo Horizonte-MG segundo porte populacional. Método: tratase de estudo avaliativo desenvolvido na Regional de Saúde de Belo Horizonte no ano de 2015. Utilizou-se questionário aplicado às referências técnicas municipais responsáveis pela vigilância dos óbitos maternos, fetais e infantis. Os municípios foram analisados quanto à adequação às normas nacionais e estaduais, nos domínios estrutura, processo e resultado, segundo categorias de porte populacional. Para a avaliação de cada domínio foram somados os itens adequados, estabelecendo-se um escore de adequação. Resultados: foram avaliados 38 municípios e constatados diversos níveis de adequação dos comitês, sendo os piores percentuais para a estrutura (5,3% como adequados) e os municípios de menor porte. Nos domínios processo e resultado, o percentual de adequação foi 30,6%. Conclusão: as inadequações evidenciadas revelaram a necessidade de se estruturar os comitês municipais com provisão de investimentos financeiros, técnicos e profissionais, de forma a otimizar sua capacidade operacional e de resposta ao óbito ocorrido. Outra melhoria necessária é a expansão das ações técnicas e políticas dos comitês em conjunto com o controle social.
Objetivo: analisar a existência de disparidade urbano-rural na utilização de serviços de saúde por pessoas vivendo em um município de pequeno porte. Métodos: estudo epidemiológico transversal com amostra representativa e aleatória de pessoas com 18 anos ou mais de idade na área urbana (n=1.235) e toda a população de um distrito rural (n=190). Utilizou-se o questionário sobre utilização de serviços da Pesquisa Nacional por Amostra de Domicílios. A análise considerou modelos de regressão logística para examinar fatores associados à utilização nos 30 dias anteriores ao estudo. Resultados: a taxa de utilização pelas pessoas da área urbana foi significativamente maior que da área rural (45,0% vs 15,8%). Mulheres e aqueles que reportaram sintomas de doenças no período e hipertensão arterial tiveram mais chances de utilização. Conclusão: pessoas residentes em área rural utilizam menos serviços de saúde quando comparadas àquelas da área urbana quando estão sob a mesma organização municipal de saúde.
