Williams-Beuren syndrome (WBS) is caused by a 1-2 Mb microdeletion in the region 7q11.23. The clinical presentation may vary and most of the connective tissue abnormalities can be explained by the haploinsufficiency of the ELN gene in this region. The purpose of this study was to determine the value of a polymerase chain reaction assay that uses three polymorphic markers to detect the microdeletion and compare the clinical features. Thirty-two patients with WBS were ascertained accordingly to clinical diagnostic criteria. The markers D7S1870, ELN 17/exon 18, and Hei 1.3/1.4 were designed to detect the heterozygosity in the region 7q11.23. The three markers were informative in 78% and uninformative in 22% of the cases. The most informative marker (69%) was D7S1870, followed by Hei (55%) and ELN 17/exon 18 (44%). The microdeletion was present in 56% and absent in 22% of patients. The craniofacial and cardiovascular abnormalities did not have significant statistical differences in the cases with and without microdeletion. Two of the syndrome characteristics (an overfriendly personality and hyperacusis) were more frequent in the microdeletion group and these differences were statistically significant (p = 0.006 and p = 0.02, respectively). Polymorphic markers might be a good alternative for the molecular diagnosis of WBS in centers where fluorescence in situ hybridization analysis is not available.
a ResumoO excesso de veículos circulantes tem como consequência o aumento de acidentes automobilísticos. Desta forma, este índice se tornou uma preocupação, devido ao tempo de reabilitação e reinserção social do indivíduo. É comum em acidentes ocorrerem casos de fratura, de forma que nem sempre, a consolidação acontece de maneira adequada. Com isso, em toda falha na fusão óssea, a etiologia pode apresentar diversos fatores como causa, de uma patologia denominada como Pseudoartrose. O presente estudo tem como objetivo relatar um caso desta doença: paciente do sexo masculino, 23 anos, com a aquisição de politraumatismo, ocasionando fratura diafisária em terço distal de fêmur direito. Como intervenção, foi submetido a tratamento cirúrgico tardio e implantação de placa de compressão. Após o ato cirúrgico foi observada a não consolidação óssea devido à ausência de repouso do paciente no pós-operatório, apresentando então um prognóstico desfavorável, havendo a necessidade de troca da osteossíntese. A nova abordagem escolhida para o caso foi a implantação de osteossíntese com estabilidade absoluta, mais conhecida como Illizarov. Palavras-chave:Pseudoartrose. Fraturas do Fêmur. Estima-se que aproximadamente 5% de fraturas de ossos longos evoluem para tal, que se associa a sintomas específicos e difícil tratamento clínico [1][2][3][4] . Todavia, constitui um processo grave, capaz de gerar incapacidade em nível severo ou lesão permanente nos pacientes. Exemplificação disto é que a patologia por exibir a ausência de consolidação, progressivamente ao longo do caso, acarreta mobilidade anormal no membro afetado 5 . Sendo assim, mostra-se a necessidade de esforços mais apurados em seu entendimento e na proposição de medidas efetivas, que visem amenizar a gravidade dos casos e conduzam à evolução favorável na abordagem terapêutica adotada. Segundo Reis 1 o termo "Pseudoartrose" é aplicado para designar a situação em que a fratura não mostra evidencias radiográficas de progressão do processo de consolidação óssea, indicadas por esclerose nas extremidades da fratura, presença de um hiato, calo ausente ou hipertrófico e persistência ou alargamento do traço de fratura. Quando pronunciada essa nomenclatura, existe uma associação á falsa artrose, porém não pode ser considerada, pelo fato de que a definição desta doença é baseada em um processo patológico crônico das articulações caracterizado pela degeneração da cartilagem e dos ossos proximais, onde é observada a presença de sinais clássicos como dor, rigidez e redução da funcionalidade...
