Homem, 33 anos, usuário de drogas, foi encontrado em via pública confuso e com dificuldade para deambular, sendo então encaminhado à unidade de pronto-atendimento. Ao exame físico apresentava desvio da comissura labial e perda da força muscular à esquerda. Não apresentava febre. Foi realizada Ressonância Nuclear Magnética (RNM) de crânio que mostrou lesões multifocais supra e infratentoriais, de aspecto nodular, muitas com realce periférico pelo contraste (figura 1). A análise do líquor não mostrou anormalidade. O teste ELISA foi positivo para HIV e as sorologias tanto para toxoplasmose como para Citomegalovírus (CMV) foram IgM negativas e IgG positivas.Figura 1. Qual sua hipótese diagnóstica? a) Linfoma primário do SNC b) Leucoencefalopatia multifocal progressiva c) Neurotoxoplasmose d) Ventriculite por CMV e) Tuberculoma A neurotoxoplasmose é a principal infecção oportunista do sistema nervoso central e uma das principais causas de déficits neurológicos focais em pacientes infectados pelo HIV. Em geral, decorre da reagudização de uma infecção latente pelo Toxoplasma gondii (pacientes com títulos IgG positivos), associada a níveis de CD4+ inferiores a 200/mm 3,1 . A doença geralmente se instala de forma insidiosa, evoluindo no decorrer de dias ou semanas. Os sinais e sintomas dependem da localização das lesões. Inicialmente, os pacientes podem apresentar confusão mental, cefaleia e febre, em associação com déficits neurológicos focais. Com a progressão da doença podem surgir convulsões, afasia, fraqueza motora e perturbação da fala 2 . O diagnóstico baseia-se na apresentação clínica e nos exames de neuro-imagem, associados à resposta terapêutica com sulfadiazina e pirimetamina. A presença de anticorpos (IgM ou IgG) no soro ou no líquor não confirma o diagnóstico e a sua ausência não o exclui. A maior utilidade na análise do líquor é a de afastar infecções concomitantes, principalmente a criptococose, porém também podem estar associados: herpes simples, candidíase, citomegalovírus ou sífilis. O diagnóstico definitivo é dado pela biópsia cerebral, mas esta é raramente indicada pelo risco de sangramentos, de danos aos tecidos circundantes, e infecção. A biópsia fica reservada aos pacientes cujo diagnóstico é incerto e aqueles que não respondem ao tratamento empírico 3 . Na suspeita clínica de neurotoxoplasmose, está indicada a realização de tomografia computadorizada ou ressonância nuclear
We describe a case of probable sporadic Creutzfeldt–Jakob disease in the setting of well-controlled HIV and discuss whether exist, in fact, HIV-related factors that may predispose to the development of prion disease. To the best of our knowledge, this is the third report of this association.
