RezumatÎn ultimele decenii am asistat la dezvoltarea tehnicilor chirurgicale în paralel cu progresul tehnologic, cu apariţia unor dispozitive din ce în ce mai sofisticate care au ca scopuri o intervenţie chirurgicală cât mai puţin traumatizantă pentru pacient dar şi obţinerea unor rezultate cât mai bune perioperator şi la distanţă. După publicarea rezultatelor trialurilor randomizate, ghidurile actuale recomandă laparoscopia ca opţiune de tratament pentru cancerul de colon acolo unde există expertiză. Spre deosebire de colectomiile laparoscopice, rezecţiile de rect prin abord laparoscopic sunt intervenţii dificile tehnic şi sunt asociate cu o rată de adopţie mică, o rată de conversie ridicată, rezultate funcţionale şi oncologice discutabile. Aceste dezavantaje au creat nevoia unui instrument mai bun. Sistemul robotic a fost conceput pentru a depăşi limitele laparoscopiei în ceea ce priveşte vizibilitatea si manevrabilitatea instrumentelor. Abordul laparoscopic este indicat în cancerul de colon şi de rect superior, în timp de abordul robotic facilitează mult intervenţiile pentru cancer de rect mediu şi inferior (în special la bărbaţi -pelvis îngust şi la pacienţii cu IMC ridicat). În aceste cazuri avantajele tehnice oferite de sistemul robotic facilitează mult abordul minimal invaziv şi permite, de cele mai multe ori, prezervarea plexurilor nervoase, conducând implicit la o calitate a vieţii mai bună. Datorită costului ridicat şi disponibilităţii scăzute a acestei metode, este greu de anticipat dacă va duce o creştere semnificativă a ratei de adopţie a chirurgiei minimal invazive în cancerul colorectal.Cuvinte cheie: chirurgie robotică, chirurgie laparoscopică, chirurgie minimal invazivă, cancer de rect, cancer de colon Abbreviations: BMI -Body Mass Index; RCT -Randomized Control Trial;
Cystic echinococcosis is a zoonosis caused by the larval stage of Echinococcus granulosus. Liver and lungs are the most commonly affected organs whereas splenic infection is rare and its primary involvement occurs in less than 2% of cases. We report a case of primary giant splenic and hepatic hydatid cyst in a 28-year-old woman who was admitted for upper right quadrant pain. The abdominal ultrasonography and computed tomography showed two cystic tumors with hydatid features in liver and spleen. Total splenectomy was performed for the splenic cyst and partial pericystectomy with drainage for the liver cyst using a laparoscopic approach. One drain was kept in place for two months due to a biliary leak of about 20 ml/day and removed afterward. The patient was discharged on postoperative day 7. Laparoscopic approach for patients with concomitant splenic and hepatic hydatidosis is a safe and effective option.
Background: In the last few decades central pancreatectomy is increasedly adopted in treatment of benign pancreatic lesions situated in the body of the pancreas, especially in young patients. With recent advances in technology, patients with these lesions could benefit from the advantages of the robotic approach. Case presentation: We present the case of a 30-years old woman investigated for epigastric pain. CT showed a 3 cm cystic tumor located in the proximal part of the pancreas body. Robotic central pancreatectomy with pancreatogastrostomy was performed. Results: Operative time was 220 minutes with minimal blood loss. The postoperative course was marked by a basal left pneumonia on postoperative day 3 treated conservatively. The patient was discharged home on postoperative day 8. Pathology showed a 30 mm well differentiated neuroendocrine tumor with free resection margins. Conclusions: Robotic systems offer several advantages compared to laparoscopy: increased and stabile tridimensional view of the operative field and articulated instruments. Robotic central pancreatectomy seems to be a suitable candidate for robotic surgery requiring delicate vascular dissection and a safe reconstruction. Although there are only few cases published in the literature so far, robotics could lead to an increased adoption of minimally invasive techniques in difficult pancreatic procedures, especially in those requiring reconstructions.
RezumatSpleno-pancreatectomia efectuată pentru tratamentul pseudoanevrismelor arterei splenice localizate la nivelul unui pseudochist pancreatic ce comunică cu tubul digestiv (stomac sau colon) reprezintă una din indicaţiile rare, puţine astfel de proceduri fiind descrise în literatură. Tratamentul pseudo-anevrismelor peripancreatice este complex şi dificil. Tratamentul chirurgical este curativ şi poate include rezecţia altor organe. Intervenţiile chirurgicale pot fi executate in condiţi de urgenţă, dacă pacientul prezintă sângerare cataclismică sau în condiţii elective în cazul în care diagnosticul se pune accidental sau dacă pacientul reuşeşte să depăşească episodul iniţial de sângerare. În această lucrare prezentăm două cazuri de pseudochist pancreatic ce asociază pseudoanevrism al arterei splenice şi fistula cu tubul digestiv (comunicare pseudochisto-colonică şi pseudochisto-gastrică). Amândoi pacienţii sunt bărbaţi, cunoscuţi cu pancreatită cronică şi pseudochiste pancreatice. Primul pacient a fost operat electiv după investigaţii amănunţite. Cealaltă intervenţie chirurgicală a fost efectuată în condiţii de urgenţă pentru a controla un episod de sângerare cataclismică. Concluzionăm ca abordul chirurgical rămâne principala metodă pentru tratamentul acestor cazuri.
