Objetivo: Evaluar los hallazgos clínicos y hormonales relacionados con la poliquistosis ovárica. Diseño: Estudio retrospectivo. Lugar: Servicio de Ginecología e Infertilidad del Hospital Arzobispo Loayza. Material y Métodos: Se determinó el perfil hormonal de 69 pacientes en edad reproductiva con diagnóstico de ovarios poliquísticos por ecografía transvaginal. Resultados: Se encontró oligoamenorrea en 67,7%, infertilidad en 28,8%, hirsutismo en 23,7% e índice de masa corporal > 25 en 42,4%. El LH estuvo elevado en 47,1 %, con relación LH/FSH > 2 en 24,5% y testosterona elevada en 20,9 %. Se observó que 64,3% de las mujeres obesas presentaban oligoamenorrea, 42,8% hirsutismo, 50% infertilidad y 50% elevación de LH. Conclusión: Se observó una correlación hormonal y clínica con la presencia de obesidad, en pacientes con poliquistosis ovárica.
Objetivo: Determinar los factores de riesgo, asociados al retardo de crecimiento fetal Diseño y Método. Estudio caso control en el Servicio de ObstetriciaHospital Loayza, durante 1998. De 396 neonatos vivos con retardo de crecimiento, se tomó una muestra representativa (144). Se evaluó indicadores maternos y perinatales. Resultados: La edad materna, control prenatal, peso preconcepcional, talla materna, índice de masa corporal, ganancia de peso materno y la hemoglobina en el embarazo no mostraron diferencias significativas. Las patologías asociadas al retardo de crecimiento fetal (p < 0.05) fueron el embarazo múltiple, preeclampsia, insuficiencia uteroplacentaria, parto pretérmino y sífilis. La vía de parto fue similar en ambos grupos. El índice de Apgar fue significativamente menor en los casos, tanto al minuto como a los 5 minutos, sin llegar a la asfixia. Conclusiones: El embarazo múltiple, preeclampsia, insuficiencia uteroplacentaria, parto pretérmino y las sífilis fueron las patologías asociadas según significativamente al retardo del crecimiento fetal.
OBJETIVOS: Evaluar obstétricos y perinatales, en pacientes a término con ruptura prematura de membrana (RPM) sometidas a inducción con con oxitocina MATERIAL Y METODOS: Trabajo retrospectivo realizado en el servicio de obstetricia del Hospital Arzobispo Loayza, entre abril 1998 y marzo 1999 (1año). Incluye a todas las gestantes a término que presentaron RPM y fueron sometidas a inducción de trabajo de parto con oxitocina. RESULTADOS: 324 pacientes presentaron RPM (8,1%), 288 (88,9%) fueron gestantes a término y de éstas 89 cumplieron los criterios de inclusión. La vía de parto fue vaginal en 76 pacientes (85,3%) y por cesárea en 13 pacientes (14,6%). Nuve pacientes (10,1%) presentaron coriamnionitis. Solo 3 neonatos presentaron Apgar entre 4 y 6; 10 recien nacidos (11,2) calificaron como neonato potencialmente infectado y 14 (15,7%) tuvieron cuadro compatible con sepsias neonatal. La evolucion posterior fue buena en todos los casos. CONCLUSIONES: La inducción con oxitocina en gestantes con RPM tuvo resultado favorable en 85% de los casos, pero 15,7% de los recien nacidos presentaron cuadro compatible con infección neonatal.
Objetivos: Determinar la eficacia del uso de minidosis de ácido acetilsalicílico (aspirina) en la prevención de la preeclampsia. Diseño: Ensayo clínico aleatorio. Lugar: Servicio de Obstetricia del Hospital Arzobispo Loayza. Material y métodos: Se incluyó 43 primigestas con hipocalciuria, con menos de 20 semanas de gestación, a quienes se les administró 100 mg/día de ácido acetilsalicílico vía oral. Otras 43 primigestas con características clínicas similares no recibieron la medicación y sirvieron de control. Resultado: La incidencia de la preeclampsia fue 16,3% para ambos grupos. Conclusión: El uso de minidosis de ácido acetilsalicílico no disminuyó la incidencia de preeclampsia en este grupo de primigestas.
OBJETIVOS: Determinar las características maternas con probabilidades estadísticas de asociación con infección neo natal en pacientes ron rotura prematura de membranas (RPM), tanto en embarazos a término, como pretérmino. Evaluar la morbimortalidad perinatal MATERIAL Y MÉTODOS: Estudio observacional retrospectivo, caso-control (neonato infectado-no infectado) entre abril 1998 y marzo 1999, según protocolo establecido en el Servicio de Obstetricia del Hospital Loayza, ingresando las embarazadas con feto único, vivo, sin fiebre y con diagnóstico de RPM. RESULTADOS: De 3985 partos, 284 presentaron RPM: 263 fueron a término y 21 pretérmino; 8 neonatos no fueron evaluados (transferidos por malformaciones; cuatro maduros, cuatro prematuros). De 259 embarazos a término 42 neonatos presentaron infección (16,2%) y de 17 prematuros 11 estaban infectados (64,7%). En los embarazos a término, el estado civil soltera fue estadísticamente significativo en los casos; el control prenatal no fue determinante para infección neonatal. Existe riesgo de infección neonatal en embarazos a término desde más de 6 horas (máximo cuando sobrepasan las 24 horas); la cesárea fue significativamente mayor en los casos (47.6%) - (controles: 29.9%). Más de 60% del total de embarazos pretérmino tuvo cesárea. Los índices Apgar fueron estadísticamente menores en los casos (en embarazos a término y principalmente en pretérminos); también fueron menores en los casos prematuros vs casos maduros. En los embarazos pretérmino hubo dos óbitos y un neonato vivo falleció por prematuridad extrema. CONCLUSIONES: El estado civil soltera, tiempo de RPM después de seis horas (máximo más de 24 horas).
