OBJETIVO: Determinar la incidencia y las características de presentación de la endometriosis en pacientes con infertilidad. DISEÑO: En un estudio retrospectivo se revisó las historias clínicas de 2 720 pacientes que acudieron al Consultorio de Infertilidad del Hospital Arzobispo Loayza, entre los meses de enero de 1993 y julio de 1997. RESULTADOS: De estas pacientes 532 fueron sometidas a laparoscopias o laparotomías y se encontró 773 casos de endometriosis, lo que representa el 32,5%. Entraron al estudio 166 casos, que cumplían los criterios de inclusión. La edad de las pacientes fue 33,3 ± 4,9 años (promedio± desviación estándar). El 59% de los casos se presentó en el estadio I de endometriosis. La mayoría de las pacientes (59,6%) presentó infertilidad primaria y el tiempo promedio de infertilidad fue de 51,2 (± 38,4) meses. El 75,9% de los casos presentó algún tipo de sintomatología, principalmente dolorosa (dismenorrea, dispareunia, dolor pélvico crónico), pero sólo el 25,9% tuvo alteraciones en el examen pélvico. El 66% de pacientes tenía patología asociada, sobre todo tubárica. En cuanto al tratamiento recibido el 8,4% de los casos recibió tratamiento médico, el 29. 5% tratamiento quirúrgico; 7,8% tratamiento combinado (médico-quirúrgico); el 38,5% manejo expectante. La tasa de embarazos fue de 25%. CONCLUSION: La endometriosis fue encontrada en un tercio de mujeres infértiles en nuestro estudio.
Objetivo: Determinar el estado actual de la infertilidad masculina en un hospital general. Diseño: Trabajo de investigación descriptivo, retrospectivo. Material y Métodos: En el servicio de infertilidad del Hospital Arzobispo Loayza, se revisó 371 historias de pacientes varones atendidos en un periodo de 4 años. El criterio de inclusión fue diagnóstico de infertilidad masculina con resultados patológicos en dos o más espermatogramas y/o disfunción sexual eyaculatoria. Resultados: Se encontró 79 casos de infertilidad masculina, con incidencia 21,3%. La edad promedio de diagnóstico fue 35,9 años, tiempo de infertilidad promedio cuatro años, 64,8% con infertilidad primaria. Los diagnósticos más frecuentes fueron glándulas sexuales accesorias infectadas (38`%), anormalidad de plasma seminal (16,5%), azoospermia no determinada (15,2%), astenozoospermia idiopática (12, 7%), germen más frecuente Estreptococo alfa hemolítico (50%). Gestó 22,8% de las parejas, lo que se logró en la mayoría con antibióticos, inseminación intrauterina con semen homólogo o sin tratamiento. Conclusiones: La incidencia de infertilidad masculina en el Hospital Arzobispo Loayza en cuatro años fue 21,3%. En 87,3% de los casos coexiste la infertilidad en ambos conyuges. El diagnóstico de infertilidad masculina más frecuente es infección de las glándulas sexuales accesorias.
OBJETIVO: Determinar las características demográficas y clínicas de las pacientes con infertilidad tubárica. LUGAR: Hospital Arzobispo Loayza, entre enero y junio de 2001. MATERIAL Y MÉTODOS: Se revisó las historias clínicas de 77 pacientes con infertilidad debida a salpingitis. RESULTADOS: Cincuenta y dos (67%) pacientes tenían entre 30 y 39 años; 54 (70%) pacientes iniciaron su actividad sexual después de los 18 años, 31 (40%) tuvieron sólo una pareja sexual y 35 (46%), dos. En 18 (23%) pacientes se encontró un episodio previo de enfermedad pélvica inflamatoria (EPI) y en 30 (39%) se evidenció dolor y/o anexos engrosados al examen pélvico. CONCLUSIONES: la mayoría de pacientes con infertilidad por salpingitis atendidas en el Hospital Arzobispo Loayza no suele tener hábitos de riesgo para EPI ha tenido sintomatología clínica de infección genital, habiéndose realizado el diagnóstico en forma retrospectiva, a través de las secuelas de infertilidad.
Este estudio pretende contribuir al conocimiento de la incidencia y factores de riesgo de la enfermedad trofoblástica gestacional en nuestro medio. El estudio en forma de caso control retrospectivo en el Hospital Arzobispo Loayza, incluyó 100 mujeres con enfermedad trofoblástica gestacional y como controles, 98 pacientes obstétricas normales atendidas en el hospital entre enero de 1984 y diciembre de 1992. La incidencia de mola hidatidiforme fue de 4,7 por 1000 partos, y la de coriocarcinoma, 0,149 por 1000 partos. La edad promedio fue 28,42 años. No se demostró que el estrato socioeconómico constituya un factor de riesgo. El grado de instrucción secundaria se asoció con un riesgo menor (OR=0,48, IC=0,233-0,98). No se demuestra un efecto protector o de riesgo de la multiparidad en la adquisición de enfermedad trofoblática gestacional. Las mujeres con esta patología tuvieron una edad de primera gestación significativamente menor (19,97 años), en comparación a sus controles obstétricos (21,22 años). El antecedente de embarazo molar previo se asoció con un incremento del riesgo, pero, sin alcanzar significación estadística (OR=6,51, p=0,101).
Objetivo: Evaluar los hallazgos clínicos y hormonales relacionados con la poliquistosis ovárica. Diseño: Estudio retrospectivo. Lugar: Servicio de Ginecología e Infertilidad del Hospital Arzobispo Loayza. Material y Métodos: Se determinó el perfil hormonal de 69 pacientes en edad reproductiva con diagnóstico de ovarios poliquísticos por ecografía transvaginal. Resultados: Se encontró oligoamenorrea en 67,7%, infertilidad en 28,8%, hirsutismo en 23,7% e índice de masa corporal > 25 en 42,4%. El LH estuvo elevado en 47,1 %, con relación LH/FSH > 2 en 24,5% y testosterona elevada en 20,9 %. Se observó que 64,3% de las mujeres obesas presentaban oligoamenorrea, 42,8% hirsutismo, 50% infertilidad y 50% elevación de LH. Conclusión: Se observó una correlación hormonal y clínica con la presencia de obesidad, en pacientes con poliquistosis ovárica.
