RESUMO:Acidente vascular encefálico (AVE) caracterizase por um complexo de sintomas decorrentes de alterações no suporte sanguíneo cerebral, que duram pelo menos 24 horas causam lesões cerebrais. Pode ser classificado em isquêmico (AVEi) e hemorrágico (AVEh), sendo que o mais comum deles é o AVEi, caracterizado por interrupção do fluxo sanguíneo em uma determinada área do encéfalo devido a obstrução arterial ou venosa. A falta de suprimento sanguíneo causa um infarto na área vascularizada pelo vaso obstruído, causando morte neuronal. Existem fatores de risco modificáveis e não modificáveis para o AVE, sendo alguns deles: hipertensão, diabetes, tabagismo, etilismo, sedentarismo, dislipidemias, idade superior aos 55 anos, raça negra, história familiar e AVE prévio. O diagnóstico segue protocolos de acordo com o tempo de início dos sintomas, e basicamente, avalia-se quadro clínico, TC de crânio sem contraste ou ressonância magnética de crânio e exames laboratoriais. O tratamento varia de acordo com sua classificação. No caso apresentado a paciente do sexo feminino, sem comorbidades prévias e fatores de risco, desenvolveu um AVEi mesencefálico com graves sequelas. Não foram investigados alterações em genes da coagulação que pudessem explicar a causa da patologia. O AVEi jovem consiste no acidente vascular encefálico presente em adultos com menos de 45 anos. A patologia necessita, além das investigações padrões, de uma averiguação quanto a etiologia genética. Várias patologias estão associadas ao quadro, como CADASIL, doença de Fraby, coagulopatias, anemia de células falciformes, síndrome de Sneddon, MELAS, síndrome de EhlersDanlos tipo IV, neurofibrimatose tipo 1 e doença de Moya-Moya. Diante disso evidencia-se a necessidade de se conhecer sobre novos estudos e atualizações que descrevam causas não tão conhecidas de AVE.Descritores: Acidente vascular cerebral; Pacientes; Adulto jovem; Adulto; Fatores de risco.ABSTRACT: Encephalic stroke (stroke) is characterized by a complex of symptoms resulting from changes in brain blood support, which last at least 24 hours cause brain damage. A púrpura trombocitopênica idiopática (PTI) é uma doença autoimune relacionada a baixos níveis plaquetários, relativamente frequente, que tem por principal característica clínica a presença de hemorragias espontâneas. Sangramento da pele sob a forma de petéquias, gengivorragia, epistaxe, melena e sangramentos do trato genito-urinário são manifestações comuns, e embora raro, o sangramento intracraniano é uma complicação grave da doença. Diagnóstico diferencial de exclusão é necessário e o tratamento se baseia no uso de corticosteroide, imunoglobulina ou esplenectomia, de acordo com o quadro clínico. É relatado um caso de paciente portador de PTI complicada com hemorragia intracraniana a qual só foi diagnosticada após a realização de ressonância nuclear magnética (RM) de encéfalo, um exame de imagem que apresenta maior sensibilidade a processos infiltrativos quando comparado a tomografia computadorizada (TC). A paciente foi transferida para cuidados hematológicos devido à persistência da baixa contagem plaquetária. Diante do quadro, foi realizada esplenectomia a qual resultou em aumento dos níveis plaquetários, sendo possível a suspensão do corticosteroide.
Introdução: A escolha da posição do paciente na neurocirurgia é um processo que se baseia no sítio de intervenção e na prevenção de complicações, considerando-se a máximasegurança para o paciente. O posicionamento em prona está ligado a complicações transitórias e permanentes pouco discutidas. Método: Pesquisou-se os termos “pronação”,“decúbito ventral”, “prona”, complicações”, “intercorrências” e variações utilizado os operadores booleanos OR e AND, em diferentes estratégias. Resultados: Selecionou-se49 artigos, que relataram complicações hemodinâmicas, visuais, neurológicas, musculares e venosas, de caráter transitório ou permanente. As complicações estão associadastanto a aspectos da cirurgia, como movimentação adotada durante o procedimento, hipovolemia, hemorragia e tempo cirúrgico, quanto a comorbidades dos pacientes, como obesidade, hipertensão arterial e diabetes mellitus. Discussão: A posição prona exige observação dos fatores de risco, para escolha de posicionamentos alternativos, e, caso adotada, a monitorização do paciente, a fim de reduzir complicações. Conclusão: Nenhum posicionamento está destituído de riscos, sendo necessário conhecer as complicações a fim de se melhor avaliar a escolha do posicionamento. Discussion: The prone position requires patient monitoring to reduce complications. Alternative positions must be considered if there are patient risk factors. A embolia cerebral gordurosa é uma patologia decorrente da liberação de glóbulos de gordura na circulação sanguínea provenientes, principalmente, de fraturas de ossos longos. A presença de êmbolos significativos de gordura no sangue resulta oclusão vascular e manifestações clínicas dependentes da localização da obstrução. O caso relatado apresenta um paciente jovem, vítima de acidente automobilístico com fratura de acetábulo e diáfise femoral direita, que evoluiu com rebaixamento do sensório. Diante do quadro clínico, foi solicitada ressonância magnética, que apresentou resultado compatível com o diagnóstico de embolia cerebral gordurosa. Realizado os devidos cuidados para tratamento da embolia, o paciente apresentou prognóstico favorável.
Objetivo: Relatar o caso de um paciente adulto, sexo masculino, portador de diabetes mellitus e acometido por hanseníase. Detalhamento do caso: No caso relatado o paciente é diabético em tratamento irregular e sem acompanhamento médico periódico, o que propiciou o desenvolvimento de um quadro característico de consequente complicação da doença, o “pé diabético”, sobreposto às sequelas neurológicas da hanseníase. A hanseníase é causada pelo bacilo M. leprae, que possui tropismo por células da pele e nervos periféricos, causando lesões cutâneas indolores. Considerações finais: Até o presente momento da elaboração do artigo, o paciente segue em tratamento poliquimioterápico para hanseníase, controle glicêmico rigoroso e reabilitação de sequelas.
