Recently, a new coronavirus (SARS-CoV-2) was discovered in China. Due to its high level of contagion, it has already reached most countries, quickly becoming a pandemic. Although the most common symptoms are related to breathing problems, SARS-CoV-2 infections also affect the gastrointestinal tract culminating in inflammation and diarrhea. However, the mechanisms related to these enteric manifestations are still not well understood. Evidence shows that the SARS-CoV-2 binds to the angiotensin-converting enzyme receptor 2 (ACE2) in host cells as a viral invasion mechanism and can infect the lungs and the gut. Other viruses have already been linked to intestinal symptoms through binding to ACE2. In turn, this medical hypothesis article conjectures that the ACE2 downregulation caused by the SARS-CoV-2 internalization could lead to decreased activation of the mechanistic target of mTOR with increased autophagy and lead to intestinal dysbiosis, resulting in diarrhea. Besides that, dysbiosis can directly affect the respiratory system through the lungs. Although there are clues to other viruses that modulate the ACE2/gut/lungs axis, including the participation of autophagy and dysbiosis in the development of gastrointestinal symptoms, there is still no evidence of the ACE2/mTOR/autophagy pathway in SARS-CoV-2 infections. Thus, we propose that the new coronavirus causes a change in the intestinal microbiota, which culminates in a diarrheal process through the ACE2/mTOR/autophagy pathway into enterocytes. Our assumption is supported by premises that unregulated intestinal microbiota increases the susceptibility to other diseases and extra-intestinal manifestations, which can even cause remote damage in lungs. These putative connections lead us to suggest and encourage future studies aiming at assessing the aforementioned hypothesis and regulating dysbiosis caused by SARS-CoV-2 infection, in order to confirm the decrease in lung injuries and the improvement in the prognosis of the disease.
RESUMOA epidermólise bolhosa é uma doença hereditária ocasionada por mutações em várias proteínas estruturais da pele, representando um quadro clínico grave, cujos defeitos adquiridos ou congênitos da adesão intraepidérmica ou dermoepidérmica levam à formação de bolhas (vesículas) na pele e nas mucosas, podendo ser espontâneas ou provocadas por trauma mínimo. Objetivou-se neste estudo identificar, por meio da literatura científica, temas importantes relacionados à assistência de enfermagem frente ao paciente com epidermólise bolhosa. Tratou-se de uma revisão integrativa de literatura com pesquisa realizada nas bases de dados MEDLINE (via PubMed) e PubMed Central Canada (PMC Canada). Oito artigos científicos compuseram a amostra deste estudo. Foi evidenciado um ambiente sofrível tanto para o paciente como para o enfermeiro durante o tratamento, bem como foram encontradas dificuldades pelo paciente em dispor de curativos e coberturas eficientes para as feridas. O impacto da epidermólise no ambiente psicossocial também foi destacado, com enfoque para o tempo gasto na troca de curativos. Concluiu-se que a enfermagem desempenha um papel relevante no tratamento do paciente com epidermólise bolhosa, haja vista as condições que tal sujeito enfrenta e o potencial do enfermeiro para efetuar cuidados paliativos. It represents a serious medical condition. The acquired or congenital defects of the intra-epidermal or dermal-epidermal adhesion lead to the formation of blisters (vesicles) on the skin and on mucous membranes. In fact, this process may be spontaneous or provoked by minimal trauma. The objective of this study was to identify, using scientific evidence, the major issues related to nursing care towards the patient with epidermolysis bullosa. This study was an integrative literature review that was conducted on MEDLINE (via PubMed) and PubMed Central Canada (PMC Canada). Eight scientific articles were selected to be the sample of this study. A painful environment for both the patient and the nurse during the treatment was evidenced. In addition, some difficulties were seen when the patients were trying to find effective covers and dressings for wounds. In addition, the impact of epidermolysis on the psychosocial environment was also highlighted with emphasis on the time spent on dressing changes. Thus, considering the conditions that the patient faces during the treatment and the potential of nurses to implement a palliative care, nursing professionals play an important role in the treatment of patients with epidermolysis bullosa. Objective: to report experiences that nursing students had in an Academic League of Stomatherapy during one year. Methodology: It is a report of activities performed by nursing students who were members of the League at the Universidade Estadual do Piauí. Results: Durante el período de un año, the students improved their knowledge on the three areas of Stomatherapy (wound, ostomy, and incontinence). This improvement was a result of activities that embraced teaching, research and extension. During the lectures, students had opportunities to ask questions and talk about their experiences. The members were also able to apply all the knowledge acquired in clinical practice, improving their skills and contributing to the improvement of care provided to the population. In addition, some members of the league participated in scientific events as exhibitors of scientific production. Conclusion: The league has been an effective tool capable of promoting autonomy on the search for knowledge required for the formation of professionals with knowledge, abilities, and attitudes to practice the Stomatherapy. Este artigo compara aspectos do Sistema Único de Saúde (SUS) brasileiro e do Serviço Nacional de Saúde (SNS) português.Mostra-se interessante comparar esses dois países por conta de sua proximidade cultural, histórica e linguística, além deseguirem o modelo de sistema de saúde regido pelo governo. O SUS resultou de uma construção histórica de vários setores nasociedade brasileira e baseia-se nos princípios de universalidade, integralidade, equidade, descentralização e participação social.Assim, no Brasil, a saúde é um direito do cidadão e um dever do Estado garantido pela Constituição Federal de 1988 (CF/1988).Em Portugal, a saúde também é um direito do cidadão garantido pela Constituição. Como ocorreu no Brasil, estabeleceu-se que asaúde deveria ser gratuita, porém, em 1989 o texto constitucional foi alterado e a saúde gratuita passou a ser tendencialmentegratuita. Portugal adotou o caminho da não gratuidade; no caso brasileiro, a sociedade civil optou pelo ideal do SUS (uma saúdeigualitária, gratuita e universal), mas ainda é preciso que o governo aplique mais recursos no setor para que ele se efetive naprática.
