La eritrodermia consiste en eritema y descamación generalizada que involucra el 90% o más de la superficie cutánea. Es una patología poco común, cuya incidencia global y nacional se desconocen con exactitud, pero predomina en hombres de 42 a 61 años. Su fisiopatología involucra una interacción compleja de citoquinas que generan un reclutamiento masivo de células inflamatorias a la piel, aumento de la actividad mitótica y un recambio epidérmico elevado con la consecuente exfoliación de los estratos superficiales. Su etiología es amplia, pero la mayoría de las veces existe una generalización de una dermatosis preexistente, principalmente dermatitis atópica o psoriasis; también puede verse como reacción de hipersensibilidad a fármacos, en linfoma T cutáneo u otras malignidades o ser idiopática. El diagnóstico de esta enfermedad subyacente requiere una historia clínica detallada, un examen físico minucioso, exámenes de laboratorio, biopsias y otras pruebas dirigidas según la sospecha. El manejo de la eritrodermia debe dirigirse al control de la enfermedad cutánea, a su causa subyacente y a las complicaciones sistémicas.
La siguiente revisión bibliográfica brinda un acercamiento general a la patología de dermatomiositis juvenil desde una visión dermatológica y reumatológica. La dermatomiositis juvenil (DMJ) es una enfermedad autoinmune sistémica caracterizada por miositis de músculos proximales junto con lesiones cutáneas características. Es una patología con aparición temprana en la infancia y predilección por el sexo femenino. Su aparición se ha relacionado con factores genéticos y ambientales específicos. A pesar de ser la piel y el tejido muscular los más afectados, también se puede presentar afectación pulmonar, cardiovascular y gastrointestinal. Actualmente, el diagnóstico se realiza por medio de la clínica, la resonancia magnética (RMN) y la búsqueda de autoanticuerpos específicos. La calcinosis representa una de las complicaciones más frecuentes y graves de la DMJ, con alta morbimortalidad. El tratamiento general de la DMJ se centra en el inicio temprano de corticoesteroides a altas dosis; así como la integración de fármacos modificadores de la enfermedad en casos más severos. El pronóstico actual de la enfermedad representa una disminución importante en la mortalidad desde el inicio de la era de los corticoesteroides.
