RESUMODesde o surgimento das primeiras infecções por coronavírus (SARS-CoV e MERS) as gestações foram associadas a riscos de abortamentos, partos prematuros, óbitos fetais intrauterinos e mortes maternas. Com o surgimento do novo coronavírus em 2020, as repercussões do COVID-19, têm sido estudadas e as gestantes enquadradas como grupo de risco. O estado hiper-inflamatório originado pela COVID-19 pode estar associado com hipóxia placentária, levando a um estado antiangiogênico, resultando os mesmos sinais clínicos da pré-eclâmpsia, como hipertensão, proteinúria, trombocitopenia e enzimas hepáticas elevadas. Relato de caso: Gestante de 29 anos, G2P1CA0, apresentando hipertensão arterial crônica (HAC) e diabetes gestacional (DMG) diagnosticada no primeiro trimestre. Encontrava-se em uso de polivitamínicos, metildopa 750 mg/dia e AAS 100 mg/dia profilático para PE, desde o primeiro trimestre. Com 28 semanas, positivou para COVID-19 com doença sintomática e no 10º dia da doença foram prescritos amoxicilina e dexametasona para uso ambulatorial. No 17º dia apresentou exames de controle que demonstraram elevação de transaminases e D-dímero, sem alteração pressórica. Foi iniciada a enoxaparina 40mg no pronto socorro e controle em 48h. No 19º dia os exames apresentados foram D-dímero elevado (1.390 mg/dl) e plaquetas em queda (100.000/mm3). A mulher foi internada com suspeita de síndrome HELLP, apesar de paradoxalmente, a pressão arterial mensurada ser de 120 x 80 mmHg, Os níveis plaquetários permaneceram em queda até o limite de 30.000/mm3), enquanto não houve elevação de pressão arterial, sendo portanto concluído diagnóstico de síndrome HELLP-like. Foi indicada a interrupção da gestação com 32 semanas e 2 dias de gestação, após 25 dias de curso da doença e transfusão de 10 bolsas de plaquetas. Após o parto, foi entubada e encaminhada para UTI, onde após 48 horas, assintomática e com parâmetros todos normalizados, foi extubada e recebeu alta após 72h. Conclusão: Os autores discutem o diagnóstico diferencial das síndromes HELLP clássica e HELLP-like associada à COVID-19, bem como as possíveis diferenças de mecanismos para o seu desenvolvimento.
