Ziel: Die Gruppe der Klienten mit der Hauptdiagnose (HD) Cannabis wird über ein Jahrzehnt (2000 – 2009) beschrieben und mit den Klienten mit anderen HD verglichen. Dabei soll untersucht werden, ob sich soziodemographische und epidemiologische Parameter der Klienten verändert haben. Methodik: Es handelt sich um eine deskriptive Reanalyse der soziodemographischen Daten von Klienten aus ambulanten Suchtberatungs-/-behandlungseinrichtungen in Deutschland, die im Rahmen der Deutschen Suchthilfestatistik (DSHS) dokumentiert wurden. Die Datenerhebung orientiert sich am neuen Deutschen Kerndatensatz zur Dokumentation in der Suchthilfe. Die Zahl der betrachteten Fälle pro Jahr lag zwischen 51.840 aus 368 Einrichtungen (2001) und 152.154 aus 779 Einrichtungen (2009). Ergebnisse: Der Anteil der Klienten mit einer primären Cannabisproblematik hat sich von 2000 (6,3 %) bis 2009 (13,5 %) deutlich vergrößert, 2009 beträgt die Gesamtzahl 20.599 Fälle. Es zeigen sich folgende Parameter zur aktuellen Lebenssituation: Fast zwei Drittel der Klienten mit HD Cannabis sind alleinstehend. Der Anteil der Cannabisklienten in prekärer Wohnsituation hat sich von 2002 (0,5 %) nach 2003 (1,1 %) verdoppelt. Seitdem sind die Werte von leichten Schwankungen abgesehen stabil. Fast ein Sechstel der Cannabisklienten hat zwischen 2007 und 2009 die Schule ohne Schulabschluss oder mit Sonderschulabschluss beendet. Die Arbeitslosigkeit hat sich unter den Klienten mit HD Cannabis zwischen 2000 (18,1 %) und 2007 (42,0 %) mehr als verdoppelt und ist seitdem nahezu unverändert hoch (2009: 43,6 %). Schlussfolgerungen: Der seit 2000 deutlich gestiegene Anteil der Klienten mit HD Cannabis an allen Klienten steht möglicherweise in Beziehung mit der gestiegenen Zahl spezieller Programme zur Behandlung von Cannabis bezogenen Störungen. Der hohe Anteil an alleinstehenden Cannabisklienten ist vor allem auf das unterdurchschnittliche Alter im Vergleich zu den anderen Klienten zurückzuführen. Auch wenn sich der Anteil von Klienten in prekärer Wohnsituation seit 2000 mehr als verdoppelt hat (von ca. 0,5 % auf mehr als 1 %), befanden sich 2009 doch mindestens 85 % in stabilen Wohnverhältnissen. Die aktuelle berufliche Situation der Klienten ist besorgniserregend. Daher scheinen Maßnahmen zur Bekämpfung der hohen Arbeitslosigkeit, insbesondere unter den jüngeren Klienten mit HD Cannabis, angezeigt. Sinnvoll wären Frühinterventionen, die speziell auf das Setting „Schule, Ausbildungsplatz, Berufsberatung“ zugeschnitten sind. Dazu sind intensivere Anstrengungen und Kooperationen von Suchthilfeeinrichtungen, Sozialämtern, Jugendhilfe und Arbeitsagenturen nötig.
This study aimed to investigate prevalence and correlates of alcohol consumption frequency in a sample of Swiss conscripts (n=25,611) in order to identify factors that predispose for frequent consumption. A self-report of drinking frequencies, as well as socio-demographic and psychosocial variables, was collected at psychiatric baseline screening. Based on univariate analyses, relevant variables were included in a multivariate multinomial logistic regression model. Six percent were abstainers, 15% reported rarely drinking, 53% occasional drinking, 24% regular drinking and 2% daily drinking. Except for substance use, most associations followed a "J"-shaped curve across the categories of alcohol frequency. Abstinence and frequent drinking can be perceived as deviations from the social norm. Both behaviors are associated with more psychosocial stressors and might be therefore special targets for further studies and new prevention programs.
Ziel: Zum einen werden Veränderungen des Erwerbsstatus der Klienten in ambulanten und stationären Suchthilfeeinrichtungen von 2007 bis 2011 beschrieben sowie ausgewählte Hauptdiagnosen (HD) miteinander verglichen. Zum anderen erfolgt für das Datenjahr 2009 eine Gegenüberstellung von soziodemographischen und epidemiologischen Parametern von Klienten, die arbeitslos nach SGB II oder SGB III waren (ALO), mit Klienten, die erwerbstätig waren. Methodik: Es handelt sich um eine Reanalyse der Daten von Klienten aus der Deutschen Suchthilfestatistik (DSHS). Im ambulanten Bereich lag die Zahl der betrachteten Fälle mit dokumentierter HD pro Jahr zwischen 107.041 aus 720 Einrichtungen (2007) und 155.276 aus 779 Einrichtungen (2009); im stationären Setting zwischen 24.586 aus 147 Einrichtungen (2007) und 39.329 aus 189 Einrichtungen (2010). Eine Sonderauswertung des Jahres 2009, die ausschließlich Klienten beinhaltet, die in den letzten sechs Monaten vor Betreuungsbeginn arbeitslos waren, enthält 55.479 Fälle aus 779 ambulanten sowie 13.792 Fälle aus 157 stationären Einrichtungen. Ergebnisse: Zwischen 2007 und 2011 ist der Anteil der arbeitslosen Klienten im ambulanten Setting um ca. einen Prozentpunkt gesunken (2011: 39,1 %) und im stationären Bereich um ca. einen Prozentpunkt gestiegen (2011: 48,5 %). Am höchsten liegt der Anteil der Arbeitslosen bei Klienten mit HD Opioide (ambulant: 59,9 %; stationär: 64,7 %). In allen untersuchten HD übersteigt der Anteil der SGB-II-Arbeitslosen diejenigen der SGB-III-Arbeitslosen mit mindestens 80 % zu 20 % bei weitem. Die stärkste Veränderung lässt sich bei Klienten mit HD Stimulanzien in stationären Suchthilfeeinrichtungen beobachten (+13,7 Prozentpunkte). In den DSHS-Daten aus dem Jahr 2009 waren die Anteile arbeitsloser Klienten bei folgenden Variablen höher als die der erwerbstätigen Klienten: alleinlebende Klienten und Klienten in prekärer Wohnsituation (ohne Wohnung, Notunterkünfte), Klienten ohne abgeschlossene Hochschul- oder Berufsausbildung, mit problematischen Schulden, sowie wiederbehandelte Klienten. Der Anteil der regulären Beendigungen dagegen liegt bei arbeitslosen Klienten 13,3 Prozentpunkte (ambulant) und 10,9 Prozentpunkte (stationär) niedriger als bei erwerbstätigen Klienten. Schlussfolgerungen: Die vorliegenden Daten zeigen, dass soziale Ressourcen unter den arbeitslosen Klienten stärker eingeschränkt sind als bei der erwerbstätigen Suchthilfeklientel. Die Indikatoren für Rückfallquoten deuten auf eine schlechtere Prognose für diese Klienten hin. Daher sollten zum einen Angebote zur (Re‐) Integration in den Arbeitsmarkt im Sinne von Ausbildungen und suchtpräventiven Maßnahmen beibehalten und ggf. verfeinert werden. Zum anderen scheinen für Klienten ohne berufliche Perspektive spezielle rückfallpräventive Maßnahmen angezeigt, die ohne den Faktor berufliche (Re‐) Integration auskommen. Wertvoll könnten sich in diesem Zusammenhang noch intensivere Kooperationen zwischen Suchthilfeeinrichtungen, (Berufs‐) Schulen, Arbeitsagenturen, Jobcenter, Schuldnerberatungen sowie Jugend- und Sozialämtern erweisen.
Ziele: Die Ergebnisse der Fachtagung »Bewährtes übertragen – Modelle der Cannabisbehandlung für die Praxis«, d. h. insbesondere hilfreiche und problematische Aspekte beim Übergang von Modellprojekten zur Prävention und zur Behandlung von cannabisbezogenen Störungen in die Regelversorgung sollen dargestellt werden. </p><p> Ergebnisse: Als hilfreich für den Transfer in die Regelversorgung erwiesen sich die Kompatibilität neuer Programme mit dem bestehenden Angebot, die Strukturiertheit sowie Transferseminare für Fachkräfte und Plattformen für den Erfahrungsaustausch. Als Hauptschwierigkeiten wurden das aufwändige Kooperationsmanagement mit zum Teil ungewohnten Partnern (z. B. der Polizei) genannt und die Berücksichtigung der speziellen regionalen Bedingungen (u. a. bestehende Programme, Finanzierung). </p><p> Fazit und Ausblick: Die vorgestellten Programme eröffnen neue Zugangswege zu Patienten, die bisher keine Suchtberatungsstellen aufsuchten und können somit bei spezifischen Zielgruppen eine Lücke im Suchthilfesystem schließen. Die vorgestellten Angebote sind überwiegend Kooperationsprojekte, die zur Erreichung der gemeinsamen Ziele ein Zusammenwirken der Akteure auf unterschiedlichen Ebenen sowie klar definierte Aufgaben und Standards erfordern – auch unter Akzeptanz unterschiedlicher Einstellungen.
