İnfraklavikular brakial pleksus bloğu el, bilek ve dirsek operasyonları için uygun bir rejyonal anestezi tekniği olup, sinir stimülatörü eşliğinde gerçekleştirilmektedir. Son yıllarda ultrasonografinin periferik sinir bloklarında kullanıma girmesiyle birlikte, uygulamalar değişmeye başlamıştır. Çalışmada amacımız, infraklavikular brakial pleksus bloğunda, tek başına ultrasonografi kullanımı ile ultrasonografi ile birlikte sinir stimülasyonu kullanımını, uygulama süresi, başarılı blok oranı, duyusal blok kalitesi açısından karşılaştırmaktır. Gereç ve Yöntem: Çalışma, elektif koşullarda el, bilek ve dirsek operasyonu geçirecek 40 ASA I-II hastada yapılmıştır. Hastalar US (Ultrasonografi) ve USSS (Ultrasonografi + Nörostimulatör) şeklinde iki gruba ayrılmıştır. US ve USSS eşliğinde hastalara, 100 mg bupivakain, 200 mg prilokain 40 ml olacak şekilde SF ile sulandırılarak infraklavikular bölgeye uygulanmıştır. Uygulama süresi kronometre ile takip edilerek kayıt altına alınmıştır. Uygulamadan sonra 30 dk süresince 5 dk'da bir elin motor ve duysal bloğu değerlendirilmiştir. Bulgular: Uygulama süresi Grup US'de 6.68±0.75 dk, Grup USSS'de 6.9±1.02 dk olarak bulunmuştur. Uygulama süresi açısından gruplar arasında istatistiksel anlamlı fark bulunmamıştır (p>0.05, p=0.62). Grup US'de 20. dk'da 16/20 hastada tam blok ortaya çıkmışken, Grup USSS'de 20. dk'da 14/20 hastada tam blok ortaya çıkmıştır, fark istatistiksel olarak anlamsızdır. Grup US'de 2/20 hastada cerrahi işlem sırasında lokal anestezik infiltrasyonu, Grup USSS'de 1/20 hastada lokal anestezik infiltrasyonu gerekti. Sonuç: İnfraklavikular brakial pleksus blokajında ultrasonografi ile birlikte sinir stimülasyonu kullanımının, sadece ultrasonografi kullanımı ile karşılaştırıldığında blok başarısına ek bir katkısı olmadığı görüldü. Sinir stimülasyonuna bağlı olarak oluşan rahatsızlık hissi ve ağrı göz önüne alındığında, tek başına ultrasonografi kullanımının, sinir stimülasyonuna tercih edilebileceği kanısına varılmıştır. Bununla birlikte istatistiksel olarak anlamlı sonuçlar elde etmek ve kesin sonuçlara ulaşabilmek için, bu konuda çok sayıda çalışmalara ihtiyaç vardır.
Objective: Postoperative pain seen in the first 48 hours after surgery, is an important problem and various methods are used in postoperative pain control. This study aims to investigate the effects of general anesthesia and combined general and regional anesthesia use on postoperative pain in arthroscopic rotator cuff repair operations.Materials and Methods: Twenty-eight female patients who were planned for arthroscopic rotator cuff surgery were included. The patients were randomly divided into two groups. Scalene block was applied to patients in Group 1 (n=14) in addition to general anesthesia while the patients in Group 2 (n=14) received only general anesthesia. Pain levels of the groups at the sixth, 12th, and 24th hours were evaluated with the Visual Analogue Scale (VAS). The findings were analyzed with Student's-t test and Mann-Whitney U test using SPSS 18.0 statistical program. Results:The average age of the Group 1 was 57 (45 ± 68); and the average age of the patients in Group 2 was 57.93 (47 ± 69). The postoperative pain levels of Group 1 was 1.57 (± 1.74) at the 6th hour; 2.57 (± 1.28) at the 12th hour, 2.71 (± 1.20) at the 24th hour; in Group 2 the 6th hour pain level was 8.00 (± 1.75); 12th hour level was 7.00 (± 1.52); and it was 6.00 (± 1.57) at the 24th hour. There was a statistically significant difference in all measurements in terms of VAS scores (p <0.001). Conclusion:It is concluded that implementation of scalene block in addition to general anesthesia, increases the success of the operation in arthroscopic rotator cuff repair surgery.
Background: To give a good planned anesthesia to a morbidly obese patient, it is better to have knowledge about morbid obesity's interaction and impact on drug dosages. For safe drug dosing, not only increased body weight and changed body composition but also physiological changes and comorbid conditions have to be considered.
