Introdução: A doença periodontal é uma patologia desencadeada pelo acúmulo de biofilme patogênico a superfície dentária e a mucosa oral. Considera-se uma das doenças crônicas mais comuns em todo o mundo, e uma das principais causas de perdas dentárias. Recentemente, o uso de probióticos tem sido defendido como um benéfico adjuvante para o tratamento não cirúrgico e manejo da periodontite, proporcionando o equilíbrio ecológico da microbiota oral. Materiais e métodos: Para este estudo, foi realizado uma revisão integrativa na literatura, da base de dados PubMed, utilizando os descritores “Probiotics and Periodontics and Therapeutics and Gengivitis and Periodontitis”, de estudos clínicos realizados entre 2017 à 2022. Ao final da busca, obteve-se um total de 42 artigos e após a análise criteriosa dos títulos e resumos, foram selecionados 15 artigos que obedecem aos critérios de elegibilidade. Resultados e discussão: A natureza disbiótica da doença periodontal, viabilizou o surgimento de novas estratégias preventivas e terapêuticas. Clinicamente os resultados com probióticos são favoráveis, como, estimular e modular a imunidade da mucosa, aumentar a atividade de citocinas anti-inflamatórias, reduzir a produção de citocinas pró-inflamatórias como IL-8, IL-1β e TNF-α, além de reduzir o número de sítios com graves inflamações em pacientes com gengivite. Algumas cepas probióticas podem formar biofilme nos tecidos duros e moles, impedindo a adesão de bactérias patogênicas, também são capazes de alterar a composição proteica da película salivar adquirida. Os tratamentos com probióticos mostraram melhores resultados que o desbridamento sozinho, além de não causa efeitos adversos. Conclusão: O uso de probióticos se mostra uma medida eficaz e de fácil implementação para restabelecimento da saúde periodontal, fornecendo nova opção terapêutica no controle clínico das inflamações gengivais, além de representar um valioso tratamento adjuvante não invasivo na redução dos parâmetros inflamatórios periodontais.
INTRODUÇÃO: A periodontite é caracterizada como doença inflamatória desencadeada pelo acúmulo de biofilme supra e subgengival, levando a destruição óssea e do ligamento periodontal. Osteonecrose mandibular (ONJ) é uma patologia na qual é definida como área de osso exposto na região maxilofacial, ou região óssea que possa ser sondado através de fístula intraoral ou extraoral, por mais de oito semanas, sem históricos de radioterapia ou metástases, que fazem uso de potentes antirreabsortivos. Portanto, objetiva-se avaliar a relação entre periodontite e osteonecrose mandibular associada a medicamentos. MÉTODOS: Um levantamento bibliográfico foi realizado de 2017 a 2022 nas bases de dados Pubmed e Web of Science, utilizando-se os descritores “Gingivitis”, “Periodontal disease”, “Periodontitis” e “Osteonecrosis of the jaw”, utilizando os operadores booleanos OR e AND. Obteve-se um total de 155 e 122 artigos, respectivamente. Após análise rigorosa dos títulos e resumos, foram selecionados 10 artigos (8 do Pubmed e 2 do Web of Science) seguindo os critérios elegíveis. RESULTADOS E DISCUSSÃO: Um estudo em ratos com periodontite tratados com ácido zoledrônico tiveram redução significativa na prevalência de ONJ ao tratamento periodontal prévio. Em um estudo clínico esse fármaco mostrou-se eficaz na prevenção de periodontite em pacientes com boa higiene oral. No entanto, um caso clínico demonstrou ONJ após terapia periodontal não cirúrgica. Estudos em camundongos, apresentaram ocorrência de ONJ, combinado a zolendronato, sem extrações dentárias. Em contrapartida, outro estudo evidenciou que condições inflamatórias pré-existentes, associadas a exodontias, desempenham papel importante no desenvolvimento de ONJ associada a drogas. Ainda foi relatado que zolendronato inibe a reabsorção óssea periodontal, mas não interfere na reação inflamatória. CONCLUSÃO: Em suma, os estudos demonstram que o processo inflamatório da periodontite pode influenciar no desenvolvimento de ONJ, assim como, as drogas antirrebsortivas, como o aledronato, podem influenciar beneficamente na periodontite, no entanto, o tratamento periodontal prévio é indispensável.
