Introduction With coronary artery bypass grafting, patients are subjected to additional risk caused by both surgical treatment itself and pathophysiological changes in homeostasis, provoked by the action of anesthetics and cardiopulmonary bypass. Material and methods The study involved 60 patients, who had been subjected to coronary artery bypass grafting with cardiopulmonary bypass. All patients were divided into two groups: group I (30 patients) – low-opioid scheme of anesthesia and group II (30 patients) – standard scheme of anesthetic management. Blood interleukin-6 (IL-6) was identified before and after cardiopulmonary bypass using an ELISA test. Results Having compared IL-6 values between study groups after completion of cardiopulmonary bypass, it was established that IL-6 levels were 27.51% ( p = 0.001) lower in patients of group I compared with the results of patients in group II. Patients in the first group had a significantly shorter time of mechanical ventilation compared to group II (2.1 ±0.7 hours vs. 3.9 ±0.9 hours, p = 0.021). Low cardiac output syndrome was significantly less frequently reported in patients of group I (10.0% vs. 33.3%, p = 0.028). In addition, patients in group I had a significantly shorter time of intensive care unit (ICU) stay (2.5 ±0.7 days vs. 3.5 ±1.0 days, p = 0.044). Conclusions Application of multimodal low-opioid anesthesia was associated with significantly lower IL-6 at the end of surgery, shorter mechanical ventilation duration, less frequent low cardiac output syndrome and need for catecholamines, and shorter ICU stays.
Introduction Infusion therapy is an integral part of intensive care patients with severe traumatic brain injury, addressing such important issues as to replenish the deficit blood volume in the first day of treatment, thereby providing hemodynamic stability and as a result, adequate perfusion of the damaged brain that is essential to avoid secondary ischemic damage. The most widely used at present as a basic solution for volemic support in patients with severe TBI, sodium chloride 0.9%. The main advantages of crystalloid solutions-low reactogenicity, no effect on renal function and immune system, and no significant effect on the hemostatic system. The main disadvantage is their rapid crystalloid redistribution of the vascular bed in the intercellular space, 75-80% of the drug after one or two hours after the infusion is found in the interstices. Significantly affect the degree of edema syndrome in these patients possibly by influencing the osmotic pressure, as well as through volemic support of solutions that are as close in their ionic composition to blood plasma. Any infusion liquid which contains a physiological buffer HCO3-, will always create dilution acidosis, such as infusion solution reduces the concentration of HCO3-throughout the extracellular space, while the partial pressure of CO2 (buffer acid) remains constant. When the extracellular fluid volume expands with a solution without bicarbonate, there is hyperhydration. Materials and Methods The study involved 60 patients with severe TBI, aged 18 to 65 years, with the level of consciousness 5-11 points for the Glasgow Coma Scale at admission and 12 hours after the initial resussitation and severity on a scale APACHE II-21 ± 2,7 points, which were divided into two groups: control and research. Victims in the control group performed volemic support solution NaCl 0,9%, by the conventional method. The group conducted research volemic support a balanced crystalloid solution. In both groups monitoring indicators increasing the thickness of solid tissue in the frontal area, the rear foot and the circumference over radiocarpal joint. Results As shown by our study in both groups, patients with severe traumatic brain injury occurred edema syndrome manifested the thickness of solid tissue of the periosteum to the line skin measured sonographic method in constant point on the rear foot and in the frontal area and the increase dynamics circumference of the radiocarpal joint. However, the group suffered from severe traumatic brain injury where volemic support held sodium chloride 0.9% growth increase in the thickness of the array of tissues in the studied areas significantly higher than in the group, which held volemic support a balanced crystalloid solution. In the control group edema syndrome observed during all ten days of treatment, whereas in the study group observed regression edema syndrome since 8 days of treatment. Стаття присвячена впливу різних способів інфузійної терапії та адреноміметичної корекції на функціональні показники кишечника у постраждалих з розвитком синдрому поліорганної недостатності (СПОН) внаслідок тяжкої політравми. При вивченні показників функціонального стану кишечника в умовах використання протоколу модифікованої інфузійної терапії з урахуванням стадії травматичної хвороби виявилося, що використання цієї стратегії супровождується стабільними рівнями внутричеревного та абдомінального перфузійного тиску, які були у межах референтних значень протягом усього періоду дослідження. При співставленні з даними групи контролю в період розвитку ранньої та пізньої СПОН встановлено, що в групі травмованих із використанням протоколу модифікованої інфузійної терапії з урахуванням стадії травматичної хвороби рівні абдомінального перфузійного тиску виявилися на 8,41–19,35 %, об’єми ентерального харчування — на 26,92–41,50 % вище, в той час як рівень внутричеревного тиску — на 22,39–22,46 % нижче порівняно з даними групи контроля. Використання протоколу модифікованої інфузійної терапії та оптимізованої адреноміметичної корекції з урахуванням стадії травматичної хвороби в умовах ранньої та пізньої СПОН не супроводжувалося розвитком внутрішньочеревної гіпертензії, більш того, на фоні впроваджуваної стратегії відмічалося вірогідне збільшення абдомінального перфузійного тиску, при цьому об’єми ентерального харчування зростали, що свідчило про адекватну перфузію гастроінтестинального тракту на фоні інфузії норадреналіну.
посвящена изучению влияния модифицированной инфузионной терапии и адреномиметической коррекции с учетом стадии травматической болезни на показатели транспорта, потребления кислорода, периферического кровообращения у пострадавших с тяжелой политравмой. Использование протокола модифицированной инфузионной терапии с учетом стадии травматической болезни при развитии синдрома полиорганной недостаточности (СПОН) позволяет поддерживать уровень транспорта кислорода свыше 550 мл/мин/м 2 ; его потребление более 150 мл/мин/м 2 ; восстановить уровень артериального рН в пределах референтных значений, нормализовать показатели лактата в артериальной крови, достичь референтных значений градиентов PvCO 2-PaCO 2 , T perif-T atm , T centr-T perif , T skin diff , поддерживать темп диуреза свыше 1 мл/кг/час. Использование протокола модифицированной инфузионной терапии и оптимизированной адреномиметической коррекции с учетом стадии травматической болезни в условиях ранней и поздней СПОН позволяет достичь транспорта кислорода свыше 600 мл/мин/м 2 , уровня сатурации крови в верхней полой вене свыше 70 % наряду со снижением показателей экстракции кислорода, что сочеталось с понижением уровня лактата в артериальной крови и ростом ВЕ. Полученные результаты свидетельствуют о том, что применение протокола модифицированной инфузионной терапии и оптимизированной адреномиметической коррекции с учетом стадии травматической болезни в условиях СПОН у тяжелотравмированных позволяет улучшить периферический кровоток, о чем свидетельствует возрастание градиента T perif-T atm и темпа диуреза, а также снижение градиентов PvCO 2-PaCO 2 ; T centr-T perif ; T skin diff .
