Цель. Определение факторов, способствующих развитию острого холангита (ОХ) или являющихся его признаками при доброкачественной обструкции внепеченочных желчных путей (ДОВЖП). Материалы и методы. Ретроспективно проанализированы 144 наблюдения ДОВЖП, ОХ отмечен в 17 наблюдениях. Проведено сравнение встречаемости различных факторов и признаков у больных с ОХ и без ОХ. Результаты. Различия выявлены (p < 0,05) между такими показателями: гипертермия, триада Шарко, лейкоцитоз, содержание палочкоядерных нейтрофилов, уровень общего билирубина, активность амилазы крови, толщина стенки желчного пузыря (ЖП), наличие отключенного ЖП, фиксированного камня большого сосочка двенадцатиперстной кишки (БСДПК), размер БСДПК. Различий не обнаружено (p > 0,05) между следующими показателями: пол, возраст, индекс массы тела, наличие абдоминального болевого синдрома, холецистэктомия в анамнезе, объем ЖП, причина обструкции, расширение гепатикохоледоха, эндоскопическая папиллосфинктеротомия в анамнезе, наличие околососочкового дивертикула и острого билиарного панкреатита. Выводы. Факторы риска развития ОХ: уровень общего билирубина 70 мкмоль/л и выше, утолщение стенки ЖП до 4 мм и более, отключенный ЖП, фиксированный камень БСДПК, размер БСДПК 15 мм и более. Проявления ОХ: гипертермия, наличие триады Шарко, лейкоцитоз 9 × 109 в 1 л и выше, увеличение содержания палочкоядерных нейтрофилов до 7% и выше, гиперамилаземия.
Зміни печінки при повній обструкції позапечінкових жовчних шляхів (ПОПЖШ) з урахуванням морфометричних показників можуть відображати наступаючу декомпенсацію функції печінки і служити об 'єктивними критеріями прогнозу перебігу захворювання. Мета: вивчення морфологічних змін гепатоцитів при експериментальній ПОПЖШ з використанням макро-та мікроморфометріі. Матеріали та методи. На 41 щурах ПОПЖШ відтворювали шляхом перев'язки і перетину загальної жовчної протоки. Тварин виводили з експерименту на 1, 3, 7, 14, 21, 28 і 35 добу. Контроль -10 неоперованих щурів. Досліджували вміст загального білірубіну крові (ЗБК), об'єм печінки (ОП), площа гепатоцитів (ПГ), ядерно цитоплазматичне відношення (ЯЦВ) гепатоцитів, об'ємна щільність гепатоцитів (ОЩГ). За допомогою ОП і ОЩГ вираховували загальний об'єм гепатоцитів (ЗОГ). Результати. Найбільша летальність припадає на 22-35 добу експерименту (7 з 11 тварин), в ході якого ЗБК значно вище контролю з максимумом на 1 добу (295±100 vs. 8±6, мкмоль/л, р<0,001). Найбільший ОП доводиться на 14 добу (14,1±1,1 vs. 8,4±1,4 см³, р<0,01). У печінки превалюють процеси клітинної проліферації, фиброзування, з поступовим витісненням гепатоцитів проліферуючими жовчними протоками і повною втратою нормальної гістоструктури. Частка гепатоцитів в печінці (ОЩГ) прогресивно знижується з 0,94 (контроль) до 0,44 (35 доба). Незважаючи на це ЗОГ спочатку зростає (max 9,7±0,36 см³, на 3 добу експерименту vs. 8,3±0,26 см³ на 1 добу експерименту, р <0,05), а після 14 діб знижується, не відрізняючись від контролю на 21 добу (8,2±1,2 vs. 7,9±1,6 см³, р>0,05) і досягаючи 5,0±0,9 см³ на 35 добу експерименту (р<0,05 в порівнянні з max на 3 добу). Статистично значущих відмінностей ПГ в ході експерименту не виявлено, а ЯЦВ, що відображає проліферативну активність гепатоцитів, має свій максимум на 14 добу (0,53±0,01 vs. 0,21±0,05 контролю, р<0,001). Висновок: при експериментальній ПОПЖШ зниження ОЩГ менше 60 % і зниження ЗОГ менш значення характерного для нормальної печінки супроводжується піком летальності, тобто є ознакою декомпенсації печінкової функції. Objective. To improve the quality of diagnosis and results of treatment in patients, suffering an acute cholecystitis, complicated by formation of perivesicular infiltrate, abscess and Mirizzi’s syndrome. Materials and methods. Results of diagnosis and surgical treatment of 694 patients, suffering an acute cholecystitis, ageing 38 - 87 yrs old, admitted to the clinic in 2010 - 2019 yrs, were analyzed. The examination have included general clinical investigation, biochemical investigations of the blood, ultrasonographic investigation of a gallbladder and extrahepatic biliary ducts, and in accordance to certain indications – computer tomography, papilloscopy and endoscopic retrograde cholangiopancreaticography. Results. Of 694 patients, suffering an acute cholecystitis in 541 (78.0%) perivesical complications were revealed. In 215 (31.0%) patients perivesical infiltrate was formed, while in 76 (11.0%) – perivesical abscess. In 250 (36.0%) patients an acute cholecystitis have developed on background of obturation jaundice, caused by choledocholithiasis in 138 patients, while in 98 patients Mirizzi’s syndrome Type I was diagnosed, and in 14 - Mirizzi’s syndrome Type II. Of 215 patients with an acute cholecystitis and perivesical infiltrate in 84 laparoscopic cholecystectomy was performed after course of antibacterial therapy, while in 131 patients – open cholecystectomy. In all 76 patients with perivesical abscess open cholecystectomy was performed. Of 138 patients, suffering obturation jaundice on background of choledocholithiasis in 82 endoscopic retrograde cholangiopancreaticography with simultaneous lithoextraction and subsequent laparoscopic cholecystectomy was conducted. In 56 patients naso-biliary drainage was installed and was held in place till calculi from common biliary duct have gone away and subsequent laparoscopic cholecystectomy performed. Of 98 patients with an acute cholecystitis and confirmed Mirizzi’s syndrome Type I in 95 laparoscopic cholecystectomy was performed, while in 3 – the open one. Of 14 patients, suffering Mirizzi’s syndrome Type II, in 10 open operation was done with sanation of biliary ducts and plasty of a common biliary duct defect, while in 4 – laparoscopic cholecystocholedocholithotomy with restoration of the bile physiological passage. Conclusion. In 78.0% patients with an acute cholecystitis perivesical complications were diagnosed. Of 531 patients with perivesical infiltrate, choledocholithiasis and Mirizzi’s syndrome in 321 (60.5%) laparoscopic operations on biliary ducts were accomplished. Open laparotomy was performed in 210 (39.5%) patients. In all the patients, suffering Mirizzi’s syndrome of both Types, physiologic passage of bile was preserved.
Abstract. The aim of the research was to studying histological and some morphomethrics feathers of the liver and related with them lethality during experimental complete and partial obstruction of the extrahepatic bile ducts (COEHBD and POEHBD). Materials and methods. The experiment was included 83 rats that were sacrificed on the 3rd, 7th, 14th, 21st, 28th and 35th days. COEHBD was modeled by ligation and transaction of the common bile duct, POEHBD was modeled by ligation of the common bile duct with a needle of a given diameter. Histological analyses of the liver was performed together with morphometric study. Results. The death of rats occurs during the first 3 days of the experiment and after 14 days, with a predominance of mortality on 14-35 days compared to the period up to 14 days (p<0.05). Death after 14 days of the experiment occurs in the model of complete cholestasis. From the all investigated morphometric measures such as the hepatocytes volume density (HVD), liver volume (LV), total hepatocytes volume (THV), the last one is the most accurately reflects the compensatory capabilities of the liver in conditions of complete and partial cholestasis and can serve as a criteria for predicting of the fatal outcome. Conclusion. The THV most accurately reflects the compensatory capabilities of the liver in experimental COEHBD and POEHBD and can serve as a criterion for predicting a fatal outcome.
