INTRODUCCIÓNListeria monocytogenes (LM) es un bacilo gram positivo asociado a infecciones en mujeres embarazadas y responsable de sepsis neonatal. En adultos no gestantes es una infrecuente pero severa infección que usualmente afecta a pacientes inmunodeprimidos.En este trabajo describimos nuestra experiencia en listeriosis del adulto resaltando factores predisponentes, curso clí-nico, respuesta terapéutica y pronóstico en una serie de 10 casos. CASOS APORTADOSPresentamos un estudio retrospectivo efectuado en el hospital de Cabueñes (grupo 2 del antiguo Insalud ahora SESPA) que atiende a una población cercana a los 300.000 habitantes en Gijón, Principado de Asturias, y que carece de los servicios de neurocirugía, cirugía vascular y maxilofacial, y en el que no se realizan transplantes de órganos. Nosotros revisamos las historias de todos los pacientes con cultivo positivo para LM entre enero de 1991 y diciembre de 2002. Se consideró caso de listeriosis del adulto al episodio de enfermedad compatible en paciente con aislamiento de LM en localización habitualmente estéril, en mayores de 14 años. Se definió como sepsis primaria cuando los hemocultivos fueron positivos, en ausencia de infección en otros lugares del organismo. Se diagnosticó de meningitis o meningoencefalitis al hallazgo de LM en líquido cefalorraquí-deo (LCR), y en los casos con cultivo negativo del LCR en los que se observaron signos citoquímicos de inflamación meníngea siempre que los hemocultivos fueran positivos. Se definió caso adquirido en la comunidad aquel cuyo periodo de tiempo entre la fecha de ingreso y la fecha de aislamiento del microorganismo en relación con la toma de la muestra, fuese inferior a 72 horas, mientras que se consideró nosocomial aquel con un periodo superior o igual a 3 días.Se hallaron 10 casos cuyas principales características se describen en la tabla I. El absceso esplénico (AE) piogénico es una patología infrecuente, de difícil diagnóstico y elevada mortalidad. La introducción de la ecografía y tomografía axial computerizada (TAC) han permitido una mayor precisión diagnóstica y terapéutica, modificando el pronóstico infausto que tenía esta entidad. Presentamos un estudio descriptivo, retrospectivo, en el hospital de Cabueñes, de 490 camas, que atiende a una población de 300.000 habitantes en Gijón. Recogimos todos los casos diagnosticados de AE por ecografía o TAC, con confirmación microbiológica o histopatológica, desde enero 1991 hasta diciembre 2005. Los AE por micobacterias o diagnosticados en otros hospitales se han excluido. Se hallaron 7 casos, 6 varones y 1 mujer, con una edad media de 66 años (35-87), cuyas principales características se describen en la tabla I. La inmunodepresión fue el factor predisponente principal, seguido de diseminación hematógena. Las manifestaciones clínicas más frecuentes fueron fiebre y dolor abdominal. La exploración fue inespecífica, con soplo cardíaco en los casos de endocarditis. Los estudios analíticos demostraban leucocitosis neutrofilica y reactantes de fase aguda. Se identificó el germen responsable en sangre y/o bazo en todos, excepto en el paciente que fue exitus. La ecografía demostró lesión focal en 5, mientras que la TAC tuvo una sensibilidad del 100%. El tratamiento antibiótico se pautó según cultivo y antibiograma, manteniéndolo entre 4-6 semanas. Se realizó drenaje percutáneo (DP) en 5 y esplenectomia en 2. Sólo hubo un exitus a las 24 horas de ingreso, que se sospechó por TC y se confirmó en autopsia. Los AE son una patología rara, con una incidencia en series de necropsias de 0,14-0,70%, similar a nuestra serie 0,19% (1-3). Afecta de forma predominante a varones sin que se conozcan las causas (1-3). Los primeros trabajos demostraban que AE afectaban a gente joven. Sin embargo, en sucesivas publicaciones se objetiva que la edad media va ascendiendo, y así en nuestro medio se encuentra en 66 años, relacionada con la mejoría en la profilaxis y tratamiento de las infecciones, y mayor esperanza de vida entre otros (1-3). En la actualidad entre las infecciones que pueden conducir la formación de AE la endocarditis aparece en primer lugar debido al alto porcentaje de embolismos en el bazo 39% (3,4). La inmunodepresión constituye el otro factor preponderante debido una mayor incidencia de bacteriemias de cualquier origen, y a una alteración de mecanismos de defensa (2,3). Las manifestaciones clínicas son similares en todas las series incluida la nuestra, con predominio de fiebre y dolor abdominal (1-3,5). La exploración es poco relevante. Los estudios analíticos son similares a otros procesos piogénicos. El diagnóstico microbiológico, bien en sangre o cultivo del AE, suele ser positivo en cerca del 90% de casos (3). Existen casos con cultivo negativo, en relación anaerobios no aislados o pacientes con antibioterapia previa. patológico del QT (QT corregido para la frecuencia > 460 milisegundos), junto con alteraciones de la amplitud o polaridad de la onda T, hallazgo que puede ser transitorio (3). Dentro de los diversos fármacos que pueden producir alargamiento del QT (antimicrobianos, antihistamínicos, antipsicóticos, antidepresivos...) se encuentran los macrólidos, antibióticos ampliamente utilizados en la práctica clínica habitual (4,5). Entre ellos destaca por su frecuencia de utilización la eritromicina, la cual produce una inhibición del canal de corriente rápida de potasio (I kr ) en el miocito, impidiendo la salida de potasio en las células del miocardio ventricular y prolongando el potencial de acción transmembrana en las fibras de Purkinje. Ello conduce a un retraso en la repolarización y a un alargamiento del QT (6). La aparición de estos efectos se halla influido por la velocidad de infusión del fár-maco, lo que indica una relación con su concentración plasmáti-ca. En el caso de nuestro paciente el macrólido utilizado fue la claritromicina, con una estructura molecular similar. Un alargamiento mayor de 500 mseg incrementa el riesgo de arritmias malignas, por lo cual cuando aparece un intervalo de esta magnitud en relación con alguna droga, debería reevaluarse su uso en función del riesgo-beneficio de dicha terapia y de la existencia de fármacos alternativos, principalmente cuando aparecen otros predisponentes como la hipokalemia o la interacción con otras drogas (2).Algunos trabajos sugieren que los mecanismos responsables de la repolarización normal del corazón varían en cada persona, pero no son aparentes en situación basal. Sin embargo, al exponerse a drogas que prolongan el QT o ante factores de riego como bradicardia o hipokalemia, es más fácil causar una prolongación del intervalo en pacientes susceptibles (2).Para establecer el diagnóstico de QT largo adquirido es fundamental la existencia de una relación temporal entre la administración de la droga y la aparición de las alteraciones electrocardiográficas, una vez descartadas otras causas (7). El diagnóstico se apoya también en la resolución del cuadro tras la retirada del fár-maco, como ocurrió en nuestro caso.Con el incremento en el uso de la claritromicina para el tratamiento de las infecciones pulmonares y la erradicación del Helicobacter pilory, los clínicos deben de estar atentos ante la posibilidad del alargamiento del QT inducido por este antimicrobiano y la producción de torsades de pointes, especialmente en pacientes con cardiopatía estructural, insuficiencia hepática y/o renal (8).
