Objectives: This study aims to investigate the prevalence, etiology, and risk factors of cervicogenic dizziness in patients with neck pain. Patients and methods: Between June 2016 and April 2018, a total of 2,361 patients (526 males, 1,835 females; mean age: 45.0±13.3 years; range, 18 to 75 years) who presented with the complaint of neck pain lasting for at least one month were included in this prospective, cross-sectional study. Data including concomitant dizziness, severity, and quality of life (QoL) impact of vertigo (via Numeric Dizziness Scale [NDS]), QoL (via Dizziness Handicap Inventory [DHI]), mobility (via Timed Up-and-Go [TUG] test), balance performance [via Berg Balance Scale [BBS]), and emotional status (via Hospital Anxiety- Depression Scale [HADS]) were recorded. Results: Dizziness was evident in 40.1% of the patients. Myofascial pain syndrome (MPS) was the most common etiology for neck pain (58.5%) and accompanied with cervicogenic dizziness in 59.7% of the patients. Female versus male sex (odds ratio [OR]: 1.641, 95% CI: 1.241 to 2.171, p=0.001), housewifery versus other occupations (OR: 1.285, 95% CI: 1.006 to 1.642, p=0.045), and lower versus higher education (OR: 1.649-2.564, p<0.001) significantly predicted the increased risk of dizziness in neck pain patients. Patient with dizziness due to MPS had lower dizziness severity scores (p=0.034) and milder impact of dizziness on QoL (p=0.005), lower DHI scores (p=0.004), shorter time to complete the TUG test (p=0.001) and higher BBS scores (p=0.001). Conclusion: Our findings suggest a significant impact of biopsychosocial factors on the likelihood and severity of dizziness and association of dizziness due to MPS with better clinical status.
P arkinson Hastalığı (PH), parkinsonizm bulgularıyla ortaya çıkan, genellikle 60 yaş civarında ortaya çıkan nörodejeneratif bir hastalıktır. Hastalığın ilk tanımı İngiliz hekim James Parkinson. [1] tarafından 1817 yılında 'Titreten Beyin Felci Üzerine Bir Çalışma' (An Essay on The Shaking Pal-sy) monografında yapılmıştır. Alzheimer Hastalığı'ndan sonra ikinci sıklıkta görülür. Parkinson Hastalığı'nın prevalansı 100.000'de 360 ve insidansı 100.000'de 18 olup tüm parkinsonizm olgularının %80'inden fazlasını oluşturmaktadır. [2-5] Denge, transfer, yürüme becerilerinin gerilediği, fiziksel aktivitelerin ve yaşam kalitesinin azaldığı bu hastalıkta postür ve yürüme bozuklukları ileri derecede özürlülük oluşturmakta, bu da rehabilitasyonu güçleştirmektedir. [3, 6-8] PH'nın ana belirtileri; bradikinezi, istirahat tremoru, rijidite ve postural instabilitedir. Yürüme paterni, evreye göre değişmekle beraber adım uzunluğunda kısalma, dakika adım sayısında azalma görülür. İleri evredeyse yürümeye başla-İnme ve Parkinson Hastalığı (PH), kişide hareket yeteneğini, kendine bakım aktivitelerini azaltan iki önemli hareket bozukluğu nedenidir. Bu sunumda inmeli olgularda, eşlik eden Parkinsonizm buguları nedeniyle rehabilitasyonun güçleştiğini, klinik ölçeklerle açıklamayı amaçladık. Fiziksel Tıp ve Rehabilitasyon Kliniği'nde Mart-Mayıs 2016 tarihlerinde yatan dört inme olgusunda Parkinsonizm birlikteliğini gözden geçirdik. Hastaların giriş ve taburculuk sırasında klin-ik ölçeklerle rehabilitasyonun etkinliği değerlendirdik. Değerlendirmede kullanılan klinik ölçekler Standardize Mini Mental Test (SMMT), Fonksiyonel Bağımsızlık Ölçeği (FBÖ), Barthel İndeksi (BI), Berg Denge Ölçeği (BDÖ) ve İnme Etki Ölçeği versiyon 3.0 (İEÖ 3.0) idi. Olguların üçü kadın, biri erkek, yaş ortalaması 74.5±9.3 yıl, ortalama yatış süresi 19±5.3 gün idi. Giriş sırasında düşük olan puanlar taburculuk sırasında da düşüktü. En iyi puana göre kıyaslandığında, olguların FBÖ ortalaması en yüksek değerin %42'si, SMMT ortalaması %55'i, taburculuk BI ortalaması %18'i, BDÖ ortalaması %0.08'i, İEÖ 3.0 ortalaması %25'i kadardı. Üç olgu tekerlekli sandalye, bir olgu yürüteçle yürüme seviyesinde taburcu edildi. İnme ile birlikte Parkinsonizm bulguları varlığının, özellikle denge bozukluğu olmak üzere, hastaların mobilite ve fonksiyonel durumunu olumsuz etkilediğini gözledik. Kognitif fonksiyonları normalin yarısı kadar olmakla beraber dengeleri ve fonksiyonel durumları yarıdan fazla oranda olumsuz etkilenmişti. Objectives-This study aimed to investigate the stiffness of the gastrocnemius (GC) muscle with acoustic radiation force impulse (ARFI) elastography after botulinum toxin-A (BTX-A) injection in children with spastic cerebral palsy (CP) and to examine the relationship between elastographic and clinical parameters. Methods-This prospective randomized single-blind controlled clinical study included 49 lower extremities of 33 children with spastic CP. They were randomized into 2 groups: group 1 (n = 25 extremities in 17 children) received BTX-A injection and a home-based exercise program; group 2 (n = 24 extremities in 16 children) received only a home-based exercise program. Patients were evaluated in pretreatment and posttreatment periods in the first and third months with ARFI elastography, the Modified Ashworth Scale, Modified Tardieu Scale, Pediatric Functional Independence Measure, Gross Motor Function Classification System, and goniometric range of motion measurement of the ankle. Results-A statistically significant difference was found in elastography of the GC muscle in group 1 only at the first month after treatment (P < .05). No statistical difference was found in elastography of the GC after treatment in group 2. According to the Modified Ashworth Scale, Modified Tardieu Scale, and ankle passive range of motion, group 1 showed significant improvements after treatment (P < .05). Also, there was a significant correlation between these clinical parameters and elastographic measurements (P < .05). Conclusions-According to the results of this study, the measurements from ARFI elastography combined with clinical parameters might be useful for evaluation of spasticity after BTX-A treatment in children with CP. Also, they might be useful in distinguishing patients who will benefit clinically, especially in the early stages of treatment.
