Sebagian besar sindrom nefrotik pada anak memberikan respons yang baik pada pengobatan awal dengan steroid. Namun sekitar 57% di antaranya menunjukkan kekambuhan berulang yang memerlukan pengobatan steroid yang berulang-ulang, hingga sebagian menunjukkan efek toksik. Pemberian steroid awal yang lebih lama dapat mengurangi jumlah pasien yang mengalami kambuh sering. Di Bagian Ilmu Kesehatan Anak FKUI-RSCM telah dilakukan penelitian uji klinik terhadap 2 kelompok pasien baru sindrom nefrotik. Kelompok I mendapat terapi steroid 60mg/m2/hari (FD) selama 4 minggu yang dilanjutkan dengan dosis 40mg/m2/hari selang sehari (AD) selama 4 minggu, sedang kelompok II mendapat terapi steroid FD dan AD masing-masing selama 6 minggu. Terhadap kedua kelompok tersebut diikuti selama 1 tahun untuk melihat kekeraban kekambuhannya. Dalam tahun 1994-1998 terdapat 35 anak yang memenuhi kriteria inklusi, namun hanya 18 anak (10 anak kelompok I dan 8 anak kelompok II) yang menyelesaikan terapi dan ikut dalam observasi selama 1 tahun. Tidak didapatkan perbedaan klinis dan laboratoris pada kedua kelompok tersebut. Kekambuhan pertama kali timbul dalam 4 minggu setelah pengobatan pada 2 dari 8 (25%) anak kelompok II dan 3 dari 10 (30%) anak kelompok I. Kekambuhan lebih dari 2 kali atau lebih dalam 6 bulan pertama setelah pengobatan terdapat pada 4 (40%) anak kelompok I dan 2 (25%) anak kelompok II, sedangkan kekambuhan 4 kali atau lebih dalam 1 tahun setelah pengobatan tidak ditemukan pada kelompok II, namun didapatkan pada 2 (20%) anak kelompok I. Terdapat 1 (10%) anak kelompok I dan 2 (25%) anak kelompok II yang tidak pernah kambuh. Disimpulkan bahwa sindrom nefrotik yang mendapat terapi steroid awal lebih lama (12 minggu) mempunyai kecenderungan lebih jarang kambuh bila dibandingkan dengan kasus yang mendapat terapi steroid awal lebih pendek (8 minggu).Kata kunci: sindrom nefrotik, steroid, remisi, kambuh, kesintasan.
Pengobatan sindrom nefrotik pada anak mengalami perubahan dari masa ke masa. Sudah sejak lama kita mengadopsi rekomendasi International study of kidney disease in children (SKDC) yaitu pemberian kortikosteroid 8 minggu, 4 minggu dosis penuh dilanjutkan dengan 4 minggu dosis alternating. Dengan skema pengobatan ini 80% sindrom nefrotik idiopatik pada anak mengalami remisi, tetapi 70%-80% di antaranya mengalami relaps dan separuhnya relaps berulang/frekuen. Pengobatan hanya 8 minggu dirasakan kurang adekuat (Kidney Disease Improving Global Outcome= KDIGO), maka KDIGO membuat rekomendasi baru pada tahun 2013. Pengobatan inisial dapat dipilih dengan pemberian kortikosteroid 12 minggu atau tetap 8 minggu dan dilanjutkan dengan penurunan dosis selama 2-3 bulan (tapering-off). Namun usulan KDIGO dibantah oleh Japanese Study Group of Kidney Disease in Children (JSKDC) tahun 2015 dengan melakukan penelitian untuk membandingkan prednisolon 4-4 minggu selama 6 bulan yaitu 4-4 minggu + tapering off. Hasil penelitian tersebut membuktikan bahwa pemberian prednison 4-4 minggu tidak lebih inferior dan pemberian 6 bulan tidak mengurangi relaps. Rekomendasi lain dari KDIGO 2013 diterima dengan sedikit modifikasi yang akan dikemukakan pada makalah ini secara rinci, baik untuk pengobatan sindrom nefrotik sensitif steroid (SNSS) maupun sindrom nefrotik resisten steroid (SNRS). We have been adopted ISKDC (International study of kidney disease in children) recommendation which were the administration of 8 weeks course of steroid (4 weeks of full dose corticosteroid, followed by 4 weeks of alternate dose). Remission was achieved by 80% of idiopathic NS children, although 80% of them were relapsed and half of the relapser were the frequent one. Since the 8 weeks course of treatment was considered inadequate, KDIGO (Kidney Disease Improving Global Outcome) released new recommendation in 2013. Initial treatment was proposed to be 12 weeks or remain 8 weeks which was continued by 2-3 months of tappering off. Another publication comparing the 6 months course of prednisolon administration (4-4 weeks) to the previous recommendation (4-4 weeks followed by tappering off of prednisolon) by JSKDC in 2015 refuted the KDIGO recommendation. AbstrakAntara tahun [1985][1986][1987][1988][1989][1990][1991][1992][1993][1994][1995] (10 tahun) telah dilakukan penelitian terhadap 30 anak dengan nefritis lupus yang dikumpulkan dari 7 pusat Nefrologi Anak di Indonesia. (Jmur rata-rata penderita ialah I I,7 tahun (berkisar antara 8 satnpai I8 tahun) dengan perbandingan anak perempuan terhadap taki-laki 5 berbandtng l. Gejala klinis yang terbanyak diremukan adalah panas tinggi 8loÀ, ruam muka 66,6ok dan artritis atau artralgia 44,4%. JengkoL intoxication is well-known in Indonesia. We report a series of 39 parents with jengkol intoxication admitted to the Department of Child Health Cipto Mangunkusumo Hospital, Jakarta, during the period of 1984 through 1993. Patients ranged in age from 3 to 14 years. The male to female ratio of all cases was 1.8 to 1 but the ratio of patients suffering from acute renal failure was 5.7 to 1. Oliguria or anuria presented in all cases with acute renal failure. Three patients underwent peritoneal dialysis which gave rapid improvement; 2 patients had died due to acute renal failure before dialysis could be performed. When compared with previous reports, it seems that the admission for jengkol intoxication has been declining. Change in the way of consuming the bean and increasing number of hospitals in Jakarta may be responsible for the decline of cases admitted.
