The authors report a rare case of simultaneous dorsal dislocation of all five metatarsophalangeal joints of the right foot after a motorcycle accident. The first MTP joint dislocation was an open injury. A formal debridement was performed under spinal anesthesia. After applying the reduction maneuver to all lesser metatarsophalangeal joints simultaneously, a satisfactory and stable reduction was obtained, fixation with Kirshner wires was not necessary, the leg was immobilized in a short plaster splint, and a full-thickness skin graft was performed five days later. The soft tissues healed adequately, and after three months, the patient had returned to his pre-injury activities. At 36 months follow-up evaluation, the patient had a full painless range of motion and no radiographic evidence of arthrosis.
