RESUMENLa conducta suicida podría considerarse una continuidad que va desde aspectos cognitivos como la ideación suicida, hasta los conductuales, como el intento suicida o el suicidio. El suicidio en la adolescencia se ha convertido en un importante problema de salud pública. Según las estimaciones de la OMS, uno de los hallazgos más preocupantes a nivel mundial es el aumento de las tasas de suicidio que se está produciendo entre los jóvenes (15-24 años), que se sitúa como una de las tres causas de muerte más frecuentes en este grupo de edad. Un gran número de factores que podrían contribuir con la ocurrencia de la conducta suicida han sido identificados, los cuales incluyen factores individuales, familiares y sociales. El modelo diátesis-estrés ha sido propuesto para dar un sentido causal del amplio rango de factores que contribuyen con la conducta suicida. La diátesis refleja un incremento de la vulnerabilidad a lo largo del tiempo para la conducta suicida, por ejemplo, ser más impulsivo o agresivo y por lo tanto más posibilidades de actuar sobre sentimientos suicidas. El estrés puede incluir factores, tales como, un divorcio parental y otros eventos de vida adversos y factores ambientales. Se pueden identificar varios factores que contribuyen con la vulnerabilidad (diátesis) para fenómenos suicidas en adolescentes y otros que actuarían como factores estresantes. En adición, hay varios factores que podrían actuar en ambas vías, dependiendo de su asociación temporal con fenómenos suicidas.PALABRAS CLAVE: Conducta suicida, factores asociados, adolescentes. SUMMARYSuicidal behavior could be considered a continuum from cognitive aspects such as suicidal ideation, to behavioral, such as suicide or suicide attempt. Suicide in adolescence has become a major public health problem. Diathesis-stress model has been proposed to give a causal sense to the wide range of factors that contribute to suicidal behavior. The diathesis reflects an increase in vulnerability over time for suicidal behavior, for example, being more impulsive and aggressive, and therefore more likely to act on suicidal feelings. Stress can include factors, such as, parental divorce and other adverse life events and environmental factors. This paper presents a descriptive study, using the Birleson Scale to determine the frequency of depressive symptomatology in children and adolescents with chronic renal insufficiency (CRI) undergoing hemodialysis (HD) and chronic peritoneal dialysis (CPD). There were 67 patients (40 female and 27 male) with a mean age of 14.76 ± 2.71 years, duration of illness ≥3 months, 43 (64.18%) patients with CPD and 24 (35.82%) undergoing HD. The frequency of high occurrence, low occurrence, and absence of depressive symptomatology was 10.45% (n = 7), 43.28% (n = 29), and 46.27% (n = 31), respectively; all of the seven (100%) patients with high occurrence of depressive symptomatology were female (P = 0.04), and none of these (0%) had a friend to confide in (P = 0.03). Depressive symptomatology in patients with CPD was associated with a lower weekly K t/V compared to those without depressive symptomatology (2.15 ± 0.68 versus 2.52 ± 0.65; P = 0.01). There was no association with patient age, caregiver, time and dialysis type, anemia, bone disease, nutritional or financial status, origin, schooling, or employment.
La depresión en adolescentes constituye un problema de salud pública por su frecuencia creciente y sus consecuencias.Objetivo: Determinar las prevalencias y los factores asociados con el episodio depresivo en adolescentes de lasciudades de Cajamarca, Huaraz y Ayacucho. Material y Métodos: Estudio epidemiológico de corte transversal, conmuestreo probabilístico trietápico por conglomerados. Se usó la base de datos del Estudio Epidemiológico en SaludMental de la Sierra Peruana 2003. La población estudiada comprendió a los residentes de 12 a 17 años. Se utilizaroncuestionarios para medir variables demográficas y socioeconómicas, aspectos generales de salud mental, trastornosclínicos y relaciones intrafamiliares. Se ajustaron modelos de regresión logística múltiple. Resultados: La prevalenciade episodio depresivo en adolescentes fue 5,7% (IC 95%: 4,5%-7,2%), observándose según el análisis multivariado,asociaciones estadísticamente significativas con: sexo femenino (OR: 2,7, IC 95%: 1,4-5,2), haber tenido enamorado(a) (OR: 2,3, IC 95%: 1,2-4,1), síndrome psicótico (OR: 5,3, IC 95%: 2,2-13,2) y deseos de morir (OR: 3,2, IC 95%: 1,8-5,8). Conclusiones: La prevalencia de episodio depresivo en adolescentes de las tres ciudades estudiadas fue similar a la encontrada en otros países. Los factores asociados que mejor predijeron el episodio depresivo fueron: sexo femenino, haber tenido enamorado(a), síndrome psicótico y deseos de morir alguna vez en la vida; los cuales podrían ser evaluados como factores de riesgo en estudios posteriores.
