Fatores genéticos, neurobiológicos e ambientais participam da gênese das depressões. Esta breve revisão visa enfatizar os estudos sobre os aspectos genéticos, neuroquímicos e neuroanatômicos na etiologia e fisiopatologia das depressões e suas implicações no desenvolvimento de novos tratamentos. Procura-se enfatizar as limitações encontradas até o momento na tentativa do estabelecimento de uma etiopatogenia comum às depressões, principalmente em função da dificuldade no diagnóstico e da heterogeneidade na fenomenologia do episódio agudo e no curso longitudinal. Perspectivas para futuras pesquisas também são apresentadas.
Genetic, neurobiological and environmental factors play a role in the pathogenesis of depressive disorders. This brief review points to the major studies on the genetics, neurochemistry and neuroanatomy of depressive disorders as well as their implications on the development of new treatments. The limitations found by several groups in establishing a common etiopathogenesis are emphasised in light of the difficulty in establishing a reliable diagnosis and of the clinical heterogeneity found in the phenomenology of acute episodes and long-term outcome. Future research perspectives are also presented

In this article we examined the heritability of alcohol dependence. A review of family, twin and adoption studies, allowed us to support the thesis of an important genetic component in this dependence. The transmission of this heritability occurs through a biological vulnerability associated to environmental factors, in a model called epigenetic. We also discussed the relationship between biological vulnerability and high-risk phenotypes for alcohol dependence. In the end, we briefly comment on the molecular genetic studies associated with this disorder.

Os autores revisam neste artigo o conjunto de evidências genético-epidemiológicas que indicam a presença de fatores genéticos na vulnerabilidade para os transtornos afetivos. Apresentam também os dados obtidos até o momento, através de estratégias de genética molecular na busca de genes de susceptibilidade para o transtorno afetivo bipolar e para a depressão.

Os autores revisam os estudos que buscam determinantes genéticos para uma boa ou má resposta ao tratamento farmacológico da depressão, incluindo a verificação de diferenças interindividuais nos mecanismos farmacocinéticos -estudos de variantes gênicas das enzimas metabolizadoras das drogas -e farmacodinâmicos -estudos de variantes funcionais dos genes que codificam os sítios envolvidos no mecanismo de ação das drogas. Concluem que o conhecimento acerca da presença de variantes funcionais nos genes das isoformas envolvidas no metabolismo dos antidepressivos como o CYP2D6, ou nos genes codificando seus sítios de ação como a proteína transportadora de serotonina (5-HTT), podem fornecer importantes subsídios para a personalização da escolha de medicações e dosagens utilizadas no tratamento da depressão. ResumoThe present article reviews the studies on genetic determinants for drug response to the treatment of depression. These investigations assess the individual differences in the pharmacokinetic mechanisms -such as the genetic variants of genes coding for drug-metabolizing enzymes -and in the pharmacodynamics, looking at the genetic variants of genes coding for drug targets. The authors conclude that the knowledge about the functional variants of the genes coding for antidepressant metabolizing enzymes such as CYP2D6, or genes coding for antidepressant targets such as the serotonin transporter protein (5-HTT), could assist in individualizing the selection of medication and dosage for depression treatment.

A psicofarmacogenética é uma nova área de investigação derivada da psicofarmacologia e da genética. Esta disciplina procura identificar genes de suscetibilidade associados a diferentes padrões de resposta terapêutica e/ou ao variado grau de efeitos colaterais dos psicofármacos. O presente artigo discorrerá, após um breve histórico, sobre alguns exemplos de variantes genéticas relacionadas com a resposta terapêutica de antipsicóticos e antidepressivos.
Psychopharmacogenetics is a new area of investigation originating from the application of genetics to psychopharmacology. This discipline attempts to identify susceptibility genes which are responsible either for the drug response of a treatment or for the side effects of different psychopharmarcological drugs. The present article will discuss, after a brief historical background, some examples of gene variants which are associated with the therapeutic response of antipsychotic and antidepressive drugs
