The RTs studied are not useful for screening for NG at the point of care. In case of CT a recommendation about their use in routine care should be supported by a cost-effectiveness analysis. In screening populations at high risk of sexually transmitted infections or pregnant women, the RTs for syphilis should be used.
Introducción: en la evaluación de la utilidad de una prueba diagnóstica, se requiere en algunas situaciones valorar la reproducibilidad de los resultados o la concordancia de los mismos al compararla con otra prueba que no sea usada como patrón de oro de la entidad. El objetivo de este documento es presentar los métodos estadísticos utilizados para evaluar la reproducibilidad y/o concordancia de las observaciones clínicas o paraclínicas, sus bases teóricas y algunos ejemplos de cómo se han aplicado.Metodología: se realiza una revisión sobre las bases teóricas de la evaluación de la concordancia y la reproducibilidad, además se ilustra su aplicación en la literatura con ejemplos relacionados con la obstetricia y la ginecología.Resultados: la estimación de la concordancia se hace por medio de la prueba Kappa en variables dicotómicas u ordinales. En el caso de variables continuas, se debe preferir el uso del coeficiente de correlación intraclase o el coeficiente de correlación y concordancia sobre el uso del coeficiente de Pearson o la prueba t de Student pareada. Los métodos utilizados deben ser interpretados de acuerdo al contexto clínico donde fueron empleados.Conclusiones: la selección de los métodos estadísticos para la evaluación de la concordancia y la reproducibilidad depende del tipo de variable a medir y de los parámetros que se quieran evaluar, ya sea sólo la reproducibilidad o también la exactitud.
BackgroundHIV and congenital syphilis are major public health burdens contributing to substantial perinatal morbidity and mortality globally. Although studies have reported on the costs and cost-effectiveness of rapid diagnostic tests (RDTs) for syphilis screening within antenatal care in a number of resource-constrained settings, empirical evidence on country-specific cost and estimates of single RDTs compared with dual RDTs for HIV and syphilis are limited.MethodsA cluster randomised controlled study design was used to compare the incremental costs of two testing algorithms: (1) single RDTs for HIV and syphilis and (2) dual RDTs for HIV and syphilis, in 12 health facilities in Bogota and Cali, Colombia. The costs of single HIV and syphilis RDTs and dual HIV and syphilis RDTs were collected from each of the health facilities. The economic costs per woman tested for HIV and syphilis and costs per woman treated for syphilis defined as the total costs required to test and treat one woman for syphilis were estimated.ResultsA total of 2214 women were tested in the study facilities. Cost per pregnant woman tested and cost per woman treated for syphilis were US$10.26 and US$607.99, respectively in the single RDT arm. For the dual RDTs, the cost per pregnant woman tested for HIV and syphilis and cost per woman treated for syphilis were US$15.89 and US$1859.26, respectively. Overall costs per woman tested for HIV and syphilis and cost per woman treated for syphilis were lower in Cali compared with Bogota across both intervention arms. Staff costs accounted for the largest proportion of costs while treatment costs comprised <1% of the preventive programme.ConclusionsFindings show lower average costs for single RDTs compared with dual RDTs with costs sensitive to personnel costs and the scale of output at the health facilities.Trial registration number NCT02454816; results.
Background The use of rosuvastatin plus colchicine and emtricitabine/tenofovir in hospitalized patients with SARS-CoV-2 disease (COVID-19) has not been assessed. The objective of this study was to assess the effectiveness and safety of rosuvastatin plus colchicine, emtricitabine/tenofovir, and their combined use in these patients. Methods This was a randomized, controlled, open-label, multicentre, parallel, pragmatic study conducted in six referral hospitals in Bogotá, Colombia. The study enrolled hospitalized patients over 18 years of age with a confirmed diagnosis of COVID-19 complicated with pneumonia, not on chronic treatment with the study medications, and with no contraindications for their use. Patients were assigned 1:1:1:1. 1) emtricitabine with tenofovir disoproxil fumarate (FTC/TDF, 200/300 mg given orally for 10 days); 2) colchicine plus rosuvastatin (COLCH+ROSU, 0.5 mg and 40 mg given orally for 14 days); 3) emtricitabine with tenofovir disoproxil plus colchicine and rosuvastatin at the same doses and for the same period of time (FTC/TDF+COLCH+ROSU); or 4) the Colombian consensus standard of care, including a corticosteroid (SOC). The primary endpoint was 28-day all-cause mortality. A modified intention-to-treat analysis was used together with a usefulness analysis to determine which could be the best treatment. The trial was registered at ClinicalTrials.gov: NCT04359095 Findings Out of 994 candidates considered between August 2020 and March 2021, 649 (65.3%) patients agreed to participate and were enrolled in this study; among them, 633 (97.5%) were included in the analysis. The mean age was 55.4 years (SD ± 12.8 years), and 428 (68%) were men; 28-day mortality was significantly lower in the FTC/TDF+COLCH+ROSUV group than in the SOC group, 10.7% (17/159) vs. 17.4% (28/161) (hazard ratio [HR] 0.53; 95% CI 0.29 to 0.96). Mortality in the FTC/TDF group was 13.8% (22/160, HR 0.68, 95% CI 0.39 to 1.20) and 14.4% in the COLCH+ROSU group (22/153) (HR 0.78, 95% CI 0.44 to 1.36). A lower need for invasive mechanical ventilation was observed in the FTC/TDF+COLCH+ROSUV group than in the SOC group (risk difference [RD] - 0.08, 95% CI 0.11 to 0.04). Three patients presented severe adverse events, one severe diarrhoea in the COLCH+ROSU and one in the FTC/TDF+COLCH+ROSU group and one general exanthema in the FTC/TDF group. Interpretation The combined use of FTC/TDF+COLCH+ROSU reduces the risk of 28-day mortality and the need for invasive mechanical ventilation in hospitalized patients with pulmonary compromise from COVID-19. More randomized controlled trials are needed to compare the effectiveness and cost of treatment with this combination versus other drugs that have been shown to reduce mortality from SARS-CoV-2 infection and its usefulness in patients with chronic statin use.
Objetivos: establecer la concordancia entre la estimación visual y la medición del volumen recolectado en una bolsa del sangrado intraparto, según las características del personal encargado de la atención del parto en un hospital universitario de mediana complejidad y la correlación entre el volumen recolectado y el cambio en la hemoglobina y el hematocrito posparto.Materiales y métodos: se realizó un estudio de concordancia en una cohorte de 168 embarazadas atendidas por parto normal, en un hospital general de nivel medio de complejidad ubicado en Bogotá, Colombia. Se evaluó la concordancia por observadores simultáneos de niveles crecientes de formación y experiencia, mediante el coeficiente de correlación concordancia (CCC Lin), límites de acuerdo del 95% y la correlación entre el cambio en la hemoglobina y hematocrito con el volumen recolectado mediante el coeficiente de Pearson.Resultados: la mediana del volumen recolectado fue 494 mL (p5 = 110, p95 = 1.320), la de la caída del hematocrito de 3,8% y de hemoglobina de 1,2 g/dL. La concordancia global fue aceptable [CCC = 0,72 IC 95%: 0,67-0,76)] con una subestimación visual media de 110,2 mL (límites de acuerdo 95% = -562,1 mL y 341,8 mL). La subestimación y la pérdida de la confiabilidad fueron crecientes a mayor volumen recolectado para todas las categorías de evaluadores. La correlación entre volumen recolectado y el cambio en hemoglobina y hematocrito fue 0,55 y 0,53.Conclusiones: la confiabilidad de la estimación visual es aceptable para volúmenes pequeños y disminuye al aumentar el volumen recolectado. Se deben introducir instrumentos y educación continuada que permitan mejorar la confiabilidad de la estimación del sangrado intraparto para prevenir la hemorragia posparto.
Objetivo: estimar la prevalencia y etiología de las infecciones de transmisión sexual, y de las infecciones endógenas en mujeres en edad reproductiva con síntomas de infección del tracto genital inferior sintomáticas, y describir los factores asociados. Materiales y métodos: estudio de corte transversal en tres centros de atención en Bogotá. Se realizó el diagnóstico etiológico mediante puntaje de Nugent para vaginosis bacteriana (VB), cultivo para Candida, y frotis en fresco para trichomonas. En un subgrupo de pacientes se realizó cultivo In Pouch TM para T. vaginalis (TV), PCR para C. trachomatis (CT) y N. gonorroheae (NG), y pruebas serológicas para sífilis y VIH. Los factores de riesgo fueron evaluados comparando los dos grupos por medio del Odds Ratio (OR) y el intervalo de confianza del 95%. Resultados: 1385 mujeres fueron incluidas, de ellas 115 (8,3%) eran trabajadoras sexuales. Se confirmó la presencia de alguna infección del tracto genital (ITG) en 731 (52,7%) de las mujeres; 560 (40,4%) presentaron infecciones endógenas y 170 (12,3%) infecciones de transmisión sexual (ITS). La etiología más frecuente fue la VB en 549 (39,6%), seguida por candidiasis en 153 (11%). CT fue detectada en 134 (9,7%) y NG en 19 (1,4%). TV fue detectada por frotis en fresco en 11 (0,8%) y por cultivo en 8 de 634 (1,2%), sífilis en 12 (0,8%) y VIH en 1 (0,07%). Las mujeres trabajadoras sexuales tuvieron mayor riesgo de presentar cualquier ITS (OR: 2,0; IC 95% 1,2-3,3). Las mujeres con ITS tuvieron una edad promedio de 27,9 (± 7,8), y aquellas que no tenían ITS de 31,9 (± 8,9) años. El consumo de licor con frecuencia entre diaria y semanal fue mayor en las mujeres con ITS (OR: 2,6; IC 95%: 1,4-4,5). Conclusiones: se identificó la etiología en el 52,7% de las mujeres que consultaron por síntomas de infección del tracto genital inferior. La infección más frecuente fue vaginosis bacteriana, y clamidia dentro de la infecciones de transmisión sexual. No se identificó ninguna etiología infecciosa específica en el 47,3% de las mujeres aun cuando se utilizó el patrón de oro diagnóstico para los diferentes microorganismos.
