Most liver tumors can be removed with conventional resection techniques employing partial or total vascular occlusion when needed. Duration of tolerable warm ischemia has not yet been defined, but it seems to be well tolerated up to 60 min. In a few cases with extended vascular resection and reconstruction liver protection by hypothermic perfusion is advantageous. This can be achieved by in situ perfusion, ante situm resection or ex situ resection. Major reconstruction of hepatic vessels with good technical access should be performed under in situ hypothermic protection using veno-venous bypass. Tumors involving the hepatic venous confluence and/or retrohepatic vena cava should be approached by either the in situ, or preferentially, the ante situm resection technique. The indication for an ex situ liver resection resulting in autotransplantation of the remnant liver exists only in rare cases for oncological reasons.
Wghrend im entziindungsfreien Stadium die Operationen am Gallenwegssystem ausgezeiehnete Erfolge und die niedrige Sterbliehkeitsziffer yon 0,6--2% aufweisen, steigt die Mortalitgt auf 20--35% an, wenn ein Versehlul3ikterus vorliegt, insbesondere wena Bauchfell und Pankreas mitbeteiligt sind oder der VersehluB einem alten Steinleiden mit Cholangitis aufgepropft ist. Parallel geht die Zahl der mehr oder minder schweren postoperativen Komplikationen. Da es aber bis heute nicht gelungen ist, eine Funktionsprfifung auszuarbeiten, die /~hnlieh wie in der Urologie fiber die ehirurgische Prognose aussagen kann, muf~ man versuehen, mit Hilfe der ~qaehbehandlung den Komplikationen erfolgreich zu begegnen. Leider entbehren auch die bisher ausgearbeiteten Mal~nahmen der ~achbehandlung der letzten Vollst~ndigkeit, da nut einzelne pathologisehe Stoffweehselabl~ufe n~her bekannt sind.Naeh A~SCRi3TZ ist die Gefahr der Operation beim Versehluf3ikterus in erster Linie in der mehr oder minder schweren Lebersch~digung zu suehen, die meist proportional der DaRer des Versehlusses zunimmt. Leider finder man in der Anzeigestellung zum ehirurgischen Vorgehen noch immer eine a]lzugro6e Zuriickha]tung, so da$ die Patienten nieht selten zu einem sparer Zeitpunkt iiberwiesen werden. Die einzelnen Untersuehungsergebnisse und Funktionspriifungen sind nieht endgfiltig entseheidend, denn trotz normaler oder gering versehobener Untersuehungswerte kSnnen die latenten StSrungen gleiehsam einen ttShepunkt erreieht haben, der nach dem Eingriff dureh das Versagen aller lebenswiehtigen Funktionen offensiehtlich wird.Mit groSer Gewil3heit nimmt man an, dR6 ein urs/~chliches Moment der Intoxikation in einer Leberschgdigung zu suchen ist. Als aug f/~lligstes Merkmal dieser Leberinsuffizienz stellt sieh bei dem lgnger bestehenden meehanischen Choledoehusversehlu6 die Glykogenverarmung ein. BEI~IqltARD konnte zwar naehweisen, dR6 die Leber auch naeh Versehlul~ der Gallenwege bei Angebot yon Traubenzueker noeh die F~higkeit zur Glykogenbildung besitzt, selbst wenn bei lgnger dauerndem Versehlu$ keine Spur mehr yon Glykogen nachzuweisen
