Objective: This study aimed at evaluating whether changes in the insertion angle is a determining factor in the positioning of the miniscrews body in a region with larger interradicular space in the posterior maxilla. Methods: Analysis of 60 posterior maxillary quadrants were made using images obtained by means of cone-beam computed tomographic image (CBCT), with 0.076-mm voxel, which presented a real miniscrew inserted in the mesial region of the maxillary first molars, serving as reference point for the placement of the virtual miniscrews. Measurements of the distances between roots were made in three points on the body of the virtual miniscrews (A, B and C), at four different angulations, 70o, 60o, 50o and 40o (T1 to T4), in relation to the long axis of the second premolar. This evaluation was made in four groups, selected in accordance with the disposition of the roots of the second premolars and first molars: Group 1 (all types of roots), Group 2 (convergent roots), Group 3 (divergent roots) and Group 4 (parallel roots). Results: There were no statistically significant differences in the measurements of points A, B and C, at the different angles (70o, 60o, 50o and 40o) and in the different groups (p > 0.05). Conclusions: Changes in the insertion angle is not a determinant factor in the positioning of miniscrews body in regions with larger interradicular space in posterior maxilla.
INTRODUÇÃO: As más oclusões de Classe II de Angle de natureza dentoalveolar ou esquelética moderada podem ser tratadas por meio de exodontias na arcada superior, distalização da arcada superior e/ou mesialização da arcada inferior. Para atingir a relação dentária final desejada, além das exodontias, pode-se utilizar dispositivos intermaxilares ou diversos tipos de recursos de ancoragem convencionais; porém, todos promovem algum tipo de efeito colateral, que pode ser indesejado e de difícil controle. A introdução dos miniparafusos extra-alveolares na crista infrazigomática (IZC) aumentou a eficiência na distalização da arcada superior, simplificando a mecânica ortodôntica e anulando os efeitos colaterais indesejados, por meio de um procedimento minimamente invasivo. A maior vantagem desses dispositivos é posicionar o corpo dos miniparafusos afastado das raízes dos dentes a serem movimentados, permitindo maiores movimentos sagitais, sem a necessidade de reposicionamento desses dispositivos ou utilização de cursores. Além disso, outra vantagem a ser destacada é a simplicidade da instalação, com boa aceitação dos pacientes. OBJETIVO: Relatar um caso clínico no qual foram utilizados miniparafusos extra-alveolares na IZC como recurso de ancoragem para efetuar a distalização da arcada superior, associados a aparelhos autoligáveis com a prescrição MBT, na correção de uma Classe II de natureza esquelética suave. CONCLUSÃO: A associação dos miniparafusos extra-alveolares com o sistema de aparelhos autoligáveis se mostrou eficiente na correção da má oclusão de Classe II, simplificou a mecânica ortodôntica, inibiu os efeitos colaterais indesejados na arcada inferior, obteve excelente controle de torque e diminuiu a dependência de colaboração do paciente.
INTRODUÇÃO: o controle da ancoragem é de fundamental importância para o sucesso do tratamento ortodôntico, principalmente na correção das assimetrias, onde se torna ainda mais crítico. Os métodos convencionais de ancoragem utilizados para tratar estes tipos de anomalia são mais complexos e podem causar movimentos indesejáveis na unidade de reação ou, ainda, ser rejeitados pelos pacientes, devido ao comprometimento estético. A utilização de microparafusos como unidades de ancoragem, além de anular os efeitos colaterais indesejáveis, simplifica a mecânica ortodôntica, fornece uma maior previsibilidade ao resultado do tratamento, proporciona maior conforto e estética ao paciente, reduz o tempo de tratamento e possibilita a correção de casos com perdas dentárias, já que fornece uma ancoragem direta. OBJETIVO: realizar uma revisão de literatura sobre o tratamento das assimetrias dentárias com o uso de microparafusos ortodônticos de titânio como ancoragem, ilustrando-a com alguns casos clínicos.
INTRODUÇÃO: A correção da má oclusão de Classe II divisão 1 em pacientes com crescimento hiperdivergente e deficiência mandibular requer uma abordagem diferente dos tratamentos ortodônticos tradicionais, que não apresentam bons resultados. OBJETIVO: O presente artigo apresenta, por meio do relato de um caso clínico, uma sugestão de protocolo para corrigir esse tipo de deformidade dentofacial, associando a contenção do crescimento vertical da maxila com miniparafusos ao estímulo de crescimento da mandíbula com propulsor mandibular. DESCRIÇÃO DO CASO: Foi efetuada a intrusão da maxila de uma paciente que se encontrava no pico de crescimento, utilizando miniparafusos na região anterior, entre incisivos laterais e caninos; na região posterior vestibular, entre primeiros molares e segundo pré-molares; e no palato, entre os primeiros e segundos molares. Essa abordagem gerou uma maior quantidade de intrusão na região anterior da maxila, promovendo uma rotação anti-horária no plano oclusal de 4,5° (PHF.Pl Ocl de 13° para 8,5°). Juntamente com essa mecânica, foi instalado um propulsor mandibular rígido fixado aos arcos do aparelho ortodôntico e posicionado entre os molares superiores. Na mandíbula, o propulsor foi fixado na distal dos primeiros pré-molares e ancorado em miniparafusos posicionados entre os primeiros molares e segundos pré-molares, por vestibular. RESULTADOS: Essa associação promoveu uma redução do plano mandibular em 4° (FMA de 31° para 27°) e no ANB de 6° (de 12° para 6°). As compensações dentárias foram inibidas, com o objetivo de potencia- lizar a correção ortopédica. A inclinação dos incisivos superiores e inferiores teve uma redução de 2° (IMPA de 97° para 95° e 1 .Pp de 108° para 106°). A convexidade facial reduziu 6° (Â Conv. Face de 151° para 157°). CONCLUSÃO: A instalação do propulsor mandibular promoveu uma alteração na face da paciente, que se sentiu estimulada pela melhora imediata do seu perfil. O prognóstico desse tratamento depende da quantidade de intrusão na maxila, associada ao potencial de crescimento da mandíbula.
