I. Der vorbereitende Schaden.Seit Beginn der bakteriologischen J~ra, seit Petten]ro/ers und seines Assistenten heroischem Selbstversuch bemfhen sich Bakteriologen und Kliniker, die weehselnde Bedeutung yon Kr~nkhei~serregern und Disposition ffir die Seuchen zu erforschen. Unter den Seuchen stehen Masern und Pocken mit hoher Pathogeni't~t, bei denen Infektion fast gleichbedeutend ist mit Erkrankung (hoher Kontagionsindex), am Anfang einer Reihe von Infektionskrankheiten, an deren Ende Krankheiten mit geringer Pathogeniti~t, wie z. B. Scharlach und Kinderliihmung stehen, bei denen zwar die Infektion die Voraussetzung, aber keineswegs der wesentlichste :Faktor fiir den Ausbruch der Krankheit darstellt (niecierer Kontagionsindex). Auf dem Boden der Theorie der stillen Feiung stehend kann man fiir die Poliomyelitis (P.) leicht erreehnen, dal3 yon 100 mit P.-Virus infizierten empf~nglichen Personen hSchstens eine manifest erkrankt 1. Es drangt sieh einem yon selbst die Frage auf, welche Konstellation verschiedenster Umst~nde gerade in diesem einen ungliiekseligen Fall die Infek~ion zur Krankheit werden lieB, warum es in diesem einen Falle in bisher als ,,typisch" angesehener Weise zum Einbruch des Virus und vor allem zur Entzfindung in den VorderhSrnern des Riiekenmarks kam. Verschiedenheiten der Virulenz des Erregers konnten in solchem l~IaBe nicht gefunden werden und waren yon vornherein nicht wahrseheinlieh. Es bleibt also nut der Einzelorganismus selbst, der auf die Infektion in bestimmten Ausnahmef~llen mit Krankheit reagiert. So hat sich bei der Erforschung der Kinderliihmungspathogenese, besonders in Deutschland, in der letzten Zeit ein gewisser Wandel bemerkbar gemacht. Neben dem frtiher und jetzt noch vor ahem in Amerika ganz vorwiegend betriebenem Studium der Exposition, der Epidemiologie, der Infektiosit~tt und Virulenz sowie der Ausbreitung des Virus in Umwelt und 0rganismus ste]lt sich heute das Problem der Pathogenese ftir eine ganze Anzahl yon Forsehern als ein vorwiegend dispositionelles dar.i AnzahI der gemeldeten bis zu einem gewisson Lebensalter Erkrankter dureit Anzahl der Personen mit positivem l~eutralisationstest des gleiehen Lebensalters, erreehnet aus dem bekannten Prozentsatz nach Aycoc]~ und Kvamer.
Das yon NITSCHKE SO klar herausgearbeitete Krankheitsbild der frfihkindliehen Jnterstit.iellen plasmacellul/iren Pneumonie stellt uns immer noch vor grol~e, bisher ungel6ste l~robleme. An der infekti6sen ~tiologie zweifelt heute niemand mehr. Von vielen Autoren wurde fiber lange Infektketten in Frfihgeborenem'/iumen berichtet. Die Diskussion fiber den eigentliehen Infektionsstoffist jedoch noch keineswegs abgeschlossen. Diskutiert wird vor allem die Parasitennatur (Pneumoeystis carinii) und die Hefennatur (Saccharomyces oetospor.) des Erregers, w~hrend nurmehr wenige an ein Virus als Obertr/iger glauben (VA~v,K u. JIROVEO, GIESE, BACItlVfA:Ni'q, K. WEISSE n. BIELING, ~/~OSEI% 11. 0o.).Da die Erkrankung bisher als aussehliel31ich mensehenpathogen gilt und hier wiederum die frfihgeborenen und debilen Kinder ergreift, sind der experimente]len Forschung enge Grenzen gesetzt. Erst in letzter Zeit ist es WEnnE~ gelungen, bei neugeborenen mit Cortison vorbehandelten l%atten eine Pneumonie zu erzeugen, die Xhnlichkeit mit der menschlichen interstitie]len Pneumonie aufweist und bei der sieh im histologischen Bild vereinzelt bis reichlich ,,Pneumoeysten" nachweisen liei~en. Die Ubertragung gelang kfinstlich yon mensehliehen Lungen, aber auch die nicht kfinstlieh infizierten Stallgenossen erkrankten. Nieht vorgesehiidigte Tiere konnten bisher nicht erfolgreich infiziert werden; und auch beim Menschen spielt der Vorschaden eine wesentliche t~olle, den PETEICSE~ in einem Lebersehaden und sonstigen Infektionen erblickt, GNEISS in einer unzureichenden Ernghrung sieht. Fest steht allerdings, dal~ auch tadellos gedeihende Frfihgeborene erkranken.Bezfiglich der ,,natfirlichen" Ubertragung in Kinderkliniken und Geb/~rabteilungen ist bisher noch manches unklar. Welehe l~olle spielen z. B. Arzte und Sehwestern bei der t)bertragung ? Zwar gibt es sieher zahlreiche abortiv erkrankende Kinder, gibt es aber auch Si~uglinge und Frfihgeborene des ersten Trimenons, die Ms gesunde Keimtrgger fungieten ? Halter der Infektionsstoff fest im Raum aul~erhalb des menschliehen
