ACE-Hemmer sind Antihypertonika mit Hauptindikationen Behandlung von Hypertonie und Herzinsuffizienz. Erstere beschreibt einen erhöhten Blutdruck, welcher als Hauptrisikofaktor für kardio-und zerebrovaskuläre Erkrankungen gilt. 1 Letztere ist definiert als Unvermögen des Herzens entweder im Ruhezustand oder unter Belastung die für den Blutstoffwechsel erforderliche Menge an Blut zur Zirkulation zu bringen. 2 Zur Behandlung beider Indikationen werden in jüngerer Vergangenheit primär sogenannte Vasodilatatoren, wozu sowohl die Alpharezeptorenblocker als auch die ACE-Hemmer zählen, eingesetzt. Sie schonen das Myokard in der Nachlast, wobei die ACE-Hemmer in der Langzeittherapie als am Bedeutsamsten einzuschätzen sind. 3 Das IQWiG definiert sie als ein Mittel der ersten Wahl. 4 ACE-Hemmer sind in der Monotherapie, oder der darauf folgenden Zweierkombinationstherapie indiziert. 5 Hypertonie tritt bei ca. jedem 2. Erwachsenen im Alter von 18-79 Jahren auf. Dabei nimmt die Prävalenz bei Männern von einem Wert der Altersgruppe der 20-29 jährigen von 23 % auf einen Wert von 84% der Gruppe der 70-79 jährigen zu. 6 Bluthochdruck liegt dabei in der Definition der WHO bei einem systolischen Wert von über 160 mmHg und einem diastolischen Wert von 95 mmHg vor. Die deutsche Hochdruckliga definiert die leichte Hypertonie bereits ab einem systolischen Wert von 140 mmHg. Es entstanden im Jahr 2004 durch die direkte Hypertonie Krankheitskosten in Höhe von 8.121 Mio. €, was einer Belastung von 100 € je Einwohner Deutschlands entspricht. 7 Die Gesamtausgaben lagen im Jahr 2006 bei 236 Mrd. €. 8 Die Herzinsuffizienz stellte die zweithäufigste Todesursache bei Frauen und die vierthäufigste bei Männern in Deutschland im Jahr 2004 dar. 9 In der Sekundärprävention kardiovaskuärer Ereignisse spielen Antihypertensiva eine bedeutende Rolle.

Anhand dieses Artikels wird zunächst eine Definition des Begriffes „Purpose Economy“ hergeleitet, um dann rechtliche Gestaltungsmöglichkeiten der Purpose Economy im Gesundheitswesen darzustellen.
