Technological and medical advances are increasing which means a growing number of adults are having surgical performances, usually older, higher risk, disabled and debilitated (Abreu, 2018;McCann et al., 2019). Open-heart surgery emerges when conservative ways of treatment are no longer a viable resource, it leads to acute stress, and it is associated with morbidity and mortality related to the underlying disease, surgical technical procedures and other factors (Abreu, 2018;McCann et al., 2019). Relying on evidence found through the years, we should not discuss whether implementing a pre-operative intervention in patients waiting for open-heart surgery is necessary, but rather how to implement it, making it more accessible to all (Ettema et al., 2014;McCann et al., 2019).Open-heart surgery patients are getting older; therefore, caring is increasingly more complex due to comorbid disease, frailty and psychosocial concerns (Abreu, 2018;McCann et al., 2019). It is estimated that in the next few years, the ageing of population will affect the perioperative patient care, and the number of surgical procedures will rise and consequently surgery waiting times (Abreu, 2018).
Contexto: A situação de doença crítica despoleta frequentemente uma crise familiar, com impacto na saúde mental. A família tem necessidades específicas associadas à incerteza da situação, ao risco de vida eminente, e ainda ao ambiente tecnológico desconhecido, sendo uma das necessidades mais emergentes a de comunicação. A comunicação terapêutica em enfermagem com a família permite diminuir o seu sofrimento, porque clarifica a compreensão da situação, fornece suporte emocional e reduz a ansiedade. Objetivo: Identificar as intervenções de enfermagem que otimizam a comunicação com a família da pessoa em situação crítica. Metodologia: Revisão integrativa da literatura, realizada através da pesquisa de trabalhos publicados e literatura cinzenta nas bases de dados CINAHL, Medline, PubMed, Cochrane Database of Systematic Reviews e ScienceDirect. Identificaram-se 16 artigos publicados entre 2014 e 2019. Resultados: Os resultados mostram a importância do enfermeiro na transmissão de informação atualizada de forma regular, usando uma linguagem acessível à compreensão do familiar; na promoção de conferências familiares; na demonstração de uma atitude empática, mantendo contato visual, tocando e escutando ativamente a família em sofrimento; no acolhimento da família no contexto, fornecendo informação escrita; na promoção da confiança no enfermeiro e na equipa e no apoio emocional, gerindo as expetativas e a esperança. Conclusão O estudo permitiu identificar estratégias de comunicação terapêutica com a família da pessoa em situação crítica, que exigem competência para o seu uso enquanto instrumento do cuidar, contribuindo para a saúde mental da família em crise.
