Objetivo. Apresentar um método para identificar áreas críticas relativas a doenças infecciosas e parasitárias selecionadas para fins de vigilância em saúde, analisando a sua associação a indicadores de pobreza no Brasil. Métodos. Foram mapeadas as taxas de incidência de dengue, doença de Chagas aguda, esquistossomose, hanseníase, hepatite A, leishmaniose tegumentar, leishmaniose visceral, leptospirose, malária e tuberculose. Foram realizadas análises para os anos de 2010 a 2017 a partir de um indicador síntese, calculado como a média dos coeficientes médios de incidência para cada agravo normalizada pela média e desvio padrão durante o período analisado. A estimativa da base populacional foi de 2014. Os coeficientes calculados foram estratificados para classificação dos municípios em criticidade muito alta, alta, média, baixa ou muito baixa conforme cada doença. Também foram selecionados indicadores de diferentes dimensões que expressassem desigualdades socioeconômicas e segregação espacial nos municípios brasileiros, sendo testada a sua associação às doenças em estudo. Resultados. O indicador mostrou que 40,5% dos municípios brasileiros apresentam alta criticidade, sobretudo nas regiões Norte, parte do Nordeste e Centro-Oeste. Os indicadores “proporção de pobreza”, “lixo no entorno”, “esgoto no entorno” e “famílias chefiadas por mulheres” podem aumentar a chance de a localidade apresentar maior criticidade para as doenças. O indicador “esgoto adequado” pode ser considerado potencial fator de proteção. Conclusões. A técnica utilizada foi adequada para orientar ações de vigilância no país e permite a articulação entre vigilâncias locais e demais setores para contornar os problemas de saúde causados por doenças infecciosas e parasitárias e fatores relacionados.

The main volatile organic compounds found at gasoline stations are benzene, toluene, ethylbenzene, and xylene isomers (BTEX). They cause several harmful effects on human health. Regulatory Norm 7 (1978) provides that, in Brazil, biological monitoring of toluene and xylene is carried out by measuring the urinary metabolites hippuric acid (HA) and methylhippuric acid (MHA), respectively. The objective of this study was to assess the exposure to toluene and xylene and to identify related signs and symptoms in gasoline station workers. A cross-sectional epidemiological study was conducted with workers occupationally exposed to fuels. These gasoline station workers were divided into two groups: 94 workers exposed mainly by inhalation (convenience store workers (CSWs)) and 181 workers exposed by inhalation and dermal route (filling station attendants (FSAs)). A comparison group was formed by 119 workers not occupationally exposed to fuels (office workers (OWs)). Workers exposed to fuels had higher average levels of these exposure biomarkers (HA and MHA), which were also higher in convenience store workers than in filling station attendants. In addition, individuals exposed to the solvents present in gasoline had altered mood/depression, cramps, dizziness, drowsiness, headaches, irritability/nervousness, weakness, weight loss, and other symptoms more frequently and had higher urinary levels of HA and MHA compared to the comparison group. Gasoline station workers showed high levels of HA and MHA, reflecting high occupational exposure to the solvents toluene and xylene present in gasoline, demonstrating that changes in the current legislation and in the work environment are necessary to ensure better health protection for these workers.
Introduction: Little is known about occupational agents andthe occurrence of leukemia and myelodysplastic syndromein Brazil. Objective: The aim of this study was to trace thesociodemographic and occupational profiles of patients withleukemia and myelodysplastic syndrome in two hospitals inthe city of Rio de Janeiro, Brazil. Methods: We carried out astudy of 229 cases from the period 2000 to 2006. Interviewswere conducted with patients to obtain information onfamily history, lifestyle risk factors, occupational history,residential history, and occupational and non-occupationalexposures by using a structured questionnaire. The frequencyand duration of occupational activities related to exposureamong participants were estimated. Each individual’s levelof exposure was classified as low, moderate, high or veryhigh. Results: The main exposures observed were to chemicalcleaning and disinfectant products, followed by gasolineand lubricating oil. Most of the exposures occurred at lowfrequencies. However, for benzene, solvents, thinners, andcoal combustion, the highest prevalence was observed formoderate exposures. Furthermore, it was observed that theexposure patterns of substances varied for each group, suggestingthat the intensity of exposure was determined byspecific types of substances. Conclusion: The monitoring ofexposure in occupations that deal with solvents is critical, inorder to reduce the incidence of leukemia. Since occupation isa preventable risk factor, strategies for primary prevention andhealth promotion in the workplace should be a top priorityin cancer prevention policies in Brazil.Keywords: Leukemia; Occupational Health, Myelodysplasticsyndromes, Epidemiology.
